We report herein a case of multiple glomus tumors on the scrotum in a 14-year-old boy. The lesions were present at birth and had gradually increased in size and number. Pea to thumb tip sized, dark colored nodules without pain sensation were confined to the scrotum. Histopathologically, there were many dilated and irregular vascular spaces lined by a single layer of flattened endothelial cells in the dermis. In the periphery of the endothelial cells, there are a few layers of glomus cells. No capsule was seen. Total excision of the tumors was performed. No recurrence was observed for an year after surgical excision.

2.Two Cases of Malignant Histiocytosis.

Two cases of malignant histiocytosis, in which skin involvement was a prominent finding at initial clinical presentation, are reported. 1nteresting histopathologic findings, including Langerhans granule in one case and numerous eosinophils in another, were observed. In one case remission was achieved with BACOP chemotherapy (Bleomycin, Adriamycin, Cyclophosphamide, Vincristine, and Prednisone). In the other case, the patient died nine months after the onset of the disease without any treatment.

3.Adenomatoid Mesothelioma of the Epididymis: A case report.

Adenomatoid tumors are well-recognized neoplasms generally to be of mesothelial derivation. We experienced a case of an adenomatoid tumor of the tail of the epididymis in a 56-year-old male. Grossly the tumor was firm and whitish gray, and microscopically it consisted of glandular, cord-like, microcystic structures which were lined by flattened endothelial like to plump cuboidal cells. Immunohistochemical stains whowed positivity for keratin and negativity for facter VIII related antigen and carcinoembryonic antigen. Ultrastructually, there was many long microvilli projecting into the glandular lumina and intracytoplasmic luminal spaces, desmosomes, and prominent cytoplasmic tonofilaments. Those findings strongly support the mesothelial origin of the adenomatoid tumor especially in the glandular type. It also lead us to suggest that the term adenomatoid tumor should be remain in use for light microscopic diagnosis, and that the term adenomatoid mesothelioma should be applied when the mesothelial nature of an adenomatoid tumor is proven by electron microscopy and immunohistochemical stains.
4.A Clinical Study of Localized Scleroderma.

BACKGROUND: There has been an increase in incidence of localized scleroderma. However observations on the clinical aspects ot localized scleroderma have been scarce in Korea. OBJECTIVE: The purpose of this study was to elucidate the clinical observations and associated diseases of localized scleroderma METHODS: During a 10-year-period from Jan. 1984 to Dec. 1993, 61 patients were evaluated in regard to age incidence, sex ratio, clinical type, location and distribution of plaque and linear type, and associated conditions. RESULTS: l. Age distribution from 4 to 61 years with mean of 27.6 years. Most patients were between 11 to 30 years of age. 2. Sex distribution of male 14 female 47 with 3: 1 ratio in favor of female. 3. No difference in right and left involvement. 4. Plaque and linear forms were predominant. 5. The plaque form was distributed evenly while the linear form involved mainly the head and neck. The linear form and generalized form appeared most frequently in developement of associated conditions. 6. ANA test results were positive in 14 patients and most of them were of the speckled pattern. 7. Of the 41 patients who were treated with d-penicillamine, 26 patients showed improvement of their lesions.

5.A Cses of Total Occlusion of the Left Main Coronary Artery.

A patient had total occlusion of the left main coronary artery that was proved by coronary arteriography. Patients with total occlusion of the left main coronary artery have a varying clinical presentation and may have prolonged survival. In patients with good collaterals, left ventricular function may be preserved. This report reveiws the clinical and angiographic findings of a patient with occlusion of the left main coronary artery with symptoms of unstable angina pectoris but without congestive heart failure or EKG evidence of myocardial infarction.

6.Clinical Study of Borderline Malignant Tumor of Ovary.

Borderline malignant ovarian tumor are characterized as neoplasrns exhibiting cellular priliferative changes greater than those seen in the benign form, but not showing destnictive invasion of the ovarian strama. We reviewed 14 cases of borderline ovarian tumor who had been diagnosed and treated at Kyung Pook university hospital from 1987 to 1992. We nnalysed the clinical features, treatment and prognosis which were compared to 24 cases of the epithelial ovarian cancer at the same period The most common histological thpe of boderline tumnr was mucinous type (1 l cases, 79%), the next one was serous type (3 cases, 21%). In FIGO stage of Borderline tumor, 11 caseS weve stage I a, 1 with stage II c, 1 with stage gb and 1 with stage lV, and the FIGO stage of 24 cases of ovarian cancer was 8 cases with stage I a, 1 with stage I b, 2 with stage I c, 1 with II c, 2 with stage III a, 1 with stage III b, 7 with stage III c and 2 with stage 1V. The mean age of borderline and malignant tumor was 37.1 and 47.6 years.. 3 cases were being pregnant on the operatian. In the 14 cases of borderline turnor, 7 cases were managed by conservative operation, but 1 case recurred 4 years later so extirpative operation and pastoperative chemotherapy were given. The other 4 cases were managed by extirpative operation but postoperative chemotherapy was given in only 1 patient. Follow up ranged from 7 to 72 months. 12 cases were alive. 2 cases died, one was stage III b, the other was atage IV. This atudy suggeat, that it is neceasary to follow up carefully even the early stage of botderline tumor and manage as oyarian carcinama in the advanced stage.

