Objective To evaluate the therapeutic effect of flexible ureterorenoscopy (F-URS) with holmium laser lithotripsy in managing symptomatic caliceal diverticular calculi. Methods The records of 23 patients by flexible ureterorenoscopy (F-URS) with holmium laser lithotripsy in managing symptomatic caliceal diverticular calculi from January 2008 to December 2010 were retrospectively reviewed. The 23 cases included 15 males and 8 females, aged from 23 to 68 years. All patients had unilateral calyceal diverticulum stone, with 11,9,and 3 cases had a caliceal diverticular stone in the upper pole, midkidney, and lower pole,respectively.Sand-like stones were observed in most caliceal diverticular. The diameter of the largest stone was 18.9 mm.The presenting symptoms were flank pain, recurrent urinary tract infections, and hematuria. One week before operations, all patients was placed Double-J stent and underwent intravenous urography and renal computed tomography (CT).The ureteral access sheath was placed firstly during operation, and then ureterorenoscope was inserted into the renal pelvis. The diverticular neck was incised with holmium laser if needed, and a 200um holmium laser fiber was used to crush diverticular stone. Results Flexible ureterorenoscopes were successfully placed in all patients.Postoperatively,15 (65.3%patients were rendered stone free (SF),5 patients had clinically insigni? cant residual fragmentsless than 4 mm )( CIRF),and a residual fragment (RF) was found in three patients. The success rate ( SF ± CIRF) was 87.0%.All patients were symptom free after operation. The average operative time and hospital stay were 60 minutes and 3.5 days, respectively. No severe complications occured.A follow-up of 6 -12 months showed no recurrence. Conclusions F-URS using a holmium laser lithotripsy is a safe and effective, and may be offered as a first line therapy for symptomatic caliceal diverticula calculi.

Chelating ligand method has been used to synthesize baicalin-metal (Ni2+, Co2+, Cu2+) complexes (BMC). The composition and structure of BMC were characterized by the element analysis, ultraviolet spectrum (UV), infrared spectrum (IR), mass (MS) and thermal gravitational analysis (TGA). MTT was used to analyze the effects of BMC on SMMC-7721 cell proliferation. PI staining method and Annexin-V/FITC double staining method were used to analyze the effects of BMC on the cell cycle and apoptosis of SMMC-7721 cell. Fluorescence quantitative RT-PCR was used to analyze the expression of BMC on Bcl-2 gene and Bax mRNA, flow cytometry was used to analyze BMC on the expression of Bcl-2 protein and Bax protein. The antineoplastic activity and mechanism of action of BMC was explored comprehensively. The results showed that three new kinds of BMC (molar ratio of 2 : 1) were successfully prepared, the complexes molecular formula are: Na2Ni(C21H16O11)2 x 10H2O, Na2Co(C21H16O11)2 x 8H2O and Na2Cu(C21H16O11)2 x 8H2O. According to the results of cell cycle and apoptosis detection, BMC stopped cells at G0/G1 phase to S phase and G2/M phase. Gene and protein detection showed that under the given concentration and time, BMC can downregulate the expression of Bcl-2 gene in SMMC-7721 cells, and significantly decrease the expression of Bcl-2 protein, at the same time, with the increase of expression of Bax gene, the Bax protein's expression increased significantly. Which indicates that BMC restrain cell proliferation and cell apoptosis by stopping cell cycle, reducing the expression of Bcl-2 and increasing that of Bax; The anti-tumor activities of three kinds of complexes were: baicalin-copper (BC-Cu) > baicalin-cobalt (BC-Co) > baicalin-nickel (BC-Ni) > baicalin (BC), showing the dose-response relationship.

