Phytochemical constituents were isolated from the fruits of Acanthopanax chiisanensis by repeated column chromatography. Their structures were identified as beta-sitosterol (1), daucosterol (2), sesamin (3), chiisanogenin (4), and 22alpha-hydroxy chiisanogenin (5) by spectroscopic analysis (MS, 1H-, and 13C-NMR). Compounds 1 - 5 were isolated for the first time from the fruits of A. chiisanensis.

2.A Study of Factors Influencing Health Promoting Behavior and Quality of life in the Elderly.

The purpose of this study was to investigate the factors influencing health promoting behavior and quality of life in the elderly, to provide the basic data for health promoting intervention in order to improve quality of life. The subjects of this study were 51 elderly person over the age of 65, living in Seoul, Korea, during the period from November, 1997 to January, 1998. The instruments for this study were the health promoting behavior scale developed by Walker et (1987), the quality of life scale by RoyouJa(1988), the health concept scale by Laffrey(1986), the perceived health states scale by Lawston et al. (1992), the health value scale by Walston et al.(1978), the self esteem scale by Rogenberg(1965) and self efficacy scale by Sherer(1982). The instruments of this using descriptive statistics, t-test, Person correlation coefficients ANOVA and stepwise multiple regression. The results of this study are as follows; 1. The health promoting behavior showed significant positive correlation with health concept perception of health status, self esteem, internal health locus of control, self efficacy and quality of life in the elderly. 2. The quality of life showed significant positive correlation with health concept, perception of health status, self esteem, internal health locus of control, self efficacy in the elderly. 3. Stepwise multiple regression analysis revealed that the most powerful predictor of health promoting behavior was quality of life. A combination of quality of life, health concept, perceived health status, self esteem, internal health locus of control, and self esteem, internal health locus, and self efficacy accounted for 46% of the variance in health promoting behavior in the elderly. 4. Stepwise multiple regression analysis revealed that the most powerful predictor of quality of life in the elderly was self esteem. A combination of self esteem. health concept, perceived health status, health promoting behavior and self efficacy account for 56% of the variance in quality of life in the elderly. From the results of the study, the following recommendations are presented as follow : 1. Development of a health promoting program to improve quality of life in the elderly. 2. In developing the health promoting, the above major influencing factors be considered. 3. It is suggested that an education specialist in practice in the community be include in the program development.

3.Therapeutic Effect of the Intraperitoneal Cisplatin Installation (IPCI) in Advanced Gastric Carcinoma with Serosa Invasion Retrospective study.

PURPOSE: We performed retrospective study to evaluate the preventive effect of intraperitoneal cisplatin installation (IPCI) on peritoneal recurrence after curative resection of advanced gastric cancer. MATERIALS AND METHODS: The effect of IPCI was evaluated in 297 advanced gastric carcinoma patients from January 1993 to December 1996. In IPCI group, 100 mg/body of cisplatin in one liter of saline was installed in peritoneal cavity before wound closure in operating room and drained out 2 hours later. Postoperative adjuvant chemotherapy with combination of 5-FU and cisplain was performed. 155 cases were treated by IPCI. Median follow-up period was 26 months. RESULTS: Out of 139 (46.8%) recurred cases, peritoneal, local and distant recurrences developed in 65 (37.8%) cases, 66 (38.4%) cases and 41 (23.8%) cases respectively. In univariated analysis for survival and recurrence, IPCI, T stage and N stage were significant prognostic factors. As regards to peritoneal recurrence, IPCI and T stage were significant factors. In multivariated survival analysis, as regards to recurrence, IPCI, T stage and N stage were significant prognostic factors. As regards to peritoneal recurrence, IPCI was the only significant independent prognostic factor. CONCLUSION: We concluded that IPCI can effectively prevent peritoneal recurrence and overall recurrence and it shows marginal survival benefit in advanced gastric cancer patients with serosa invasion.

4.Endoscopic Balloon Sphincteroplasty without Sphincterotomy for Removal of Common Bile Duct Stones.

