2.Treatment of Traumatic Carotid-Cavernous Fistulas using Debrun's Detachable Balloons.

The goal of therapy in patients with traumatic carotid-cavernous fistulas is to occlude the fistula preferably while maintaining the carotid blood flow. Since the introduction of the concepts of detachable balloon technique to occlude arteriovenous fistulas, the technique has become the treatment of choice in the management of traumatic carotid-cavernous fistulas. The major symptoms of traumatic CCFs are (1) pulsating exophthalmos, (2) orbital and cephalic bruit and murmur, (3) headache, (4) chemosis, (5) extraocular palsies, and (6) visual failure. Traumatic CCFs are combined with multiple associated lesion. We tried the occlusion of fistulas using Goldvalve balloons in 8 consecutive cases of traumatic CCF and the result of our experience is reported. Transarterial approach with manually-tied latex balloons is tried in all cases and the fistulas was successfully occluded in all cases. In 5 cases, the internal carotid artery was preserved and the arterial lumen was occluded along with fistula opening in cases. In one case, surgical ligation was done because of symptoms recurred and incomplete occlusion of fistula. We experienced hemiparesis as a major complication in one case during occlusion tolerance test, which was remitted spontaneously. The results of Debrun balloon treatment were relatively excellent. We consider that the first choice of treatment of traumatic CCF is occlusion of the fistula by a detachable balloons.

5.A Study on Dietary Attitudes and Dietary Self-Efficacy of Juvenile Delinquents.

The purpose of this study was to identify the factors associated with dietary attitudes and dietary self-efficacy of juvenile delinquents. Out of 110 questionnaires distributed to the juvenile detention home students in Daegu, and 150 questionnaires distributed to the juvenile detention home students in Anyang, 210 were analyzed (80.7% analysis rate). These data were analyzed by SPSS program. The results are summarized as follows. Almost all parents' educational levels showed 'graduation of middle school', and only 43.8% of the juvenile detention students were living with parents. In case of incoming status, average income per month was 2,000,000 won, which was lower than normal one. Diet attitude score was higher in boy students than girl students (p < 0.05). There was a higher score of diet attitude in higher educational levels of father. The average score of dietary self-efficacy was 2.51 +/- 0.86. This value was lower than the normal juvenile. There was a higher diet attitude score in the higher dietary self-efficacy score (p < 0.001). Therefore, proper nutrition education is required to improve their dietary self-efficacy. The results of this study could be useful data to plan and develop nutritional education programs of juvenile delinquents

6.A Case of Group O without Anti-A,B due to hypogammaglobuluinemia.

An ABO discrepancy was observed in a 19-year-old man clinically diagnosed as congenital hypogammaglobulinemia. The patient's red cell was typed as group O, and his serum had no ABO isoagglutinins. Absence of A antigen and B antigen on patient's RBC was confirmed by adsorption- elution test and saliva test. A-transferase and B-transferase activities were not demonstrated in patient's serum. Patient's serum protein electrophoresis revealed hypogammaglobulinemia pattern, and immunoglobulin levels were markedly decreased. Complete absence of B lymphocytes was observed on patient's lymphocyte subset profile. Patient's father and mother were typed as blood group O and no abnormalities were recognized in their serum protein electrophoresis patterns and immunoglobulin concentration.

7.Multiple hepatic adenomatosis: a case report.

We expirenced multiple hepatocellular adenomatosis, which was proved by pathology, in 38 years old man who had no prior history of predisposing factors. The radiologic findings were different by the size of tumor mass and intratumoral hemorrhage. Ultrasound demonstrated inhomogeneous hyperechoic large mass in segment 5 of right lobe and hyperechoic or isoechoic multiple small nodules in right lobe. Computed tomography demonstrated low density mass without contrast enhacement. Central high density is noted in hemorrhagic portion. Magnetic resonance T1-weighted image demonstrated high signal intensity in mass and hemorrhagic portion. T2-weighted image demonstrated slightly high signal intensity in mass portion, high signal intensity in hemorrhagic portionand double-layered peripheral rim. Gradient echo image well demonstrated inhomogenesity of mass.

9.Delayed Development of Pulmonary Embolism after Total Hip Replacement: A case report.

Though anticoagulant therapy has been shown to improve outcomes dramatically, pulmonary embolism is a potentially fatal disease. A 82 years old female underwent elective operation for left femur neck fracture under general anesthesia. At the twenty-two postoperative days, she suddenly developed cyanosis with hypotension. She was transferred to intensive care unit and pulmonary embolism was diagnosed by pulmonary perfusion scan and echocardiography. Despite of diagnosis and treatment of pulmonaly embolism, she expired 29 hours after onset of symptom.

10.Laparoscopic Adrenalectomy: A Comparison with Open Adrenalectomy.

PURPOSE: The advent of laparoscopic abdominal surgery made it possible to perform a laparoscopic adrenalectomy (LA). We have successfully performed 10 such operations since November 21, 1995. We retrospectively compared the effectiveness and the safety of a laparoscopic adrenalectomy with the effectiveness and the safety of an open adrenalectomy (OA). METHODS: The 45 patients with adrenal diseases who underwent an adrenalectomy from March 1990 to January 1998 were included in this study. Ten (10) laparoscopic cases comprised the study group, and 17 of the 32 open adrenalectomy cases, performed by anterior approach, comprised the control group. RESULTS: No one died from an adrenalectomy. Nine cases (45%) from the OA group needed transfusions (average: 661 ml), but one patient from the LA group, who had required a conversion to a celiotomy, required a transfusion (400 ml). The operating times were, on average, 191 minutes in the OA group and 230 minutes in the LA group (p=0.0384). The average size of the masses was 6.5 cm. The first days of walking after the operation was 3.4 days for the OA group and 1.3 days for the LA group (p=0.0002). The patients' former regular diets were started on days 6.9 and 3.2 for the OA and LA groups, respectively (p=0.0001). Postoperative in-hospital days were 17.5 for the OA group and 7.9 for the LA group (p=0.0001). CONCLUSION: In comparison with an open adrenalectomy, a laparoscopic adrenalectomy showed better convalescent features: that is, less chance of transfusion, earlier recovery, faster resumption of the former diet, shorter postoperative hospital stays, and cosmesis. We think and suggest that a laparoscopic adrenalectomy is a safe and effective procedure, and hopefully it will become the standard choice of operation for various adrenal diseases.
