Authors report a case of intrapetrosal carotid aneurysm which was successfully treated with Debrun's detachable balloon technique. The patients was a 21 year-old female with headache, vertigo and left facial palsy in whom aneurysm arising from the intrapetrosal segment of internal carotid artery was demonstrated.

3.The Relationship between Coping Mechanisms and Psychological Symptoms in Vitiligo Patients.

BACKGROUND: Vitiligo is an acquired disorder which causes body disfiguring and may provoke emotional stress, functional impairment, and psychiatric symptoms. OBJECTIVE: (1) To assess whether the psychiatric symptoms are more frequent in vitiligo patients than control patients. (2) To investigate whether the vitiligo patients with more severe psychiatric symptoms use more negative and passive coping mechanisms than patients with less severe psychiatric symptoms. To carry out this study we evaluated psychiatric symptoms, problems in daily activities, and the relation-ship between coping mechanisms and psychiatric symptoms. METHODS: A clinical study was done on 150 vitiligo patients. Other dermatology patients were used as a control group. Both groups had visited the dermatology clinic of the Severance hospital. Questionaires regarding age, sex, severity of disease, duration of disease, SCL-90(symptom check list 90), the coping list of Weisman, and items for adaptability of daily activity were recorded. Results : Male vitiligo patients showed more psychological symptoms than female patients. Vitiligo patients had many more difficulties in daily activities than the control patients. The patients who had more severe psychiatric symptoms used more negative and passive coping mechanisms than less severe patients. Conclusion : Vitiligo provoked psychiatric symptoms and difficulties in daily activities. Psychiatrically more severe patients used negative and passive coping mechanisms. Comprehensive dermatologic and psychiatric treatment may be required in the treatment of vitiligo.

4.Aneurysms of the Proximal Anterior Cerebral Artery.

The authors report four cases of aneurysm of the anterior cerebral artery proximal to the anterior communicating artery(A1 segment). In three of these cases, the aneurysms arose from the distal A1 segment. In one case the aneurysm arose from the proximal A1 segment. The aneurysms were saccular in three cases and fusiform in a case. Hypoplasia of A1 segment was not present in any of these cases. Three saccular aneurysms were clipped and a fusiform aneurysm was trapped. Prognosis was good in two cases, poor in other two cases.

5.The Therapeutic Effect of Nizofenone in the Acute Stage Following Subarachnoid Hemorrhage.

We administered new imidazole dervative, nizofenone, to 15 cases in the acute stage following subarachnoid hemorrhage between December, 1990 and March, 1991 to determine its therapeutic value of ischemic protection. The appearance of delayed ischemic deficit was not prevented, but its prognostic adverse effect was ameliorated. The ability to perform the normal activity was 67% in the group which did not show the delayed ischemi deficit. And the ability to perform the normal activity was also 67% in the group whih showed the delayed ischemic deficit. No significant side effect was seen in the end of its administration. But in 6 cases, the administration was stopped because of deterioration of patient' consciousness level despite its administration. No therapeutic value was observed in the poor grade(Hunt and Hess grade III-IV) patients. The ability to perform the normal activity was still 29% in the poor grade group. The mortality was 13% despite nizofenone. In this clinical trial, we concluded that nizofenone had the ameliorating effect to the adverse effect of the delayed ischemic deficit in the good grade(Hunt and Hess grade I-II) patients. But the effect to the poor grade patients was not observed. So large scaled controlld study would be necessary to verify its good therapeutic effect to the whole population of subarachnoid hemorrhage patients.

6.Intradural-Extramedullary Spinal Cavernous Angioma as a Cause of Subarachnoid Hemorrhage.

Two cases of intradural extramedullary cavernous angiomas are represented. The special features of these cases are noted: the acute clinical onset due to the manifestation as bleeding from this lesion rather than compression of spinal cord or spinal nerve root is emphasized. Both of them showed the symptom of subarachnoid hemorrhage with sudden headache and neck stiffness. In one case, a 33-year-old man complained of a sudden low back pain and the lesion is located at lumbar level, and in another case, a 61-year-old man complained of a recurrent headache with the lesion at cervical level. Magnetic resonance imaging (MRI) of spinal cord was diagnostic in both of them. Both of them exhibited characteristic gross and microscopic features indicating hemorrhage. The diagnostic, therapeutic aspects of cavernous angioma and a rare but possible cause of subarachnoid hemorrhage from cavernous angioma are discussed.

7.Comparison of Embryonic Developmental Capacity by different Co-culture Time of Oocytes in IVF-ET Cycles.

