2.Antibacterial Activity of Ceftizoxime Against Gram Negative Enteric Bacteria in vitro and in vivo.

Ceftizoxime sodium is a new synthetic β-lactam antibiotic combining potent antibacterial activity with high stability to a wide range of bacterial β-lactamase. This experiment was achieved to evaluate the antibacterial activities of ceftizoxime sodium against. Gram negative enteric bacteria isolated from in outpatient visiting Yeungnam university hospital and to study the emergence of drug induced bacterial variants which resist to ceftizoxime in vitro. The antibacterial activity of the ceftizoxime was compared with that of antibiotics and its effect on population of normal intestinal flora in mice was observed. The results are summarized as follows: 1. Highly effective antibacterial activity of ceftizoxime against Gram negative enteric bacilli was demonstrated and this antibacterial activity was superior to that of ampicillin. 2. Several test strains shows multiple antibiotic resistance. Among 15 strains of Escherichia coli, 1 strain was resistant to ampicillin, cefadroxil, gentamicin, tetracycline, and 2 strains were resistant to ampicillin, cefadroxil, tetracycline, five strains of Escherichia coli and Enterobacter cloacae was resistant to ampicillin, tetracycline and Shigella dysenteriae was resistant to ampicillin, gentamicin, tetracycline. 3. The frequency of in vitro emergence of resistant variants among ceftizoxime sensitive bacteria in the presence of increasing concentrations of the compound was found to be low. 4. Plasmid was isolated in 6 of 9 strains (6 strains of Escherichia coli, Shigella dysenteriae, Enterobacter cloacae and Salmonella typhi). That showed different antibiotic resistance. They were 5 strains of Escherichia coli and 1 strain of Shigella dysenteriae. However, plasmid could not be considered as a hallmark for antibiotic resistance by this Further studies with curing experiment are to be accomplished for this purpose. 5. Changes in the bacterial count of normal intestinal flora following 25 mg/kg/day administration of ceftizoxime over 5 consecutive days were not significant. In conclusion, ceftizoxime appeared to be a drug of choice in the treatment of Gram negative enteric bacilli infection.

4.A Study as Epidemiologic and Clinical Aspect of Mycoplasma pneumoniae Pneumonia during the Last 5 Years.

PURPOSE: It is not surprising that serologically based epidemiologic studies have documented the high incidence of mycoplasma respiratory infection throughout the world. We studied the epidemiologic and clinical aspects of mycoplasma pneumonia in children if there is any differences in the peak age-related incidence, seasonal predominance and clinical manifestations in compare with other studies reported in Korea. METHODS: We reviewed 1004 cases of Mycoplasma pneumoniae pneumonia diagnosed by clinical manifestations and positive serologyc tests, retrospectively, who were admitted to the department of pediatrics in four university hospitals in Seoul and Inchon during the last 5 years from 1993 to 1997. RESULTS: 1) The age distribution of mycoplasma pneumonia showed highest incidence in children 4-6 years old. There was no difference in sex distribution at any age group. 2) Although there was little difference in annual distribution, it was highest in 1997. 3) Cough was the most prevalent prodromal symptom. 4) Vomiting, Nausea, Abdominal pain and headache were noted as the extrapulmonary manifestations. 5) Pulmonary infiltration was noted mostly in lower lobe lobes on chest X-ray. They showed interstial involvement in 34.5%, lobar involvement in 30.0%, and pleural effusion in 7.8% of the total. 6). As the complications, aggravate or trigger the asthma attack were noted in 4.0%, development of sinusitis in 3.5% and otitis media in 1.4% of the total. There were four cases of aseptic meningitis and one case of lung abscess. CONCLUSIONS: In our survey, there was little or no epidemiologic differences in the incidence of Mycoplasma pneumoniae pneumonia. We can find the increasing tendency of lobar pattern of involvement on chest X-ray in mycoplasma pneumonia in children.

5.Steroid-Induced Remission in Primary Malignant Lymphma of the Brain.

A case of primary malignant lymphoma of the brain is described in which lesions seen on computed tomography scans disapperared and clinical remissions occurred by the administration of corticosteroid. In our patient, at least three remissions occurred over a span of about 44 months. The diagnosis was delayed because of easy disappearance and confusion with other conditions. A stereotactic biopsy was performed and diagnosed as Non-Hodgkin's lymphoma, diffuse, large noncleaved cell type, B cell origin by H & E and immunologic marker analysis. We present our case with review of the literatures.

6.The involvement of K+ channels and the possible pathway of EDHF in the rabbit femoral artery.

Experiments were designed to characterize the cellular mechanisms of action of endothelium-derived vasodilator substances in the rabbit femoral artery. Acetylcholine (ACh, 10(-8)-10(-5) M) induced a concentration-dependent relaxation of isolated endothelium-intact arterial rings precontracted with norepinephrine (NE, 10(-6) M). The ACh-induced response was abolished by the removal of endothelium. NG-nitro-L-arginine (L-NAME, 10(-4) M), an inhibitor of NO synthase, partially inhibited ACh-induced endothelium-dependent relaxation, whereas indomethacin (10(-5) M) showed no effect on ACh-induced relaxation. 25 mM KCl partially inhibited ACh-induced relaxation by shifting the concentration-response curve and abolished the response when combined with L-NAME and NE. In the presence of L-NAME, ACh-induced relaxation was unaffected by glibenclamide (10(-5) M) but significantly reduced by apamin (10(-6) M), and almost completely blocked by tetraethylammonium (TEA, 10(-3) M), iberiotoxin (10(-7) M) and 4-aminopyridine (4-AP, 5 x 10(-3) M). The cytochrome P450 inhibitors, 7-ethoxyresorufin (7-ER, 10(-5) M) and miconazole (10(-5) M) also significantly inhibited ACh-induced relaxation. Ouabain (10(-6) M), an inhibitor of Na+, K(+)-ATPase, or K(+)-free solution, also significantly inhibited ACh-induced relaxation. ACh-induced relaxation was not significantly inhibited by 18-alpha-glycyrrhetinic acid (18 alpha-GA, 10(-4) M). These results of this study indicate that ACh-induced endothelium-dependent relaxation of the rabbit femoral artery occurs via a mechanism that involves activation of Na+, K(+)-ATPase and/or activation of both the voltage-gated K+ channel (Kv) and the large-conductance, Ca(2+)-activated K+ channel (BKCa). The results further suggest that EDHF released by ACh may be a cytochrome P450 product.

