We assessed the accuracy of clinical evaluation, arthrography and arthroscopy in the diagnosis of meniscal lesions in ninty knees in which arthrotomy was performed for disabling symptoms after evaluation by these three methods. At surgery, ninty-two menisci were removed, of which seventy-two were abnormal and twenty were normal. In these ninty-two menisci, correct diagnostic rate was made clinically 78%, arthrographically 70% and arthroscopically 86%. Arthrographic diagnosis was least accurate for lesions of the lateral meniscus and arthroscopic diagnosis was least accurate for lesions of the posterior horn of the medial meniscus and peripheral lesions. Based on this study, it was concluded that both the arthrography and arthroscopy are valuable technique for diagnosis of meniscal lesions.

15 cases of acute dislocation of the elbow were treated at Department of Orthopaedic Surgery, Capital Armed Forces General Hospital, Seoul, Korea during the period of March, 1985. To June, 1986 were analysed. The length of follow-up period ranged from 6 months to 16 months, with a mean of 9 months. The results were as follows; 1. 7 cases had been surgically treated, and they revealed rupture of the medial collateral ligament in all. So, we think that the medial collateral ligament of the elbow plays an important role in elbow stability. 2. The average immobilization period for the patient with non-operative treatm ent was 2 weeks. And normal ran ge of motion was obtained at 8 weeks. So, there was no. significant difference from other reports in the duration of recovery. 3. The period of recovery in operated patients was 14 weeks, and it was 6 weeks longer than non-operatively treated patients. 4. There was no chronic elbow instability in all 15 patients, but 3 cases with non-operative treatment complained of medial elbow pain on vigorous exercise.

Craniofacial clefts are rare among facial anomalies with an incidence of 1.5 to 5 per 100,000 births, and 1 per 100 cases of cleft lip and palate. The Tessier No. 7 clefts are unusual lesions that result from failure of the embryonic mandibular and maxillary processes to properly fuse and form the corners of the mouth. We experienced a case of Tessier No. 7 craniofacial cleft in a 1 day-old female patient who presented with a macrostomia and auricular malformation. The diagnosis was established by clinical and radiographic findings. A brief review of literature was made.

A case of intraneural ganglion, 5cm×2cm×1.5cm, which was located between the funiculi of the common peroneal nerve was excised completely under the operative microscope. There was no communication with adjacent bursa or joint.

Traditionally, evaluation after ACL reconstruction has been focused on physical characteristics and measures of knee stability. Recently, however, reliance on such criteria has been refuted based on the lack of a strong relationship between these measures and both the patient s perception of knee function and return to sports activity. In present study, preoperative and postoperative physical examinations, isokinetic dynamometer, arthrometer test and functional tests were performed on 17 patients with chronic anterior cruciate ligament tears treated by arthroscopic reconstruction using autogenous bone-patellar tendon-bone graft with at least 1 year of follow-up. Muscle power, joint stability and functional recovery were checked for functional evaluation by specific methods. Lysholm score rating scale and functional tests (single leg hop test, vertical jump test, timed single jump test) were performed preoperatively and 6, 12 months postoperatively. Pivot-shift test, Lachman test, anterior drawer test, thigh circumference index and Cybex II+ isokinetic dynamometer test were done preoperatively and at 3, 6, 12 months postoperatively. KT-2000 arthrometer test was done at last follow-up. Lysholm score has showed increasing tendency at 6, 12 months postoperatively (p < 0.01). In Cybex study, deficit percentage of peak torque and total work of the quadriceps and hamstrings was same or slightly decreased at 3, 6 months, but have decreased at 1 year follow-up (p < 0.01). Functional tests and physical examinations have also showed improving tendency at 12 months (p < 0.01). Arthrometer test revealed no significant laxity of the knee joint at last follow-up. In conclusion, muscle strengh and functional capacity in ACL reconstructed knee showed significant improvement at postoperative one year compared with preoperative status. It was considered that arthroscopic reconstruction using autogenous bone-patellar tendon-bone graft is one of the useful method for chronic anterior cruciate ligament injury.

Extraskeletal Ewing's sarcoma is a rare primary malignant soft tissue tumor that has the same pathological characteristics of skeletal Ewing's sarcoma. It is known that the differential diagnosis of Ewing's sarcoma from undifferentiated rhabdomyosarcoma, neuroblastoma, reticulum cell sarcoma, and malignant lymphoma by the histologic studies only are not easy because their histologic findings are nearly similar each other. Therefore, immunohistochemical method, electron microscopic examination, cytogenetic study, or molecular gene study are used together to confirm the diagnosis. It chiefly involves the lower extremity, soft tissues of the paravertebral region, and adjacent chest wall. We recently experienced a case of extraskeletal Ewing's sarcoma of duodenum in a 21-year old female. The patient underwent Whipple's operation and postoperative combined chemotherapy were done.

OBJECTIVES: This study aimed to identify the trend regarding the difference between needs and recipients for scaling in the period from 2000 to 2009, and to analyze the differences by gender and age. METHODS: This study was a follow-up study that analyzed the secondary data. The analysis was done in those > or =19 years old who were included in the data obtained from KNOHS (2000, 2003), KNHANES, and CHS (2008, 2009). The dependent variables were needs and recipients for scaling, and the rate of the difference. Independent variables were the year, gender, and age. By performing the gender-age specific directly standardized the rates, independent t-test and one-way ANOVA. For statistical analysis, the study used MS Office Excel 2010 and PASW statistics 18.0. RESULTS: The rate regarding the needs for scaling has been steadily decreased, but the rate of recipients for scaling was increased, since 2000. The difference rate was reduced from 85.6% in 2000 down to 62.3% in 2009. Men showed a higher rate of needs for scaling compared to women (P<.05). Those aged 55-64 and 65-74 years old demonstrated a higher rate of needs for scaling (P<.05), but actual recipients for scaling were largely populated in the age ranges of 25-34 years old and 35-44 years old (P<.05). CONCLUSIONS: In the last 10 years, the difference between needs and recipients for scaling has been reduced. However, there were some differences depending on gender and age. To this end, it should be necessary to consider men and mid-to-old aged (> or =55 years old) groups with preference, when setting the priority to mitigate the unmet needs for scaling.

Of 414 patients with ruptured intracranial aneurysms, 42 cases had large intracerebral hematoma over 30cc un volume and were operated on at the Pusan Paik Hospital, Inje University from 1990 to 1992. The incidence of the large hematoma was 10% and the highest age incidence was 6th decade. The most frequent site of aneurysm with large hematoma was middle cerebral artery and the mean volume of hematoma was 58 cc. There was a favorable outcome(good and fair) in 40% of all the cases and 34% mortality, Mortality rate increased in patients who had larger hematoma, had poor clinical or neurological grade at the diagnosis and were operated on after 72 hours of attack. We recommend that in aneurysmal patients with large hematoma, hematoma should be removed as soon as possible and that ruptured aneurysm be clipped at the same time.

Granulocytic sarcoma(chloroma) is an uncommon and solid tumor composed of immature cells of the mycloid series usually seen during the course of myelogenous leukemia or myeloproliferative disorders. The common sites for the occurrence of granulocytic sarcoma are soft tissue, periosteum, bone, lymphoid tissue, skin, gastrointestinal tract and testis. But extradural presentation of the spine is very rare. We report two cases of extradural granulocytic sarcoma of the spine. Early recognition of the etiology of the paraplegia and the underlying systemic involvement with leukemia resulted in an excellent neurological and hematological outcome.