OBJECTIVE: To evaluate whether co-culture of oocytes on vero cell monolayers from Day 0 (Day 0 group) after egg retrieval results in an increase in developmental capacity such as fertilization rate, embryo quality, blastulation and clinical pregnancy rate compared with co-culture of oocytes from Day 1 (Day 1 group). METHODS: Sperms were treated with Hams F-10 supplemented with 10% human follicular fluid (hFF). Vero cells for co-culture were prepared in TCM-199 with 10% FBS. Oocytes were co-cultured from Day 0 and fertilized oocytes were co-cultured from Day 1 on vero cell monolayers in DMEM with 10% and 20% hFF, respectively after egg retrieval. On day 1, 2 and 5, fertilization rate and grade of embryos and blastocysts were evaluated. Results (fertilization rate, cleavage rate, grade of embryos and blastocysts and pregnancy rate) were considered statistically significant when p value was less than 0.05 using t-test and ci2. RESULTS: In sibling oocytes of same cycles, no differences were found in fertilization rate (94.6 vs. 91.4%), cleavage rates (94.6 vs. 91.4%), embryo grade (on day 2 and 3) and blastulation (65.6 vs. 57.0%) and their grade. In different oocytes of different cycles (patients), no differences were found in fertilization (79.8 vs. 78.3%), cleavage rates (77.7 vs. 76.4%) and blastulation (56.0 vs. 45.3%), but pregnancy rate was higher in the Day 0 group than in the Day 1 group (60.0 vs. 42.9%). CONCLUSIONS: This study revealed that the embryonic development capacities were not affected by the different co-culture time in the sibling oocytes of same cycles. Although no statistical significance, because of small size of study, there was a trend for higher pregnancy rates in Day 0 group compared to Day 1 group in different oocytes of different cycles.

8.Angiolymphoid Hyperplasia with Eosinophilia of Distal Arm.

Angiolymphoid hyperplasia with eosinophilia (ALHE) and Kimura's disease were classified as same disease in the past. Since there are many different clinical and histopathological characteristics that warranted their distinction, they are classified as different disease now. Six cases of Kimura's disease in upper extremity have been reported in Korean literature but ALHE in upper extremity has not been reported yet. We experienced a case of surgically treated ALHE in the upper arm and report this case with review of literature.

9.Changes in Glucose and Cholesterol Level in Schizophrenic Patients Treated with Atypical Antipsychotics or with Valproate Combination.

OBJECTIVE: The association of obesity, diabetes mellitus, drug compliance, and body image with use of antipsychotic medication has been documented in many studies. We aimed to assess the effects of antipsychotics and sodium valproate combination therapy on glucose, cholesterol level and weight in schizophrenic patients. METHODS: Chronic schizophrenic patients were grouped according to drug combination: risperidone, olanzapine, haloperidol, risperidone+valproic acid (VP), olanzapine+VP, haloperidol+VP. Planned assessment of included glucose and cholesterol, which were collected at baseline and at the end of the 12 weeks later. RESULTS: There were significantly increased glucose and cholesterol level, in olanzapine+VP group, but not in risperidone group. Cholesterol levels were not increased at the end of 12 weeks for haloperidol and risperidone group. Glucose and cholesterol levels had no significant association with weight gain. CONCLUSION: Changes in glucose and cholesterol level, weight, and body mass index associated with antipsychotics including sodium valproate combination were reported in our report. In the future, we need follow up study and various studies about other combination drug and metabolic symptoms.

10.Ultrastructural Study on Microtumor Spheroids from Human Malignant Glioma Specimens Maintained in Organotypic Culture.

The Present study investigated not only the feasibility of organotypic spheroid culture system taken from human malignant gliomas, but also the similarities and differences between surgical specimens and cultured spheroids using light microscopy, electron microscopy, and flow cytometric examination. Surgically resected tumor specimens from eighteen human malignant gliomas were minced and explanted into agarose-coated culture wells. After three to five days, these microtumor fragments emerged as spheroids and could be maintained as organotypic spheroids for more than eight weeks. Measurements of the spheroids showed that they decreased during the initial two to three weeks and afterwards remained unaltered over a specific period of time. This growth pattern of the spheroids was consistent with the condition of tumors in vivo suggesting the linkage of cell proliferation and loss. Light microscopic and electron microscopic studies of the spheroids demonstrated that morphological structures were similar to those of the original tumor tissue in vivo and histopathologic characteristics of the original tumor were maintained over a long culture period. The spheroids contained connective tissues, blood vessels, macrophages, and neutrophils maintaining a three-dimensional architectural resemblance to the original tumors. Of three pairs of the surgical and spheroid specimen examined by the flow cytometry, one showed a change of ploidy pattern and two contained increased fractions of proliferating cells. It is concluded that this microtumor spheroid system can maintain the characteristics of the original tumors, and may serve as an alternative to the in vivo xenograft model for the research of brain tumor biology, invasion and immunology while providing a valuable technique for the evaluation of new therapies, such as biological response modifiers.
