2.A Study of Mucocutaneous Manifestations Due to Cancer Chemotherapy.

BACKGROUND: Recently, inireaing numbers of chemotherapeutic agens are being used to treat cancer patients. Mucocutaeous complications are commonly sen in association with the administration of these medicaticins. OBJECTIVE: We conducted a study in cancer chemotherapy patient so determine the suspected chemotherapeutic agents ancl frequency of various mucocutaneous side effects in these patients. METHODS: The study involved 140 patients admitted from Decerrae 1993 to September 1994 for cancer chemot herapy at Korea University Guro Hospital. RESULTS: The mucocutaneous side effects during chemotherapy were alopecia(55%), hyperpig mentation(32.9%), stomatitis(20%), phlebitis(12.9%), flushing(8.6%) descending order. Visual grade II, IV alopecia patients were most common, but as the chemot retpy cycle increased so, the severer the alopecia. Although the onset of the alopecia were variake many patients experienced alopcia within 17 to 24 days after the start of chemotherapy. Serpeitie supravenous fluorouracil hyperpigmentation were commor,(17.9%), Diffuse and longitudinal or horizontal band like nail pigmentation were observed, and, pigmented macules were also observei, epecially on the palms and digits. Stomatitis usually developed within 10 days after the start of chemotherapy and it persist ed for about 7 days and the most of the lesions were resolved spor Laeously. Other clinical manifestations accompanied with chmotherapy were tinea infection(16.4%), acne(7.1%), oral thrush (7%), white nail band(4.3%), pruritus(2.9%) in descending order. CONCLUSION: So, physiciars need to be aware of the widely divergnal cutaneous reactions which may occur with the use of antiancer medications.

3.A Case of Disseminated Candidiasis with Skin Manifestations.

Candidiasis is the most common fungal infection complicating the course of patients with hematologic malignant neoplasms. Although widespread organ involvement is characteristic of disseminated candidiasis, reports of skin are rare. Reports describing typical clinical and histopathological finding of cutaneous lesions are very important since it may enable a diagnosis of disseminated candidiasis to be made and thus antifungal therapy can be initiated earlier. A 50-year-old housewife was admitted with a 5-month history of fatigue and easy bruising. She was diagnosed as ha.ving acute myelocytic leukemia and treatment was begun with daunorubicin and cytosin, arabinoside. Eight days after the start of chemotherapy, she developed a fever and generalized tender well demacated erythematous to purplish papulonodular eruption. A biopsy specimen from the skin lesion showed perivascular mononuclear cell infiltration and spore and pseudohypae v,rere found within the dermis and subcutis in PAS stain. Cultures of one skin biopsy specimen and one of four blood sarnples grew Candida tropicalis. The patient was treated with intravenous amphotericin B for disseminated candidiasis. On the tenth day of antifun gal therapy, she developed cardiopulmonary arrest and died.

4.153 Cases of Laser Lithotripsy.

The pulsed laser lithotripsy has been used in the treatment of urinary calculi. We treated 153 patients of stones, via the pulsed dye laser(Technomed Pulsolith) with 7.5 and 6.5 F. rigid ureteroscope between January 1992 and January 1995. Stones were in bladder(4 cases), upper ureter(17), mid ureter(31) and lower ureter(101). The laser lithotripsies were applied for impacted stones and relatively large stones(more than 6 mm) The success rates according to location were 47%(8/17) in upper ureter, 77%(24/31)in middle ureter, 95%(96/101)in lower ureter and 100%(4/4) in bladder and mean success rate was 86% (132/153). There were 21 failures due to upward migration(9 cases), poor visual field(7), laser resistant stone(3) and malfunction of laser(2). Complications were showed in 28 cases that were gross hematuria(16 cases), ureteral perforation(6), fever(4) and ileus(2), but most of them might be related to ureteroscopy rather than laser, and all of them were resolved with conservative management. So laser lithotripsy is safe and effective method of middle and lower ureteral calculi, and upward migration of stones, the major cause of failure may be resolved by use of extracorporeal shock wave lithotripsy and flexible ureteroscope.

7.A STUDY OF BONE APPOSITION AND MARGINAL ALVEOLAR BONE LOSS AROUND IMMEDIATE IMPLANSTS.

