1.Complications after lumbar myelography with amipaque

Amipaque is a water soluble, non-ionic myelographic contrast media, and owing to its high diagnostic accuracy and safety, its use is gradually increasing. The authors studied the complications after Amipaque lumbermyelography in 61 patients with low back pain during the period from Jan. 1981 to Nov. 1981 in Chung Ang University Hospital. The results were as follows; 1. Total complication rate was 52%(32 of 61) and there was nosexual difference in its occurrence. 2. In total, no difference in complication rate was found between head-up positioned group with a degree of 30degrees (group I) after procedure and head-up positioned group with a degree of 70degrees (group II) but female patients had more complication rate in group I than in group II (75% vs 50%).Headache was more common in group I and nausea was more common in group II. 3. Headache was most common complicaiton (44%) and there was no sexual difference in its occurrence. 4. No significant difference in complication rate was found between patients proved to have HBP and patients to have not. 5. Complications were less common in patients with punctured level of L4-5 than in patients with L2-3 or L3-4 level puncture.

2.A Study of Mucocutaneous Manifestations Due to Cancer Chemotherapy.

BACKGROUND: Recently, inireaing numbers of chemotherapeutic agens are being used to treat cancer patients. Mucocutaeous complications are commonly sen in association with the administration of these medicaticins. OBJECTIVE: We conducted a study in cancer chemotherapy patient so determine the suspected chemotherapeutic agents ancl frequency of various mucocutaneous side effects in these patients. METHODS: The study involved 140 patients admitted from Decerrae 1993 to September 1994 for cancer chemot herapy at Korea University Guro Hospital. RESULTS: The mucocutaneous side effects during chemotherapy were alopecia(55%), hyperpig mentation(32.9%), stomatitis(20%), phlebitis(12.9%), flushing(8.6%) descending order. Visual grade II, IV alopecia patients were most common, but as the chemot retpy cycle increased so, the severer the alopecia. Although the onset of the alopecia were variake many patients experienced alopcia within 17 to 24 days after the start of chemotherapy. Serpeitie supravenous fluorouracil hyperpigmentation were commor,(17.9%), Diffuse and longitudinal or horizontal band like nail pigmentation were observed, and, pigmented macules were also observei, epecially on the palms and digits. Stomatitis usually developed within 10 days after the start of chemotherapy and it persist ed for about 7 days and the most of the lesions were resolved spor Laeously. Other clinical manifestations accompanied with chmotherapy were tinea infection(16.4%), acne(7.1%), oral thrush (7%), white nail band(4.3%), pruritus(2.9%) in descending order. CONCLUSION: So, physiciars need to be aware of the widely divergnal cutaneous reactions which may occur with the use of antiancer medications.

3.A Case of Bowen' s Disease of the Nail Bed Presenting as Longitudinal Melanonychia.

Bowens disease is an intraepithelial squamous cell carcinoma. It occur s anywhere on the surface of the body. Bowens disease involving the nail bed epithelium is very rate. Recently, some literatures showed that human papillomavirus is sssociated with squamous cell carcinoma of the finger by in situ hybridization or polymerase chain reaction. A 30-year-old housewife deve]oped longitudinal melanonychia with as imptomatic fissured nail plate on her right third finger. The hiat,ologic specimen showed diffuse anaplastic change confined to the nail bed epithelium. The subungual eyithelium was found to contain human papillomavirus DNA type 31/33/35 with the use of in situ hybridization.

4.A Case of Basal Cell Epithelioma Arising in a Nevus Sebaceus during Childhood.

The potential for sebaceous nevi to evolve into secondary benign or malignant EuIror is well recognized. In general, malignant transformation does not occur until postpubertal life. We experienced a case of basal cell epithelioma arising in a nevus sebaceous on a 12-year-old boy´s parietal scalp. We recommended that surgical excision should be undertaken as early as possible to detect the malignant transformation of nevus sebaceous.

5.A Case of Sporotrichosis Associated with Lupus Vulgaris.

Sporotrichosis is a chronic infection that follows accidental implantation of the fungus Sporothrix schenckii into the skin. The infection begins with skin trauma under conditions when conidia may be present. A 48-year-old woman presented with a 5-year history of an 8X 12 cm sized, centrally depressed, erythematous plaque on the left wrist. She had been treated with antifungal agents at an other hospital for about 6 months but there was no improvement. The histopathological findings showed an infiltration of mononuclear cells and tuberculoid structures on the whole dermis and subcutis. According to the histopathological findings and clinical manifestations, she was diagnosed as having lupus vulgaris. There was much improvement of the lesion after antituberculous therapy. However, 2 months later she developed asymptomatic multiple erythematous papules around the lesion of lupus vulgaris and along the lymphatic drainage of the left arm. The histopathological findings of the newly developed lesion showed a granulomatous reaction and an infiltrate of acute and chronic inflammatory cells. Moist and creamy colonies were changed into brown-to-black ones with a leathery appearance with age. This appearance and clinical manifestations confirmed the diagnosis of lymphocutaneous sporotrichosis. This clinical appearance reflects that the sporotrichosis developed on the ulcerative lesion or biopsy site of lupus vulgaris.

6.A Clinical Observation of Ocular Injuries of Inpatients.

The author analyzed 514 patients with ocular injuries among 2676 inpatients who were admitted to Chosun University Hospital from January 1986 to December 1991. The results were as follows: 1. The incidence of ocular injuries was 19.2% of all inpatients. 2. The incidence was more common in male (82.6%) and in the order of 3rd decade (26.7%), 4th decade (20.7%) and teenage (16.4%). 3. The ocular injuries were more common in the spring (31.9%), and in March. 4. The most common cause of ocular injuries was industrial accident (25.5%), followed by individual accident, traffic accident and violence. Trauma inflicated while staging demonstration was 6.6%. 5. The most common ocular injury was traumatic hyphema (21.0%), followed by corneal laceration, canalicular disruption, and lid laceration. 6. The most common surgical procedure was corneoscleral suture (34.6%), followed by primary lid suture and canalicular reconstruction. 7. The final visual acuity was improved to 0.5 or more in 41.4%. 8. The most common complication of ocular injuries after treatment was corneal opacity (41.2%), followed by vitreous opacity and traumatic cataract.

8.Clozapine Induced Neuroleptic Malignant Syndrome.

Neuroleptic malignant syndrome is a rare, but potentially life-threatening adverse event associated with the use of neuroleptic agents. We describe the case of a 47-year-old schizophrenic woman who was treated with clozapine for years. The patient developed acute renal failure with pulmonary edema, and underwent mechanical ventilation and hemodialysis.

10.Two cases of allergic contact dermatitis to sodium fusidate.

Sodium fusidate, obtained by fermentation of the fungus, Fusidium coccineum, has a steroid structure and shows a very high antitaphycoccal activity. The allergic potentirl of sodium fusidate is low and few cases of contact allergy to sodium fusidate have been reported. We present two cases of allergic contact dermatitis to Fucidin cintentione developed postoperative dressing of a skin biopsy and the other after laser treatment of a vascular nevi. The patch test results showed positive reaction to sodium fusidate in both cases.
