Aim To study the activation of Nrf2 in-duced by baicalein ( BAI ) , and its protection against carbon tetrachloride ( CCl4 ) , ethanol and acetamino-phen ( APAP )-induced hepatotoxicity. Methods A reporter gene assay was conducted in human normal liver L-02 cells to detect the activation of transcription factor Nrf2 induced by baicalein. APAP ( 10 mmol · L-1 ) , CCl4 (10 mmol·L-1 ) and Ethanol (100 mmol · L-1 ) were used to induce hepatotoxicity in L-02 cells. After the pre-incubation with Baicalein (0, 1, 10, 25, 50, 100 μmol·L-1 ) for 15 min, cells were administrated with or without those above hepatotoxins. 48 h later, cell viability was detected by 3-(4, 5-dim-ethylthiazol-2-yl ) 2 , 5-diphenyltetrazolium bromide (MTT) method. Results Baicalein (25, 50 μmol· L-1 ) induced the activation of Nrf2 ( P <0. 01 , P <0. 05) in the reporter gene assay. As compared with control, three hepatotoxins ( APAP, CCl4 , Ethanol ) all decreased cell viability ( P<0. 01 ) , and baicalein significantly reversed such decreases in a concentra-tion-dependent manner ( P<0. 01 ) . Conclusion Ba-icalein can induce the activation of transcription factor Nrf2 , which is probably one of the mechanisms con-tributing to the protection of baicalein against hepato-toxins (APAP, CCl4, Ethanol)-induced hepatotoxici-ty.

Objective To study the clinical pathological features of Wegener's granulomatosis (WG) in middle-aged and elderly patients,and enhance understanding of this disease.Methods Totally 21 patients with WG (11 males,10 females,aged 45 to 76 years,mean age 58.1 years) in our hospial from February 1999 to July 2012 were selected.The clinical and pathological data of WG patients were retrospectively analyzed.34 biopsies including 2 autopsies from different organs were paraffin embedded and stained by hematoxylin and eosin and histochemistry.13 renal biopsies were all examined by immunofluorescence and electron microscope.Results The average time from the onset of clinical symptoms to the diagnosis was 5.3 months (from 24 days to 11.0 months).Eyes,nose and salivary glands were the most commonly involved parts at the beginning of Wegener's granulomatosis (52.4％,11 cases).The percentages of the skin,lung and renal involvement were 14.3％ (3 cases),81.0％ (17 cases) and 71.4％ (15 cases),respectively.Among 21 patients,18 patients were examined with anti-neutrophil cytoplasmic antibody (ANCA).c-ANCA was positive in 72.2 ％ patients (13 cases,13/18),p-ANCA was positive in 16.7％ patients (3 cases,3/18),and ANCA was negative in 11.1％ patients (2 cases,2/18).3 major pathological manifestations were observed:7 kinds of vasculitis including capillaritis,acute vasculitis,chronic vasculitis,fibrinoid necrosis in vasculitis,necrotizing granulomatous vasculitis,non-necrotizing granulomatous vasculitis and cicatricial vascular changes; 4 kinds of granulomatous inflammation including scattered giant cells,palisading histiocytes,poorly formed granulomas and microabscess surrounded by granulomatousinflammation;2 kinds of parenchymal necrosis including geographic necrosis and microabscess.13 kinds of histopathologic features in 3 major manifestations were found from 2 autopsies,but various kinds histopathologic features presented in small biopsy samples.Minor manifestations such as diffuse pulmonary hemorrhage were found at the periphery of WG.Conclusions The wide variation and broad spectrum of pathologic features can occur in WG.Vasculitis,granulomatous inflammation and parenchymal necrosis are the most important histopathological features.The correct diagnosis of WG requires careful correlation of pathology with complicated clinical features.

