4.A Case of Infantile Acute hemorrhagic Edema.

Infantile acute hemorrhagic edema is a benign disease followed by a spontaneous eomplete recovery without sequelae, characterized by cockade purpura and inflamrntory edema on the face and extremities without any internal origin involvement in 4-month to 2-year-old infants. We report a case of infaniile acute hemorrhagic edema which occared in a 2-year-old female after an upper respiratory illness and compare the clinical and histologic features of this disorder with Henoch-Schonlein purpura.

5.A Case of Acute Graft-versus-Host Disease.

We report a case of acute graft-versus-host disease, which developed after bone marrow transplantation because of acute myelocytic leukemia in a 39-year old male, The pruritic, erythematous maculopapular eruptions began to developed on the perioral regions, and spreaded the face, the oral mucosa, both hands, and buttocks at the twenty fourth day after bone marrow transplanta.tion. The eruptions were confluent to form erythematous patches. Iistopathological findings show parakeratosis, lymphoid cell exocytosis, and papillary edema, lymphohistiocytic infiltration, and melanophage in the upper dermis, and basal vacuolation. He was treated systemically by methylprednisolone, and antilymphocytic globulin, and tapically by emollients and steroids.

6.Clinical Observation on the Renal Tuberculosis.

A clinical observation was made on 54 cases of renal tuberculosis who were admitted to the Department of Urology, Soon-Chun-Hyaing College Hospital during the period from January, 1977 to December, 1981. The following results were obtained. 1. The incidence of renal tuberculosis was 7.5% of all urological in-patients. 2. The most common age group was 20-49 years (68.5%) and male to female ratio was 1.8:1. 3. Both kidneys were affected in 8 cases (14.8%), while 26 cases (48.2%) in right and 20 cases (37%) were in left kidney. 4. The most frequent subjective complaints was frequency in 62.9% and hematuria in 40.7%, flank pain in 38.9%, painful urination in 29.6%, nocturia in 27.8% in order of their frequency. 5. Eleven (20.4%) of 54 cases were associated with pulmonary tuberculosis. 6. Urinary findings revealed pyuria in 68.5%, proteinuria in 66.7% and hematuria in 53.7%. 7. On the intravenous pyelogram, non-visualization of the kidney was found in 31.5%, hydronephrosis in 24.1%and destruction of calices in 20.4%. 8.Cystoscopic findings showed active lesions of cystitis in 47.9%, fibrosis with trabeculation in 20.8%, deformed ureteral orifice in 18.8% and contracted bladder in 14.6%. 9. Nephrectomy was done in 26 cases (48.1%), ureteric reimplantation in 2 cases. cutaneous ureterostomy in 1case and nephrostomy in 1 case.

8.A Case of Tethered Cord Syndrome Manifested with a Neurotrophic Ulcer.

We report herein a case of tethered cord syndrome with recurrent ulcerations of the skin in a 17-year-old girl. The patient presented an asymptomatic punched-out deep ulcer on the right buttock and right heel. The lesions were noticed as bullae on the sites, which developed into ulceration. At birth, a reddish nodule was noted on the lower back, which was excised when she was 3 years old at a private clinic. Neurological examination revealed hypoesthesia below the LS dermatome. We suspected a simple neurotrophic ulcer which was treated with zinc oxide dressing and oral antibiotics. 1 month later, the ulcers had healed, but tense bullae had developed on the same right buttock. Spina bifida with intraspinal lipoma was revealed by plain X-ray film and magnetic resonance imgaing. Tethered cord syndrome was diagnosed, and treated with a surgical excision of the lipoma, and repair of the dura. This is a vary rare etiology of a neurotrophic ulcer due to a central nervous system disease, and is the first reported case in the Korean dermatologic literature.

9.Clinical Observation for Complications of Transurethral Resection in the Treatrnent of Benign Prostatic Hyperplasia.

A clinical observation for operative, postoperative complications were made from 61 case of TURP from June 1985 to April 1989. The following results were obtained. 1. Mean operative time was 81.5 minutes and mean weight of resected prostatic tissue was 13. 6gm. Therefore, it had taken average 6.97 minutes to resect 1gm of prostatic tissue. 2. Perioperative blood replacement was performed in only 16 cases except one case of death and the mean amount was 0.66 pint (210ml). The remaining 45 cases (73.8 %) were not necessary to transfuse blood. 3. Postoperative serum sodium concentration was decreased in 38 of 60 cases and not changed in 8 cases and increased in 14 cases. These changes in serum sodium concentration showed no significant correlation with operative, amount of irrigating fluid. 4. Nineteen of 61 cases of TURP showed several types of non-fatal complications and one case of these expired and the rest of people showed' no complication. 5. The most common complication was transient urinary incontinence (11.4%), the remainders were in order of urethral stricture (6.6%), late bleeding (4.9%), re-TURP due to inadequate resection (3.3%), death (severe bleeding) (1.6%).

10.A Clinical Observation on the Injuries of the Genitourinary Tract.

A clinical observation was made on the injuring of genitourinary tract of in-patients in the Department of Urology, Soon-Chun-Hyang College Hospital during the period from January. 1977 to December, 1980 for 4 years. There results were as follows; 1. Of 505 cases hospitalized, 86 cases have shown injuries of genitourinary tract, giving a ratio of 17%. 2. The age ranged from 4 to 67 years, and of these cases the favorable ages were those from 11 to 40 years for 73.3%. 3. Traffic accident was the most frequent cause of the injuries it 54.7%. 4. The kidney was involved most frequently (41.9%) and the involvement of the urethra was 27.9%. 5. Nephrectomy was performed in 6 cases (16.7%) of 36 renal injuries. 6. The ureteral injuries were occurred by traffic accident in 1 case and during uterine surgery in 1 case. 7. Six cases of 15 bladder injuries and 14 cases of 24 urethral injuries were accompanied with pelvic bone fracture.