7.A Case of Lacunar Skull.

A case of lacunar skull associated with meningomyelocele, spina bifida, kyphcsis on thoracolumbar area, hydrocephalus, bilateral club foot and paralysis of lower extremities at birth was reported. Diagnosis was made by X-ray of skull, whole spine, upper and lower extremities. On skull X-ray film, multiple small area of round rarefaction surrounded by arborizing pattern of bony ridge was visualized on both parietal area. On autopsy, outer table of the parietal bone was smooth and inner table of the parietal area was irregular and rough due to bony defect which was surrounded by bony ridges. A brief review of the literautre on lacunar skull is given.

8.The effect of LH Receptor in the Pregnancy of Poor Responders.

OBJECTIVES: To investigate the effect of LH receptor in folliculogenesis, we confirm the expression level of LH receptor (LH-R) mRNA in human granulosa cells (GCs) and its expression levels were analyzed by comparison to embryo developmental rate and pregnancy rate. MATERIALS AND METHODS: GCs were obtained at the time of oocyte retrieval from the patients undergoing IVF-ET program. The patients were divided into two groups: Group I (n=20) is poor responder (retrieved oocyte(s)< or =3ea), Group II (n=80) is normal responder (retrieved oocytes>3ea). After the extraction of total RNA, semiquantitative RT-PCR was performed and the expression level of LH-R mRNA was normalized by beta-actin. Statistical analysis was performed by using Chi(2) test, Student's t-test and Pearson correlation. RESULTS: In Group II, the relative values of LH-R mRNA (0.680 vs. 0.463, p<0.005) and pregnancy rate (54.7% vs. 23.1%, p<0.05) were significantly higher than in Group I. Number of retrieved oocyte(s) was gradually increased when the expression of LH-R mRNA was increased (p<0.05). But the quality of retrieved oocyte and transferred embryo were not related with the expression of LH-R mRNA. When the pregnancy rate was compared with FSH only group and FSH combined with hMG group in the ovarian stimulation protocol, FSH combined with hMG group was significantly higher than FSH only group in Group I (37.5% vs. 0%), and the expression of LH-R mRNA was significantly higher in hMG combined group than FSH only group (p<0.05). CONCLUSION: Expression level of LH-R mRNA has important role in ovarian function related with the response to gonadotrophin in human folliculogenesis. Furthermore these data might provide the evidence that additional use of hMG is helpful to poor responders.

9.A Study on User Satisfaction regarding the Clinical Decision Support System (CDSS) for Medication.

OBJECTIVES: Many medication errors can occur when ordering and dispensing medicine in hospitals. The clinical decision support system (CDSS) is widely used in an effort to reduce medication errors. This study focused on the evaluation of user satisfaction with the CDSS for medication at a university hospital. Specifically, this study aimed to identify the factors influencing user satisfaction and to examine user requirements in order to further improve user satisfaction and drug safety. METHODS: The study was based on survey data from 218 users (103 doctors, 103 nurses, and 15 pharmacists) at a university hospital that uses the CDSS. In order to identify the factors influencing user satisfaction with the CDSS, a multiple linear regression was performed. In order to compare the satisfaction level among the professional groups, an analysis of variance (ANOVA) was performed. RESULTS: The reliability of information, decision supporting capability, and departmental support were significant factors in influencing user satisfaction. In addition, nurses were the most satisfied group, followed by pharmacists and doctors according to the ANOVA. Areas for further improvement in enhancing drug safety were real time information searching and decision supporting capabilities to prevent adverse drug events (ADE) in a timely manner. CONCLUSIONS: We found that the CDSS users were generally satisfied with the system and that it complements the nationwide drug utilization review (DUR) system in reducing ADE. Further CDSS evaluation in other hospitals is needed to improve user satisfaction and drug safety.

10.The Citation Status of the References in the Journal of Korean Academy of Nursing.

PURPOSE: This is for analyzing the recent citations from papers enrolled in the Journal of Korean Academy of Nursing and understanding correct marking of references. METHOD: Targeted 2,334 references in 93 articles introduced from Feb. 2002 to Dec. 2002 which are applied current contribution rules of the Journal of Korean Academy of Nursing. RESULT: 1. 25 references are quoted per article and particularly foreign journals are most frequently quoted. Among foreign journals, Nursing Research is most frequently used. The Journal of Korean Academy of Nursing is also mostly referred material among national journals. 2. The retrieval rate is prove to be 1,539(83.8%) and 267(11.7%) are without errors and the other 2,022 have at least more than one error regardless of frequency. 3. Major error rate of periodical publication is 484(36.8%) and most occurred in the inconsistency of titles between articles and references. 4. Foreign journals have much more errors than national ones concerning periodical publications. CONCLUSION: Correct citation and marking of references can be achieved through strict reviewing process by authors, editors, publishers and paper investigators. In consequence, it is expected that these processes can cause improvement of quality in impact factor and papers.