Objective To investigate the feasibility and safety of laparoscopic-assisted gastrectomy for distant gastric cancer.Methods All 18 patients with distant gastric cancer receiving laparoscopic-assisted gastrectomy were analyzed.Results Laparoscopic-assisted distal gastrectomy was performed successfully in all patients.The mean operation time was(291.33?19.61) min.The mean blood loss was(151.32?71.78) ml.The mean numbers of harvested lymph node were 14.57?3.11.The mean time of gastrointestinal function recovery was(3.46?0.93) d,the mean out of bed activity time was(1.75?0.45) d.All patients were followed up for 1-24 months,mean 11 months.No local recurrence,trocar implant or distant metastasis happened.Conclusion Laparoscopic-assisted gastrectomy is a feasible and safe surgical procedure combined with minimal trauma and fast recovery.

Objective To analyze the clinical features,curative effect and prognosis of extra-pulmonary infections by mycoplasma pneumoniae(MP).Methods ELISA was used to detect anti-MP IgM and IgG.Case histories of 226 patients,whose nasopharyneal lotione and respiratory secretions were positive of virus and bacteria respectively,was analysed retrospectively.Results One hundred and eighteen of the 226 (52.2%) cases suffered from extra-pulmonary infection.Of these infections 38.98%,33.89%,21.11% and 17.79% were found in digestive, urinary, cardiovascular systems and serous membrane respectively.All of the cases were improved after treatment with macrolides antibiotics.All cases were MP IgM positive,35.5% cases were IgG positive.Spatum MP positive rate was 32.2%.Positive rate of cold-agglutination test was 47.46%.Conclusions MP infection may cause many extra-pulmonary complications.When multi-organ infections can not be explained with bacterial and viral infections,MP infection should be considered.

OBJECTIVETo search all studies that had been published in the world with regarding to the effectiveness of the extent of intralesional curettage and wide excision for recurrence rate and complications and comparative functional outcomes in patients with giant cell tumours (GCT) of the distal radius and analyze them which were in high quality by means of Meta analysis, in order to give some evidences for the choice of method dealing with giant cell tumors GCT in surgery.

METHODSCochrane central register of controlled trials(Issue 8 2014), PubMed(1970-01-01/2013-01-01), Ovid (1970-01-01/2013- 01-01), Elsevier (1970-01-01/2013-01-01), CNKI (1970-01-01/2013-01-01) were searched. Including intralesional curettage and wide excision were performed to treat giant cell tumors (GCTs) of the distal radius in the literatures, selecting on meet eligibility in the standard literatures underwent strict quality assessment. The Meta-analysis was performed with software RevMan5.0 from the Cochrane collaboration. Additionally, the analysis checked the heterogeneity of data. The effectiveness of the extent of intralesional curettage and wide excision for recurrence rate and complication in patients with giant cell tumours of the distal radius were evaluated and Odds Ratio was calculated.

RESULTSSeven relevant articles were identified involving total 163 cases. Among them, 92 cases were intralesional curettage (PMMA, n = 54; bone graft, n = 33; no PMMA or bone grafts, n = 5) and 71 cases were wide excision. The patients in the intralesional curettage group had a higher recurrence rate [OR = 3.87, 95% CI (1.42, 10.53)],especially for Campanacci grade 3 GCTs [OR = 10.12, 95% CI (1.57, 65.27)], yet fewer major complications [OR = 0.13, 95% CI (0.04, 0.40)] than the wide excision group. The use of PMMA versus bone graft did not affect the recur- rence rate [OR = 0.96, 95% CI (0.26, 3.56)]. By selecting the system evaluation of MSTS, the VAS and dynamometer, the result showed that the intralesional curettage group was equivalent or preferable to wide excision in terms of function rehabilitation.

CONCLUSIONBased on data obtained from the limited number of studies available, intralesional curettage appears to be moreappropriate for the treatment of local lesions (Grade 1 and 2) than Grade 3 GCTs of the distal radius. Moreover, PMMA was not additionally effective as an adjuvant, the intralesional curettage group was found to be equivalent or preferable to wide excision in terms of function rehabilitation.

