1.Treatment of Ossifying Fibroma

Ossifying fibroma is a benign tumor which arises typically within the jaw bone and rarely affect the long bone. The usual affected site in the long bone is tibia and fibula. Ossifying fibroma has a moderate tendency to progress during childhood and ceases to progress after puberty. So, surgery should be delayed as long as possible. But if the lesion is wide, which may cause pathologic fracture or severe deformity, it would be necessary to resect the lesion and restore the alignment. We have experienced 10 cases of ossifying fibroma which were treated at Department of Orthopaedic Surgery, Yonsei University College of Medicine, from Jan. 1984 to Nov. 1992. Results were.as follows: l. Among 10 cases, female was 5 cases and male was 5 cases. Age at operation was 9 years 1 month in average ranging from 1 year 7 months to 18 years 6 months. 2. The lesion site was 9 cases in the tibia, 1 case in the fibula. 3. Attended type of treatment was 3 cases of bone biopsy and observation, 1 case of curettage and bone graft, 1 case of curettage, external fixation with Ilizarov apparatus and internal transportation, 1 case of resection with curettage and free vascularized fibular graft, and 4 cases of wide resection and free vascularized fibula graft. 4. Second operation was needed in 2 cases due to recurrence which were performed incomplete wide resection. In conclusion, continuous observation is needed until puberty when the lesion ceases to progress in ossifying fibroma but radical surgery like wide resection and free vascularized fibula transfer is needed if there is a risk of pathologic fracture or severe bowing deformity due to its large size.

2.Management of iatrogenic ureteral injury.

We treated 19 patients with ureteral injuries during the recent 7 years. Gynecologic operations were the most common antecedent surgical procedures(89%). The diagnosis of ureteral injuries was made immediately in 3 patients and was delayed 3 to 78 days in 16 patients. All of the 3 injuries recognized during an operation were repaired at the time of injury. Of the 16 patients with injuries recognized in the postoperative period, 1 patient was managed with percutaneous nephrostomy. 14 patients received corrective surgery successfully, but the corrective surgery in remaining patient was railed and ultimately resulted in a nephrectomy because of avascular necrosis of ureter during operation. Therefore, it is better to do corrective surgery primarily in patients with ureteral injuries recognized postoperatively in order to decrease percutaneous nephrostomy associated morbidity and hospital admission period.

3.A Case of Penile Cutaneous Horn.

Cutaneous horn is a clinical term for a circumscribed concial hyperkeratotic mass amounts to at least half of its largest diameter. The number of lesion may be single or multiple. We report herein a cases of penile cutaneous horn in a 22-year-old male. He had dark brownish colored conical shaped protruded mass on the penile shaft for about 2 months. We performed excisional biopsy and curettage. Histopathologic findings showed verruca vulgaris.

4.A Case of Nodular Amyloidosis.

A 65-year-old female patient visited our clinic complaining of multiple skin lesions since one year ago. There were yellowish to brownish colored, bean to walnut-sized nodules on both lower extremities. Dylon stain with polarizing microscopy, immunohistochemical stain to amyloid P and immunoglobuhn-kappa chain showed positive reactivities but keratin stain was negative. According to histopathologic and immunohistochemical findings, she was diagnosed as nodular amyloidosis.

5.Predictors of Depression and Quality of Life among Older Adults with Osteoarthritis.

PURPOSE: The purpose of this study was to identify predictors of depression and quality of life among older adults with osteoarthritis. The predictors included in the model were the client's characteristics(age, pain, disease duration, ADLs), personal resources(hardiness, self-care agency and family support), and depression. METHOD: 150 subjects who were older than 65 years and had diagnosis of osteoarthritis participated in the study. To answer the research questions, descriptive analysis, Pearson correlation, and hierarchical multiple regression were utilized using SPSS WIN program. RESULT: Older adults who were younger and had lower levels of pain and dependency on ADLs, and higher levels of self care agency and hardiness reported lower levels of depression(R2=0.517). Older adults who had lower levels of depression, pain, and dependency on ADLs, higher levels of family support and hardiness, and who are younger reported higher levels of quality of life(R2=0.804). CONCLUSION: Based on the findings of this study, development of nursing intervention program including pain reduction, enhancing ADL abilities and personal resources (hardiness, family support) can be suggested. Further study is needed to increase the ability of generalization of the study findings to the broader population.

