2.Reconstruction of the Soft Tissue Defect Using Thoracodorsal Artery Perforator Skin Flap.

In the case of a soft tissue defect which requires thin & pliable tissues, the choice of donor site is limited due to flap bulkiness and donor site morbidity. To overcome these problems, a variety of perforator-based flaps such as paraspinous and parasacral perforator flap, deep inferior epigastric perforator flap, gluteal perforator flap, and thoracodorsal artery perforator flap have recently been introduced. We experienced 8 cases of soft tissue defects from December 1996 to March 1999 using the thoracodorsal artery perforator flap for reconstruction. We could elevate the cutaneous flap with preservation of the latissimus dorsi muscle flap only when it was based on one cutaneous perforator. Defatting procedure was possible for further thinning of skin flap. In one case, axillary defect after release of postburn scar contracture was repaired with island perforator flap and the other seven cases were repaired with free flap. The results were satisfactory. We believe the thoracodorsal artery perforator flap is useful for reconstruction of soft tissue defects which are large or under conditions requiring thin flap.

3.Relationship between the Measurement Values of the Digitial Dust Indicator and Personal Dust Sampler.

The digital dust indicator (Sibata P-5), one of the direct-reading instruments was evaluated for the respirable dust in the underground coal mine environments. As a reference, respirable dust was determined using three cyclones and/or impactors. All the tests were performed on aerosol in twenty underground coal mines. The coefficients of mass-relative concentration were 0.067+/-0.054 (Mean+/-Standard deviation) (range: 0.006-0.172). The relationship between relative concentration and temperature was not significant statistically. Also, the relationship of relative concentration and relative humidity was not significant. Mass concentration and relative concentration were 5.31+/-5.22 mg/m3 and 162+/-163 CPM (Mean+/-Standard deviation) respectively. The range of mass concentration was 1.22-22.69 mg/m3; relative concentration 16-628 CPM. The relationship of mass concentration and relative concentration was not significant in these ranges.

4.Reciprocal ST-segment depression in acute inferior myocardial infarction : Possible indicator of concomitant left anterior descending coronary artery stenosis.

BACKGROUND: Reciprocal ST-segment depression in precordial leads is a common finding in acute inferior myocardial infarction. The responsible mechanism and the significance of this finding, however, are still controversial. METHODS: Clinical characteristics, serial eletrocardiograms, angiographic findings of coronary artery and left ventricle were reviewed in 33 patients with acute inferior myocardial infarction. Reciprocal ST-segment depression was defined as ST-segment depression > or =1.0mm in two or more adjacent chest leads, I and aVL in patients with acute inferior myocardial infarction showing ST-segment elevation in II, III, aVF. Coronary angiography and left ventriculography were performed 15,2+/-16.9 hours after arrival. RESULTS: Eleven patients did not have reciprocal ST-segment depression(group A) and 22 patients had reciprocal ST-segment depression(group B). There was no significant difference in the demographic data of the patients except age and peak CK-MB, which were significantly higher in group B than group A. Left anterior descending coronary artery(LAD) stenosis was significantly more frequent in group B than group A(54.5% vs 18.2%, p<0.05). However the distribution of left ventricular regional wall motion abnormality and global ejection fraction showed no difference between two groups. In addition, there was no difference in in-hospital complications. CONCLUSIONS: These results suggest that reciprocal ST-segment depression in acute inferior myocardial infarction can be explained by anterior ischemia due to concomitant LAD stenosis in some cases, but its clinical significance is limited at least in terms of in-hospital complications.

5.Successful Management of Intractable Coronary Spasm by a Coronary Stent.

Variant angina is characterized by episodic angina due to spasm of an epicardial coronary artery at rest and concomitant transient ST elevation on electrocardiogram. While long-term survival of coronary spasm is usually excellent, but serious complications can be developed such as disabling pain, myocardial infarction, ventricular tachyarrhythmias, atrioventricular block and sudden cardiac death. We experienced 40 year-old man with intractable chest pain due to coronary artery spasm, who suffered from ventricular fibrillation and acute anterior myocardial infarction at the first admission. The patient underwent coronary angiogram, which revealed spontaneous focal spasm at the proximal left anterior descending coronary artery (LAD). He was treated by the combination of nitrate and calcium channel blocker. However, he complained of severe chest pain despite intensive medical therapy and visited emergency room 5 times during 8-month follow-up. We performed ergonovine coronary angiogram and intracoronary ultrasound, which revealed focal spasm at same site of proximal LAD with small amount of localized eccentric atheromatous plaque. Thus we placed coronary artery stent (3.0 x 24 mm GFX stent) at proximal LAD and his symptom was resolved after stenting. We performed follow-up coronary angiogram at 9 months after stenting and stent was patent without any stent recoil and in-stent restenosis.

6.Clinical Efficacy of Trimetazidine(Vastinan(R)) in the Treatment of Stable Angina.

