1.A Case of Lymphangioma Circumscriptum of the Vulva.

We present a case of lymphangioma circumscriptum in a 17 year-old girl according to tihe clinical and histopathological findings. This case is unusual in that lesions developed on the vulva and thigh without preceding lymphedema. Our patient had a plaque of grouped vesicle-like papules resembling frog's apawn on The both labia majora of vulva and several scattered, skin tag like soft papules on the right upper thigh of theree years duration. Histopathologic findings showed variable sized, dilatated lymphatic channels lined by single layer of normal endothelial cells confined to the only upper dermis.

2.Levels of Urinary Neopterin in Patients with Lepromatous Leprosy and Patients with Erythema Nodosum Leprosum.

In lepromatous leprosy, it is generally believed that there is not only defective CMl specific for M. leprae, but also generalized impairment of CMI and in erythema nodosum leprosum, an immune complex-mediated pathogenesis as well cell mediated immune pathogenesis have been proposed. Neopterin is a pyrazinopyrirnidine compound derived from GTP, its raised excretion has been related to activation of T-lymphocyte/macrophage axis. A study was performed to evaluate generalized CMI status in the LL and ENL and to investigate a relationship between levels of urinary neopterin and disease activity. Urinary neopterin was measured by high pressure liquid chromatography in 25 healthy subjects, in 25 patients with LL and in 25 patients with ENL. The results were as follaws 1. Urinary Neopterin levels of patients with LL was 188.9+147.3umol/mol creatinine, which was higher than that of control group(144.8+40.4umol/mol creatinine)(p<0.01). 2. Urinary Neopterin levels of patients with ENL was 884.1+970.5umol/mol creatinine, which was higher than of control group, and patients with LL(p<0.01, p<0.01). 3. Serial measurement of urinary neopterin from 1 week to 13 weeks after treatment of ENL in 4 cases of ENL showed good correlation between urinary neopterin levels and disease activity. In summary, it thus appears that measurement of urine neopterin in leprosy provides generalized CMI status and reliable index for activity of disease.

3.Clinical Observation on the Renal Tuberculosis.

A clinical observation was made on 57 cases of renal tuberculosis who were admitted to the Department of Urology, Capital Armed Forces General Hospital during the period from January, 1977 to December, 1979. The following results were obtained. 1. The incidence of renal tuberculosig wag 7.3% of all urological in-patients and male to female ratio was 4.7:1. The most common age group was 21 to 40 years (77.2%). 2. The site of lesion showed right to left ratio of 42.1% to 38.6% and bilateral in 19,3% of cases. 3. Vesical irritability symptoms were more frequent than other symptoms and less frequent ones showed generalized symptoms in 7 cases (12.3%) and epididymitis in 6 cases (10.5%). 4. 20(35.1%) of 67 cages bad been associated with tuberculosis other than kidney or past history. Among the 20 cases, tuberculous epididymitis was found in 9 cases (15.8%), pulmonary tubercu1osis in 6 cases (10.5%) and prostatic tuberculosis in 2 cases (3.5%) in order 5. On the intravenous pyelogram, nonvisualization of the kidney was viewed in 22 cases (38.6%). 0ther findings were calyceal destruction in 19 cases (33. 3%) and delayed visualization in 14 cases (24.6%). The most commonly viewed ureteral and vesical abnormal findings were dilatation in 19 cases (33.3%) and reduction in bladder capacity in 18 cases(31.6%) 6. The 49 operations were performed in 37 cases of renal tuberculosis. In unilateral renal tuberculosis, nephrectomy was Jone in 21 cases (88.9%), ureteric-reimplantation in 2 cases and Boari-flap in 1 case. Conservative surgeries in bilateral renal tuberculosis were ureteric reimplantation in 4 cases, nephrostomy in 3 cases, cutaneous ureterostomy, Boari flap, ureteroileocystoplasty and calycoileoileostomy in 1 cases each other.

4.A Case of Sinus Histiocytosis with Massive Lymphadenopathy.

Sinus histiocytosis with massive lymphadenopathy(SHML) is a benign, generally selflimited pseudolymphomatous disease that typically appears with cervical massive lymphaclenopathy. Extranodal involvement including skin occurs in the 28% of the cases. We report a case af SHML in 51 year-old male who had several, prominent firm masses ranging from 1-10cm in the cervical, axillary, inguinal areas and multiple, plum colored nodules and plaques in the face, trunk for about 10 years. The histopathological findings of cervical lymph node, facial nodule showed dense heavy infiltration of large histiocytes with abundant pale eosinophilic cytoplasm in the subcapsular and medullary sinuses of lymph node and dermis of skin. No atypical cells suggesting malignancy is seen in the infiltrates. The patient had been treated with combination of prednisolone and vinblasstine, but he expired 1 month later.

5.A Case of Mucosal Melanoma of the Lip Metastasized to the Brain and Lung.

Mucasal melanoma is rare and it is one of the diseases of the worst prgnosis. We report a case of metastatic mucosal melanoma of the lip in 50-yearold male with a peanut sized, round, black colored macule on the lower lip, which was metaetasized to the brain and lung for 8 years. Histologically, typical findings of malignant melanoma showed. Chest P-A showing nodular high densities on the right upper and lower lobes and left lower lobe. Brain C,T. scan revealed round nodular densities on the right and left frontal and parietal lobes. In spite of chemical and radiation therapy, he was dead.