BACKGROUND: The complications associated with endoscopic stone removal arise from the sphincterotomy that is performed to facilitate stone extraction. Early complications such as bleeding, perforation, pancreatitis and sepsis occur in about 10% of all patients. Moreover late complications may emerge due to the loss of the sphincter function. So the endoscopic removal of the bile duct stones without a sphincterotomy seems preferable in order to avoid the early complications related to the sphincterotomy, and may also help to preserve the function of the sphincter of Cddi. AIM: To evaluate the efficacy and safety of endoscopic balloon sphinteroplasty(EBS), we attempted to remove common bile duct stones, less than 12 mm, without sphincterotomy in 11 patients. We also investigated the effects of sphincteroplasy on the motility of sphincter of Oddi before and after EBS. RESULTS: All patients were treated succesefully, with two patients administered sublingual nitroglycerine for the medical sphincter dilatation. Only one patient had mild abdominal pain, while the others experienced no complications. Follow-up manometric examinations showed the papillary function to be well preserved. CONCLUSION: This results suggested that the endoscopic balloon sphincteroplasy without sphincterotomy for the removal of small-sized stones is a useful and safe method, and its major advantage may lie in the preservation of the sphincter function. We recommend that prospective studies, preferably randomized, with clear objectives, are needed to clarify the success and safety of this technique compared to the standard sphincterotomy.

5.A Clinical Trial of Topiramate for Weight Loss in Schizophrenia with Overweight or Obesity.

OBJECTIVE: Antipsychotic-induced weight gain is associated with treatment noncompliance and is also known to be associated with several medical conditions in schizophrenia. Topiramate, a relatively new antiepileptic drug, is currently used for mood and eating disorders, and also offers the advantage of weight loss. This study explored the efficacy and tolerability of topiramate as an adjuvant treatment of schizophrenia with overweight or obesity. METHODS: In this 8-week, prospective open trial, 30 hospitalized, schizophrenic patients took topiramate at a mean maintenance dosage of 159.37+/-61.15 mg/day. The primary measures were weight, body mass index (BMI), waist circumference, hip circumference, and waist-to-hip ratio. The safety measures included adverse events, physical examination, clinical laboratory data, and vital signs. The Clinical Global Impression Severity (CGI-S) Scale was used to quantify changes in schizophrenic symptoms and signs. RESULTS: Body weight, BMI, waist circumference, and hip circumference decreased significantly after treatment but the waist-to-hip ratio did not. The changes of body weight and BMI during 8 weeks treatment with topiramate were significantly correlated with the maintenance dose of topiramate. The high dose group (>100 mg/d) was significantly more changed in body weight and BMI between baseline and 8 weeks than the low dose group (< or =100 mg/d). The scores on the CGI-S scale decreased significantly over the 8 weeks of treatment. CONCLUSION: The results suggest that topiramate is both efficacious and tolerable for the short-term adjuvant treatment of schizophrenia with overweight or obesity. Further placebo controlled studies included larger samples would be needed to confirm these results. And much more clinical researches should be required to establish guideline for the optimal dose and duration of treatment using topiramate as an antiobesity agent in schizophrenia.

The hyaline membrane disease is not a common disease in Korea. Only a few reports of small scale are avaiable in the literature. We have experienced 5 cases of HMD during approximately 1 year period. The diagnosis was made either on characteristic clinical and roentgenological features or postmortem examination. The birth weights of these cases were in the range of 1,000-1,500gm in 2 cases and 2,000-2,500gm in 3 cases. And their gestational age was 28-34 weeks in most of the cases. Three cases were delivered by C-section. There was 1 case of placenta previa. Four of these 5 cases died after average 18 hours postnatum. Postmortem findings in two cases were characterized by typical hyaline membrane lining th respiratory bronchioles and alveolar ducts. Other prominent findings were atelectasis, interstitial edema and congestion and lymphatic dilatation. One case complicated with multifocal bronchopneumonia and perinatal telencephalic leukoencephalopathy. The other case showed acute subarachnoid hemorrhage probably from germinal matrix hemorrhage.

7.A Case of Acute Pancreatitis Caused by Distal Migration of a Pyloric Covered Self-Expandable Metal Stent.