The purpose of this study was to observe bone apposition and marginal bone loss and to check the possibility of success on titanium implant, HA coated implant and the implant with natural coral that were place immediately after teeth extraction in dogs. Experimental subjects were divided into 4 groups ; the 1st group is the titanium implant, second the HA coated implant, third the implant with natural coral, and the last the control group that was prepared in the extraction sockets. After 12 weeks, the dogs were sacrificed for visual observation and microscopic examination approaching histologic and histomorphometric analysis. The results were as follows : 1. Neither the infection nor the exposure of implant was found at the sites of all implant. 2. In a histomorphometric analysis, mean percentage of direct bone contact with the titanium implant was 80.7% and the HA coated implant showed 81.5% apposition, but the implant with natural coral showed 64.9% apposition(P<0.05). 3. In a microscopic examination, mature lamellated bone was found around the immediate implants and control group, while unabsorbed natural coral around the immediate implants with natural coral was found. 4. All immediate implant groups showed the loss of marginal bone in order from implant with natural coral, titanium implant, and HA coated implant. 5. Implant with natural coral that was placed by the type I interface of the Barzilay's classification immediately after teeth extraction showed low percentage of direct bone contact area, low success rate and a lot of marginal bone loss. Above results suggested that the immediate implants are osseointegrated successfully, although slightly marginal bone was loss.

8.Treatment Using Unreamed Intreamedullary Nailing for Closed and Open Tibial Fractures.

PURPOSE: The purpose of this paper was to evaluate the treatment results according to bone union, union time, and complications, including infection of unreamed nailing of tibial fractures between closed and open fractures. MATERIALS AND METHODS: We reviewed 64 tibial shaft fractures that were treated with unreamed tibial nail. These included 42 closed fractures and 22 open fractures. RESULTS: Average union time of closed fractures was 19.8 weeks and that of open fractures was 20.2 weeks, nonunion rate were 4/42 and 3/22 in closed and open fractures. Average union time were 19.2, 20.4, 21.3 weeks in open grade I, II, llla fractures. According to the type of fractures, average union time were 18.5, 20.2, 24.6 weeks and nonunion rate were 2/29, 3/26, 2/9 in type A, B, C fractures. According to the level of fractures, average union time were 20.0, 20.3, 19.4 weeks and nonunion rate were 1/5, 4/37, 2/22 in proximal, middle, and distal fractures. There was no significant differences in average period of radiologic union, infection rate and nonunion rate between closed and open fracture group, but longer union time and higher nonunion rate in complex and comminuted fractures (P<0.05). CONCLUSIONS: We consider unreamed intramedullary nailing in the tibial shaft fractures as a good treatment modality for closed and open grade I, II, IIIa fractures

9.Different Clinical Features of Organophosphate Insecticides Intoxication According to The Route of Administration: Disparity Between Clinical Severity And Plasma Cholinesterase Level.

BACKGROUND: Organophosphate insecticides poisoning is one of the most common toxicologic emergencies in Korea. There have been few reports of organophosphate intoxication via parenteral route, although many reports on organophosphate intoxication by oral ingestion were present in the literature. This study aimed to validate the clinical characteristics of organophosphage intoxication according to the route of administration to the body. METHOD: Data were collected retrospectively by the review of the medical records from 49 patients with organophosphate intoxication. Severity of intoxication was classified by the Namba's Classification. Collected data were analysed and compared on the clinical features and laboratory findings between the patient intoxicated by inhalation or contact(parenteral group, n=23) and the other oral ingestion(enteral group, n=26). RESULTS: Severity class by clinical features was higher in enteral group than parenteral group. Severity class by serum cholinestetrase level was not positively correlated with severity class by clinical manifestations. Cholinesterase level tended to overestimate the severity of intoxication in parenteral group. Ventilator therapy and admission to intensive care unit were more frequently needed in enteral group than parenteral group in case that the severity class by clinical features was equal. CONCLUSION: In patients with organophosphate intoxication by parenteral route, serum cholinesterase level of the patient had disparity with clinical severity of intoxication. Considering this disparity, clinical severity should be considered as a more important indicator for treatment of organophosphate intoxication including atropinization, rather than serum cholinesterase level in patients intoxicated by parenteral route.

10.Fracture strengths of ceromer crowns supported on the various abutment core materials.

STATEMENT OF PROBLEM: The effects of various core buildup materials which differs in the mechanical properties on the fracture strength of metal-free crowns is unknown. PURPOSE: This study was carried out to evaluate the fracture strengths of Artglass ceromer crowns supported by 3 different core materials in clinically simulated anterior tooth preparation. Material and methods. Ten crowns from each group were constructed to comparable dimensions on the various dies made by gold alloy, Ni-Cr alloy, and composite resin. The ten crowns were then cemented onto the dies and loaded until catastrophic failure took place. Fracture resistance to forces applied to the incisal edges of the anterior crowns supported by three types of dies was tested. RESULTS: The ceromer crowns on the composite resin dies fractured at significantly lower values(287.7 N) than the ceromer crowns on the metal dies(approximately 518.4 N). No significant difference was found between the fracture values of the ceromer crowns on the dies of gold alloy and Ni-Cr alloy. CONCLUSION: The failure loads of the ceromer crowns on the metal dies were almost the same and not affected by the differences of casting alloys. However, the fracture values of the ceromer crowns on the resin dies were significantly reduced by the relative weak properties of composite resin core material.