6.Effects of Several Autonomic Drugs on the Responses to Electrical Stimulation of the Detrusor Muscle Strips in Amyda Japonica.

A strip of the detrusor muscle of amyda japonica was provided in a bath which contained frog-Ringer's solution and the responses to electrical stimulation were observed by means of an isotonic myograph. The effects of several autonomic drugs on the responses to electrical stimulation of the strip were examined. The following results were obtained. 1. Electrical stimulation of the isolated detrusor muscle strip elicited contraction response and the response showed two peaks at 20 and 80 Hz between 1-200 Hz stimulation frequencies. 2. The responses to electrical stimulation of the detrusor muscle strip was completely abolished in the presence of tetrodotoxin and resumed after washing the bath fluids. 3. The responses to electrical stimulation of the detrusor muscle strip were significantly reduced in the presence of atropine, guanethidine, or regitine. 4. Clonidine did not affect the contraction response to electrical stimulation of the detrusor muscle strip. These results suggest that the detrusor muscle of Amyda japonica is innervated by both cholinergic and adrenergic nerves and it contains adrenergic alpha-receptors and that presynaptic alpha-adrenoceptors known to modify inhibit the release of neurotransmitters from the nerve endings are not involved.

8.A Clinical Study of 133 Patients with Rosacea.

BACKGROUND: A clinical study of rosacea has never been reported in Korea. OBJECTIVE: The purpose of this study was to elucidate the clinical observations of rosncea. METHODS: During a 5-year-period from Jan. 1990 to Dec. 1994, 133 patients were evaluated with regard to age, sex, location of skin lesions, clinical type, duration, and precipitating factors. RESULTS AND CONCLUSIONS: 1. Of the 7,787 cases amongst outpatients, 133 cases(1.7% ) were dignosed with rosacea. Of there 46 were male patients and 87 females, giving a sex ratio of 1:1.9 in favor of females. 2. The most common type of rosacea was vascular rosacea(60.2%) 3. The average duration of rosacea was 3.4 years. 4. Rosacea afflict persons aged 40-49 most frequently. 5. The predilection sites of the rosacea were : cheek(42.9%), nose(23.3%), entire face(11.3%), cheek & nose(9%), zygomatic area(6.8%), extra-facial area(3.8%), forehead(2.9%). 6, The aggravating factors of rosacea were : unknown cause(32.3%), coffee & tea(18%), alcohol(13.5%), emotional stress(10.5%), sun exposure(9.8%), multi-factoral(7.5%), drugs(steroids)4.6%).

9.Effect of Lysodren(R) on the Ultrastructural Changes in the Rat Adrenal Corex: Immunohistochemical staining for anti-ACTH antibody on the adenohypophysis.

The toxicity and adrenostatic effect of o,p'-DDD, a derivative of the insecticidal DDT, on the adrenal cortex were well known. It known that the toxicity was based on the blocking of steroid biopsynthesis when cholesterol was converted to pregnenolone. Lysodren(R) was also known to be capable of producing a regression of adrenocortical carcinoma and its metastases, and this drug became one of useful choice for the treatment of unoperable adrenocortical carcinomas. Recently, fine structural effect of o,p'-DDD on the adrenocortical carcinoma show that the mitochondria is the primary target organelle. o,p'-DDD was dissolved in corn oil and it was orally administered for 28 days to investigate the ultrastructural effects of zona fasciculata of rat adrenal cortex. The results obtained were as follow: 1) The body weight was decreased after feeding o,p'-DDD. 2) Light microscopic examination showed no remarkable change except increased fine lipid droplets of zona fasciculata in group I (o,p'-DDD 75 mg/kg feeding). Moderately increased intracytoplasmic lipid droplets and pyknotic nuclei bearing membrane indentations were seen in group II (o,p'-DDD 150 mg/kg feeding). Large sized lipid droplet aggregates, pyknotic nuclei with severe nuclear membrane indentations and karyorrhexis in focal area were evident in group III. 3) Immunohistochemical staining for ACTH in pituitary gland showed increasing number of ACTH secretory cell and increasing intensity of staining property according to the dosage of o,p'-DDD. 4) Ultrastructural examination showed increased intracytoplasmic lipid droplets and mild increased peroxisome. There was no remarkable ultrastructural changes in mitochondria in group I. Moderately increased lipid droplets and clusters formation, compressed mitochondria, partial disappearance of mitochondrial cristae, increased peroxisome and nuclear membrane indentations were seen in group II. In group III, nuclear membrane showed prominent indentation. Numberous cytoplasmic vacuolation, double membrane ring in mitochondria, disappearance of mitochondrial cristae, myelin figure formation in mitochondrial matrix, and fatty changes in mitochondrial matrix were seen. These findings showed that the primary target organelle of attack by o,p'-DDD on zona fasciculata of adrenal gland in rat is mitochondria and it was developed from double ring formation in mitochondrial matrix.