TCM incompatibility means that the combinations of some Chinese materia medica would produce or enhance side effects, or even reduce or damage medicine efficacy. Therefore, clinical incompatibility should be avoided. The core idea of TCM incompatibility is "Eighteen Clashes" and "Nineteen Incompatibilities". China Pharmacopoeia clearly states that "Eighteen Clashes" and "Nineteen Incompatibilities" belong to incompatibility. This article summarized the incompatibility in TCM patent medicine used in Guang'anmen Hospital of Chinese Medicine Academy of Sciences, with a purpose to provide references for clinical doctors and pharmacists.

Objective To evaluate the value of combined hepatectomy in radical resection for hilar cholangiocarinoma. Methods The clinical data and follow-up data of 67 patients of resection for hilar cholangiocarinoma in Henan Tumor Hospital from June 2005 to october 2008 were retrospectively analyzed. Results According to intraoperative exploration situation and bismuth types, tumor resection was combined performed with hepatectomy (n＝38)or non-hepatectomy (n＝29). The rate of R0 resection was 55.3% in hepatectomy group(n＝21) and 34.5% in non-hepatectomy group(n＝10), and the difference was significant（P＝0.024）. The incidence of complications were 39.5% in hepatectomy group(n＝15) and 13.4% in non-hepatectomy group(n＝4), and one patient with liver and kidney failure died in hospital. The 1, 3, 5 years of survival rate were 89.3%,53.6% and 32.1% respectively in R0 group (n＝31) and 69.7%,30% and 10% respectively in R1～R2 group(n＝36), there were significant differences in the postoperative survival rate between both groups（P＝0.018). The 1, 3, 5 years of survival rate were 81.8%,48.5% and 24.2% in hepatectomy group and 75%,32% and 16% in non-hepatectomy group respectively, and the differences were significant（P＝0.037). Conclusions Aggressive resection including combined hepatectomy for hilar cholangiocarcinoma can play an important role for curative effect and long term survival rate.

Objcetives To sparch for the change of cerebrospinal fluid ferritin (CSF-Ft) content and its clinical significance.Methods The 42 patients with acute lymphatic leukemia(ALL)diagnosed by bone marrow biopsy were divided into 3 groups. There were 14 cases in Ⅰ group [introduced therapy stage without central nervous system leukemia(CNSL)7, 24 cases in Ⅱgroup (complete remission stage without CNSL) and 18 cases in Ⅲ grotip (with CNSL). There were 17 patients with viral encephalitis in viral encephalitis group and 15 patients without central nervous system oisease in control group. The CSF-Ft and SFt were determined by radioimmunoassay.Results The CSF-Ft contents of Ⅰ、Ⅱ、Ⅲ 、viral encepbalitis and control groups are 7.03 ?2.21 ?g/L,6.75 ?1.94?g/L, 31.06 ? 8.85?g/L, 7.26?1.83?g/L and 6.52 ?1.57?g/L. The reversible covalent interaction between boronic acids and cis-diol-containing compounds provides unique affinity for recognition and separation of cis-diol-containing biomolecules such as glycoproteins and sugars. Herein, by using β-blockers and β-agonists as representative hydroxyethylamines, the interaction between phenylboronic acid and hydroxyethylamines was investigated through nuclear magnetic resonance ( NMR) and high performance liquid chromatography ( HPLC ) . The results showed that strong interaction between hydroxyethylamines and phenylboronic acid occurred at high pH value, while the interaction became much weaker and even disappeared at low pH value. This interaction was similar to boronate affinity interaction between boronic acids and cis-diol-containing compounds. However, unlike boronate affinity, the presence of an aprotic solvent disrupted the interaction. The above findings not only provided new insights for in-depth understanding boronate affinity interaction, but also paved the basis for the application of the interaction between boronic acid and hydroxyethylamines.