BACKGROUND: Trimetazidine(Vastinan(R)) is a new antianginal agent of different action mechanism specifically targeted at the metabolic cellular consequences of myocardial ischemia. The clinical efficacy of the Trimetazidine in angina pectoris is still to be defined. METHOD: To determine the antianginal effect of trimetazidine in the treatment of ischemic heart disease, 15 patients with stable angina(12 male, 3 female, mean age : 59.3 years) were studied. In 6 cases as a single agent and in 9 cases as an additive regimen to conventional antianginal medications. Trimetazidine(20mg 3 times daily) was given for 30 days or more to evaluate the clinical effect. Graded exercise tests were carried out before the trial of Trimetazidine and on the 30th day of the treatment period. RESULTS: 1) The number of episodes of anginal attacks decreased from 4.2+/-2.7 to 2.0+/-0.5 a week(p<0.05) after treatment with Trimetazidine. 2) Trimetazidine also significantly increased the duration of total exercise from 12.1+/-4.7 min to 14.5+/-3.3 min(p<0.05), and the time to 1mm ST segment depression from 7.7+/-5.9 min to 11.7+/-5.2 min(p<0.05) on treadmill exercise by modified Bruce protocol. 3) Total workload (METs) and rate pressure double product(heart rate x systolic blood pressure) slightly increased, but the differences were not significant statistically. 4) No serious clinical side effects were observed during the treatment. CONCLUSION: These results suggest that the Trimetazidine is an effective and safe as an antianginal drug in the treatment of stable angina patients as a single agent and as an additive regimen when the patients are refractory to conventional drugs.

7.A Case of Congential Atresia of Left Main Coronary Ostium.

Congenital atresia of left main coronary ostium is a rare congenital coronary anomaly. This anomaly was detected during elective coronary angiogram in a 40 year-old female patient with chest pain and exertional dyspnea. Treadmill exercise test demonstrated 2 mm upslope depression of ST segment at stage I and the test was terminated due to chest pain. Myocardial stress SPECT using 201 Tl-dipyridamole showed reversible perfusion defects at anterior, apex and lateral wall. It was impossible to select left coronary artery ostium and right coronary angiogram revealed 30% eccentric stenosis at proximal right coronary artery with grade 3 collateral flow to left anterior descending and circumflex arteries. Operative finding revealed totally occluded left coronary ostium with membrane-like, non-atheromatous tissue similar to aortic wall. The patient was successfully treated with coronary artery bypass grafts (CABG) using left internal mammary artery and great saphenous vein. She underwent follow-up coronary angiogram, which revealed patent grafts, at one year after CABG and no cardiovascular event was observed on 5-year clinical follow-up.

8.Arthroscopic Bioabsorbable Screw Fixation of Unstable Osteochondritis Dissecans in Adolescents: Clinical Results, Magnetic Resonance Imaging, and Second-Look Arthroscopic Findings.

BACKGROUND: This study aimed to evaluate the clinical and radiological outcomes of arthroscopic bioabsorbable screw fixation in osteochondritis dissecans (OCD) in adolescent patients with unstable lesions causing pain. METHODS: The study included 11 patients (10 males and 1 female) with OCD who underwent arthroscopic bioabsorbable screw fixation between July 2007 and February 2014 and were available for follow-up for more than 12 months. The mean age at diagnosis was 16.3 years (range, 11 to 19 years), and the average follow-up period was 51 months (range, 12 to 91 months). Clinical results were evaluated using the Knee Injury and Osteoarthritis Outcome Score (KOOS), Lysholm knee score, and International Knee Documentation Committee (IKDC) score measured before surgery and at follow-up. Functional evaluation was made using the Tegner activity scale. Magnetic resonance imaging (MRI) and second-look arthroscopy were performed at the 12-month follow-up. RESULTS: Between the preoperative assessment and follow-up, improvements were seen in the KOOS (range, 44.9 to 88.1), Lysholm knee score (range, 32.6 to 82.8), and IKDC score (range, 40.8 to 85.6). The Tegner activity scale also improved from 2.8 to 6.1. Based on postoperative MRI, there were eight Dipaola grade I cases and three grade II cases. No complications due to fixation failure developed in any case. Second-look arthroscopy at 12 months postoperatively revealed that the lesion was covered with cartilage in all cases. CONCLUSIONS: For unstable OCD lesions causing pain in adolescents, arthroscopic bioabsorbable screw fixation provided favorable outcomes with reduced pain and restoration of movement. Therefore, it should be considered as an effective treatment for OCD.

9.Tuberous Sclerosis.

A case of the tuberous sclerosis, 14-years old boy, was observed. In his father and sister, the cerebral tubers were noted by brain computerized tomography. This disease has been reported occasionally in dermatologic and psychiatric department because of adenoma sebaceum, mental retardation and epilepsy respectively, but it has not been reported at neurosurgical department in Korea. The histopathology of the brain lesion showed atypical cell(tuberous sclerosis cell), gliosis and nonspecific calcification which confirmed the diagnosis of tuberous sclerosis.

10.Concentrations of lead, iron and zinc in blood of coal workers' pneumoconiosis patients.

Lead, iron, and zinc concentrations in whole blood were determined by atomic absorption spectrophotometry, using a simple one-step dilution procedure, which were measured in 3 groups, 98 officers unexposed to dust or to metal, 58 coal miners without pneumoconiosis, and 113 coal workers' pneumoconiosis (CWP) patients. The results were as follows; 1. The precisions (C. V.%) of lead, iron, and zinc in blood were 12.65+/-6.95%, 1.47+/-1.25% and 6.35+/-3.34%, respectively. 2. Lead and zinc in blood showed the log-normal distribution unlike iron in blood which showed normal distribution. 3. Lead, iron, and zinc concentrations in blood of 3 groups were follows. There was significant difference of concentration far zinc in blood by groups statistically. 4. The difference of lead, iron, and zinc concentrations in blood was not significant (p>0.05) by profusion on chest radiographs.