Endoscopic metal stent insertion has been widely performed to palliatively treat gastrointestinal (GI) tract obstruction in patients with cancer. The use of metal stents for benign strictures of the GI tract is increasing due to the low risk of major complications other than stent migration. The incidence of acute pancreatitis after placing a GI tract metal stent is rare. Herein, we report a case of acute pancreatitis caused by a covered metal stent that was initially inserted for a benign stricture of a gastroduodenal anastomosis that developed after the operation, then distally migrated, bent into a U-shape, and impacted into the second portion of the duodenum in a 56-year-old female who underwent a Billroth-I gastrectomy due to gastric cancer.

8.Formation of an Intestine-Cartilage Composite Graft for Tracheal Reconstruction.

BACKGROUND: Tracheal transplantation is necessary in patients with extensive tracheal stenosis, congenital lesions and other oncologic conditions but bears many critical problems compared to other organ transplantations. The purpose of this study was to develop intestine-cartilage composite grafts for potential application in tracheal reconstruction by free intestinal graft. MATERIAL AND METHOD: Hyaline cartilage was harvested from trachea of 2 weeks old New Zealand White Rabbits. Chondrocytes were isolated and cultured for 8 weeks. Cultured chondrocytes were seeded in the PLGA scaffolds and mixed in pluronic gel. Chondrocyte bearing scaffolds and gel mixture were embedded in submucosal area of stomach and colon of 3 kg weighted New Zealand White Rabbits under general anesthesia. 10 weeks after implantation, bowels were harvested for evaluation. RESULT: We identified implantation site by gross examination and palpation. Developed cartilage made a good frame for shape memory. Microscopic examinations included special stain s howed absorption of scaffold and cartilage formation even though it was not fully matured. CONCLUSION: Intestine-cartilage composite graft could be applicable in the future as tracheal substitute and should be further investigated.

9.The effect of pomegranate on postmenopausal syndrome in ovariectomized rats.

OBJECTIVE: To determine whether or not pomegranate extract can affect postmenopausal syndrome in ovariectomized rats. METHODS: Seventy-nine female Sprague-Dawley rats were used: A, no intervention; B, sham operation and distilled water; C, ovariectomy and distilled water; D, ovariectomy and 10% dilute pomegranate; E, ovariectomy and 20% pomegranate; and F, ovariectomy and 40% pomegranate. The study samples were obtained 4 weeks later. The following parameters were investigated for analyses: the thickness of urogenital epithelium, tail skin temperature, serum levels of sex hormones, lipid profile, homocysteine, and thiobarbituric acid reactive substances. RESULTS: 20% pomegranate had a transient thickening effect of bladder epithelial layer. There was an intermittent lowering effect of tail skin temperature of pomegranate extract, however, it was not consistent. There were lowering effect in serum homocysteine and elevating effect in serum high density lipoprotein-cholesterol. Pomegranate did not reverse post-ovariectomy reduced sex hormone levels. CONCLUSION: Pomegranate extract showed a tendency to reverse ovariectomy-related urothelial changes. Also pomegranate had a serum homocysteine-lowering effect.

OBJECTIVES: To determine whether or not pomegranate extract can affect depression, anxiety, and food intake in ovariectomized rats. METHODS: Seventy-nine female Sprague-Dawley rats were divided into six groups: A, no operation and no drug intake; B, sham operation and distilled water; C, ovariectomy and distilled water; D, ovariectomy and 10% dilute pomegranate extract; E, ovariectomy and 20% dilute pomegranate extract; and F, ovariectomy and 40% dilute pomegranate extract. Beginning 2 days after surgery, drugs were administrated for 4 weeks. After that, the rats were subjected to the elevated plus maze (EPM) test and forced swim test (FST). RESULTS: The 10% pomegranate extract had a lower % closed arm entry frequency in the EPM test. A pomegranate dose-dependent decrease in the duration of immobility duration in FST was shown. Pomegranate did not reverse ovariectomy-related hyperphagia and weight gain. CONCLUSION: Pomegranate extract improved depression and anxiety in a postmenopausal model with ovariectomized rats.
