2.A Clinical Analysis of 52 Cases of Spinal Cord Tumors.

The authors analyzed 52 cases of spinal cord tumor who had been operated upon at the department of neurosurgery of the Koryo General Hospital from 1983 to 1992. The results were summerized as followings: 1) The spinal cord tumor was most common in the 6th decade of age(52 cases, 28.8%) and the ratio of male to female was 1.26:1. 2) The pathologic diagnosis was neurogenic tumor in 25 cases(48.1%), metastatic tumor in 12 cases(23.1%), glial tumor in 6 cases(11.5%), and meningeal tumor in 3 case(5.7%) in order frequency. 3) The tumors were located most frequently in the thoracic area(19 cases, 36.5%) and in the intradural extramedullary space(28 cases, 53.9%). 4) The most common initial presentation was pain(27 cases, 51.9%) and on admission 14 cases(26.9%) showed paraparesis. 5) The rate of abnormality of the plain radiologic study was 59.6% and the most common positive finding was the pedicle erosion(15 cases, 28.8%). 6) The computed tomographic scanning with water soluble contrast media was useful in planning of operation to provide the exact relationship between the tumor, spinal cord and location of dura. 7) The total removal was possible in 27 cases(51.9%). 8) The follow-up results after 19.4 months of the mean follow-up duration in 46 cases were recovery in 29 cases(63.0%), improvement in 12 cases(26.0%), stationary state in 3 cases(6.5%), worse in 2 cases(4.3%). It was excellent particulary in cases of intradural-extramedullary tumor. 9) The incidence of metastatic tumor was 23.1%(12 cases) and the lung was most common primary focus. The follow-up result of metastatic tumor was poor than the other tumors.

3.Changes of Tear Film and Ocular Surface with Time after Penetrating Keratoplasty.

PURPOSE: To evaluate the parameters of tear function and ocular surface changes with time course after penetrating keratoplasty (PKP). METHODS: Corneal sensitivity test (CST), tear film break-up time (BUT), basal tear secretion test (BST), tear clearance test (Tcr), fluorescein staining and conjunctival impression cytology were evaluated in 21 eyes of 21 patients who underwent PKP at 1, 3 and 6 months postoperatively. RESULTS: CST (p=0.02), BST (p=0.04), fluorescein scoring (p=0.03), and goblet cell density (p=0.03) showed improving tendency during 6 month after surgery, but BUT (p=0.11), Tcr (p=0.65) and squamous metaplasia (p=0.20) did not change significantly. One month after surgery, CST (p<0.01), BUT (p=0.01), BST (p<0.01), and goblet cell density (p=0.01) were lower, while fluorescein staining score (p<0.01) and the grade of squamous metaplasia (p=0.03) were higher in PKP group. Six months after surgery, CST (p<0.01) and goblet cell density (p=0.04) were lower, and fluorescein staining score (p<0.01) was higher in PKP group. CONCLUSIONS: Tear film and ocular surface are compromised after PKP and improved gradually during postoperative 6 months. However, these parameters were not recovered completely to the normal level.

4.Klinefelter's Syndrome.

Two cases revealed findings of characteristics of Kiinefelter's Syndrome such as small testes, atrophy of seminiferous tubules, azoospermia, increased urinary gonadotropin, decreased urinary 17 Ketosteroid, positive Sex Chromatin, gynecomstia and impotence.

5.Five-Year Follow-up Results of Refractive Accommodative Esotropia.

PURPOSE: To evaluate the clinical characteristics of refractive accommodative esotropia followed up for 5 years. METHODS: Forty-three subjects with refractive accommodative esotropia, who had been followed up for at least 5 years were included in this study. Age and sex distribution, age at onset and the interval between onset of esotropia and initial visit were evaluated. Changes of the angle of deviation and the amount of hyperopia during follow-up period were analyzed. RESULTS: The initial amount of hyperopia of 4.42+/-3.27D was decreased to 3.84+/-3.46D by the last exam. The average rate of hyperopia decrease per year was 0.12 diopters. The average angles of deviation at the initial and final exam were 31.74+/-13.72PD and 30.11+/-14.27PD, respectively. Among the 43 subjects who participated in this study, decompensation occurred in 2 patients in the third year and 1 patient in the fourth year. CONCLUSIONS: There is a tendency for the amount of hyperopia to decrease over time. Further, deviation may deteriorate over time as well. Thus, long term follow-up management is necessary to control deviation in patients with refractive accommodative esotropia.

7.Effects of IFN - gamma on Spheroid and Raft Culture of Squamous Cell Carcinoma of the Head and Neck.

PURPOSE: To establish new in vitro model systems that better reflect in vivo condition, multicellular tumor spheroids(MTS) and raft culture were developed using cell lines of squamous cell carcinoma(SCCHN) of the head and neck. In these 3-dimensional systems, the expression of cell surface molecules which are important for modulation of physiology of tumor cells were studied with or without the treatment of interferon(IFN)-gamma. MATERIALS AND METHODS: Four SCCHN cell lines were used for MTS and raft culture. The effects of interferon-gamma on SCCHN cells were examined by immunohistochemistry. RESULTS: All cell lines formed MTS, but only Tu-138 showed a good stratification at the air-liquid interface in the raft culture system. Immunohistochemical studies of MTS using monoclonal antibodies revealed a strong staining for MHC class I, no staining for MHC-DR, a weak patch expression of ICAM-1 and a central strong staining for integrin a 6. Staining patterns were similar for the raft cultures except integrin a 6(intense full-thickness positivity). In both systems, IFN-gamma enhanced the expression of MHC-DR and ICAM-1. No significant change was found in the expression of MHC class I and integrin a 6. CONCLUSIONS: MTS and raft culture system were established successfully from the SCCHN cell lines. IFN-gamma can modulate the surface molecules of tumor cells in the 3-dimensional culture systems.

8.Seoul Teacher's Satisfaction with and Needs for Health Examination and Health Promotion Programs.

PURPOSE: The purpose of this study was to investigate Seoul teacher's satisfaction with and needs for health examination and health promotion programs. METHODS: The participants were 3,186 teachers in Seoul. Data were collected through an on-line survey system from November 16 to December 7, 2008. Collected data were analyzed by descriptive statistics, t-test, ANOVA, and Scheffe's test using PASW 18.0 program. RESULTS: Teacher's satisfaction with teacher health examination was low and needs for detailed tests of cancer and vocal cord disorders were high. Teacher's satisfaction with teacher health promotion programs was low and needs for stress management, exercise and vocal cord protection program were high. CONCLUSION: Vocal cord disorders should be included in teacher health examination and vocal cord protection program should be provided for teacher's health promotion. Stress management and exercise program should also be provided for teacher's health promotion.

9.A Case of Small Cell Carcinoma in the Stomach.

Primary extrapulmonary small cell carcinoma(SCC) is appearing with increased frequency in the literature. These tumors have been described in the esophagus, stomach, pancreas, larynx, hypopharynx, salivary gland, nasal cavity & paranasal sinus, thymus, small & large bowel, uterine cervix, endometrium, breast, prostate, urinary bladder and skin. Small cell carcinoma of the stomach is extremely rare and a total 9 cases have been reported in the English literature. Like SCC in the lung, SCC in the alimentary tract has a aggresive behavior and prognosis of the patient is poor. We are reported a case of advanced gastric cancer diagnosed as small cell type by endoscopic biopsy. Grossly, the lesion of small cell carcinoma of the stomach, located in the upper body and gastric angle, is ulcerated with irregular margin and dirty surface covered with blodd clots and exudates.