Objective To choose proper proton magnetic resonance spectroscopy(~1XH-MRS) parameters to fit for practical femoral marrow cavity and to produce short-timed,well-repeated and excellent ~1H-MRS images. Methods The tentative study of ~1H-MRS on the normal femoral bone marrow in 26 volunteers was performed with a 1.5 T MR after the informed consent. The single-voxel spectroscopy and stimulated echo acquisition mode were used for ~1H-MRS collection.~1H-MRS parameters for 12 volunteers were 128 acquisitions,1 cm?1 cm?1 cm volume of interest(VOI)size and repeatedly 2—3 times within the same location.~1H-MRS parameters for another:14 volunteers were different numbers of acquisition (128 and 256 times, respectively)and different VOI sizes(2 cm?2 cm?2 cm and 1 cm?1 cm?1 cm, respectively). Results For ~1H-MRS with 1 cm?1 cm?1 cm size of VOI and 128 times of acquisition with the full width haft max of water≤8—12 Hz,the base-line was steady and the signal-noise ratio was high up to 11.31.~1H-MRS was different in the different femoral locations showing the maximum peak sites at near 0.90 ppm(?10~(-6))or 1.65 ppm,but~1H-MRS within the same location was always same or similar with different VOI sizes(1 cm?1 cm?1 cm or 2 cm?2 cm?2 cm)or different numbers of acquisition(128 or 256 times).~1H-MRS acquisition time was not related with the size of VOI but with the numbers of acquisition.128 and 256 times of acquisition cost 199 s and 391 s,respectively. Conclusion With the technique of small size of VOI(1 cm?1 cm?1 cm)and decreased numbers of acquisition(128 times),it is propable to get well-repeated and excellent ~1H-MRS within less time.It is also more practical for clinics to achieve ~1H-MRS of the femoral marrow with the proper technique.

Chinese Journal of Infection and Chemotherapy 2016;16(3):241-246
Objective To examine the distribution of bacterial species and antimicrobial susceptibility profile of pathogens in febrile neutropenic patients. Methods A total of 355 bacterial strains were isolated from febrile neutropenic patients in Shanghai General Hospital from January 2005 to December 2012. Antimicrobial susceptibility testing was done by Kirby-Bauer method. The susceptibility testing results were analyzed according to CLSI 2014 breakpoints. Results Gram-negative bacteria accounted for 70.4% of the 355 isolates, while gram-positive organisms accounted for 29.6%. The most common bacterial species werePseudomonas aeruginosa, Klebsiella pneumoniae, Acinetobacter baumannii, Escherichia coli, Stenotrophomonas maltophiliaand Staphylococcus haemolyticus. Non-fermentative bacteria accounted for 53.2% of all the gram-negative bacterial isolates. All theEnterococcus and
Staphylococcus isolates were susceptible to linezolid, vancomycin and teicoplanin. All theStaphylococcus strains were resistant to methicillin.P. aeruginosa isolates were relatively more susceptible to cefoperazone-sulbactam, piperacillin-tazobactam and cefepime (>70%) than imipenem (40.8%) and meropenem (59.2%). All theK. pneumoniae isolates were susceptible to imipenem and meropenem and more than 70% of the isolates were susceptible to cefoperazone-sulbactam, amikacin. More than 80% of theA. baumannii isolates were susceptible to carbapenems, cefoperazone-sulbactam, amikacin, ciprolfoxacin and aminoglycosides. All the E. coli isolates were susceptible to carbapenems and more than 70% were susceptible to cefoperazone-sulbactam and ceftazidime. More than 90% of theS. maltophilia strains were sensitive to levolfoxacin, minocycline, cefoperazone-sulbactam and trimethoprim-sulfamethoxazole.Conclusions Our data suggest that gram-negative bacteria, especiallyEnterobacteriaceae and non-fermentative bacteria, are still the primary pathogens in febrile neutropenic patients. Antimicrobial resistant strains are prevalent. Such data of bacterial species and antimicrobial susceptibility proifle of pathogens in febrile neutropenic patients are useful for empirical antimicrobial therapy of such infections.

Objective To understand the current state of community nurses in Jilin Province and its nursing training so as to provide basis for constructing community nursing training system. Methods 583 community nurses from community health care service centers (stations) in three cities of Jilin Province were investigated with cluster random sampling method to acquire their proficiency level of community nursing knowledge and its demand. Results Common basic nursing knowledge was best mastered by community nurses (63.8%), while knowledge about scientific research was worst mastered (13.6%). Community nurses who were young, in low positions, with work experience for 5 to 9 years, mastered the knowledge worse, 42.9% of them did not attend training. The commonest training problem was that the training period was too short (64.6%); the highest demand was common diseases training for the elderly. Conclusions Nursing training is an important way to improve overall quality of community nurses in the light of unbalance status of their quality in our province and training not being put into place.