7.The basal electrical skin resistance of acupuncture points in normal subjects.

The inhibitory component of the skin against given electrical current, also called as the electrical skin resistance, is subject to change in response to many factors, especially pain. In order to find out more definite relationship between pain and skin resistance, one should make measurement in the state devoid of any external disturbing stimuli to get the "basal skin resistance (BSR)", which is known to be different from point to point on body surface. Also, the "active points" have more consistent BSR characters than other points and most of them share same locations with "acupuncture points" which is easy to localize accurately and repeatedly in normal subject. Therefore, the comparison of BSR of certain acupuncture points of normal subject and pain-suffering subjects is expected to be able to figure out any pain-induced BSR changes. Our measurement at 16 acupuncture points (asymmetrical 8 pairs) of 10 normal subjects showed inconsistently asymmetrical distribution of the BSR values with significant order among themselves including left SP (spleen)-6 at their lowest position, but neither the measuring system itself nor any of the 16 points was statistically reliable enough for diagnostic purposes. So the measuring device needs to be improved with after evolution, and more acupuncture points need to be explored to complete our pain-related BSR map. In addition, the meaning of the above BSR distribution pattern is waiting to be explained through such efforts.

8.Patterns of Cytokine mRNA Expression of Various Endometriosis Lesions.

OBJECTIVE: The pathogenesis of endometriosis is generally accepted that retrograde menstruation and alterations in the local pelvic immune environment. This study was performed to help elucidate what kind of role various cytokines might play in the pathogenesis of endometriosis. METHOD: Concentrations of peritoneal fluid cytokines were compared in 7 women with normal pelvic finding and 23 women with endometriosis by enzyme-linked immunosorbent assay(ELISA). The patterns of cytokine mRNA expression in 8 ovarian endometrioma and 12 superficial pelvic endometriosis lesions were investigated by reverse transcription-polymerase chain reaction(RT-PCR) amplification method. RESULT: Both IL-6 and IL-10 levels in peritoneal fluid specimens with endometriosis tended to be higher than normal. However, there were no significant differences between peritoneal fluid concentrations of IFN-gamma, TNF-alpha, IL-1beta, and IL-5 of women with and without endometriosis. The levels of IL-6 and IL-10 were significantly higher in peritoneal fluid of women with severe endometriosis compared to women with mild endometriosis. IL-1beta mRNA was expressed in all of 8 deep and 12 superficial endometriosis lesions. IL-5 and IL-6 mRNA were expressed in only two black lesions respectively, however, both were not expressed in the all deep lesions. Expressions of IL-10 mRNA occurred in one red and one black lesion while this was expressed in only one of the deep lesions. TNF-alpha mRNA was expressed in one red and one black lesion of 12 superficial lesions compared with four of the deep lesions. There was the difference between kinds of increased cytokines in the peritoneal fluid and those of expressed cytokines in the endometriotic lesions of patients with endometriosis. CONCLUSION: This study supports the concept that local immunologic factors may be important in the pathogenesis and pathophysiology of endometriosis. The pattern of cytokine mRNA expression of endometriotic lesions would seem to indicate that proinflammatory cytokines such as IL-1beta and TNF-alpha are responsible for the development or progression of endometriosis.

9.Auto-Lock and Motorized Device for the Knee Joint of Long Leg Brace; Development and Performance Test.

OBJECTIVE: To develop and evaluate a new orthotic device for the knee to improve the ambulation patterns in paraplegic patients. METHOD: The new orthotic device combined a high torque servo motor with a bail-lock knee joint, and a its wireless controller adopting open-loop control system. The device was tried in a paraplegic volunteer for two months, simultaneously with the conventional long leg brace. RESULTS: The new device could keep the orthotic knee joint exended during the stance phase, and allow the sequential flexion and extension of the joint during the swing phase. The new orthotic device provided both longer step length(p<0.05) and fewer cadence(p<0.05) maintaining the same walking speed. Also, a circumduction during the swing phase reduced, and the posture of the trunk became straighter. The stride time and the double support period became longer(p<0.05), and the magnitude of the shift of pelvic center increased(p<0.05). CONCLUSION: The new orthotic device for the knee was developed and evaluated in a paraplegic volunteers who had failed to use the functional electrical stimulation. The results were satisfactory with a probable lower energy efficiency with the new orthosis. Further study for verification of the results seems to be necessary.