6.Clinical Effect of Prazosin Hydrochloride in Patient with Benign Prostatic Hypertrophy.

The clinical effect of prazosin, an alpha-1-receptor blocking agent, was studied in 31 patients with benign prostatic hypertrophy. The daily dose of 1.5-6 mg of prazosin was given for 2 weeks to 3 months. The efficacy was assessed by using uroflowmetry calculated from maximum flow rate, average flow rate and residual urine volume. The result revealed significant improvement of day time frequency, night time frequency (p<0.05) and maximum and average flow rate (p<0.05). It was suspected that prazosin was the valuable alpha-1-receptor blocker for benign prostatic hypertrophy and worth while to initiate medical treatment before undergoing any surgical intervention or when surgery is contraindicated.

7.Effectiveness of ultrasound in traumatized scrotum.

The decision between continued medical therapy and surgical exploration in patients with traumatized scrotum is difficult. We evaluated scrotal ultrasound as a non-invasive aid in such problematic cases. During s 42 months period. 17 patients were hospitalized for treatment of traumatized scrotum. All of them underwent careful physical examination and scrotal ultrasound. The results were as follows.; 1. In the appropriate clinical setting. ultrasound provides objective information supporting the need for surgical intervention in patients with traumatized scrotum and prevents unnecessary operation. 2. The results of treatment in 12 cases of 13 cases who underwent conservative treatment were good. 3. Scrotal ultrasound is necessary as soon as possible in traumatized scrotum and testicular salvage rates are improved markedly when early surgical correction of a ruptured tunica albuginea is performed. 4. Ultrasound is noninvasive and effective single diagnostic method in scrotal injury.

8.A Case of Modified Jensen Procedure in 6th Nerve Palsy.

Because of the long intracranial course, the 6th nerve is vulnerable to increased intracranial pressure, trauma to the cranial floor, meningeal edema, inflammation in the base of the skull, and any displacement of the brain stem. The authors had experienced a case of abducens nerve palsy resulting non-concomittent esotropia and lateral gaze palsy due to brain trauma by traffic accident. We had performed modified Jensen procedure that had good result. That procedure is presented to aid in preserving the anterior segment blood supply. The anterior ciliary artery in the lateral rectus is not included in the union. The case report is reviewed with the pertinent literature.

9.Geometrical analysis on the dentofacial composition of korean beautiful woman.

STATEMENT OF PROBLEM: The meaning of the beauty has a little different nature according to a time, culture, and nation. PURPOSE: This study was undertaken to estimate the geometric esthetic criteria for Korean woman by analysis of facial photograph in a smiling. MATERIAL AND METHODS: The facial photographs of 678 Korean beautiful woman were collected from Miss Korea candidates(from 1971 to 2003 year). The dento-facial composition was measured and analyzed geometrically on computer monitor using Adobe. Photoshop. 7.0. The statistical significance of the differences among the 4 decade groups was estimated by a chi2 test. RESULT: The Korean beautiful woman showed the geometric characteristics such as the 'coincidence'(83.0%) between facial midline and dental midline, the 'normal'(96.9%) alignment and arrangement of upper anterior teeth, the 'average'(79.1%) position and 'upward'(53.4%) curvature of upper lip, the 'parallel'(85.7%) relationship between the lower lip and upper anterior incisal curvature, the 'touching'(49.9%) and 'not-touching'(44.8%) relationship between upper anterior incisor and lower lip, the 'first premolar'(54.1%) and 'second premolar'(38.9%) displayed in a smile, and the 'symmetry'(89.4%) of smile. There was statistical significance among the 4 decades in the respect of the upper lip curvature, the relationship between upper anterior incisor and lower lip, and the most posterior teeth displayed in a smile (p<0.05). CONCLUSION: The Korean beautiful woman showed similar results to the western esthetic criteria in the some respects and the tendency that the Korean woman had more active smiling recently.
