Journal of the Korean Ophthalmological Society 2001;42(5):783-787
PURPOSE: Porous orbital implant Medpor(R) which is made of polyethylene is being widely used currently. Compared with previously used porous orbital implants, it is somewhat preferred due to cheaper and easier usage but reports on its postoperative complications are not sufficient. We report the first case of Medpor(R) infection developed domestically. METHODS: A 20 year-old male patient was admitted with Medpor(R) infection 14 months after Medpor(R) orbital implant insertion. He was treated with antibiotics. RESULTS: Infection was controlled with antibiotics without implant removal. Implant exposure occurred after infection control so dermis graft was done.

The increasing use of intravenous polyethylene catheters has led to a growing incidence of accidental catheter breakage and migration of fragments into the central vascular system usually in the inferior vena cave, the right atrium, of the pulmonary artery. The most common complications were formation of a thrombus in the area of the foreign body, infection with endocarditis, and perforation of the heart. And so obviously the polyethylene catheter must be removed. In 1967, Massumi and Ross et al have been successful in removing a catheter fragment from the right atrum percutaneously with a snare device. With some modification as suggested by Curry, method of retrieving fragments of polyethylene catheters from the heart without chest surgery utilize wire snares or endoscopic forceps. This report describe the technique and its use in successfully managing these complications in four consecutive cases.

Journal of the Korean Ophthalmological Society 1997;38(6):1044-1049
We studied the utility of hand held autorefractometer on the cycloplegic refraction for poor coorperated children by analyzing the data taken from hand held autorefractometer(Retinomax) and Canon R-10 autorefractometer. Seventy patients(140 eyes) were studied. We divided them into two groups according to the age: Group I included patients less than 4 years, Group II over 4 years. There were no statistic significant differences in the spherical and cylindrical values regardless of age and cooperability in cases of less than 6 diopter but statistic significant difference in cases of high refractive errors (>6 diopter) or cylindrical axis values. In Conclusion, this study revealed that hand held autorefractometer was useful for checking refraction of the poor coorperated children and the spherical and cylindrical values of Retinomax have no differences from the values of R-10 in the cases of less than 6 diopter.

Journal of the Korean Ophthalmological Society 1998;39(2):221-226
PURPOSE: Orbital computed tomography was useful for evaluation of orbital truama because it can visualize most soft tissues, foreign bodies and bone details. Advancements in computer software makes three diemensional CT available with more higher quality and reasonable cost. We studied the officency, merit and limitation of orbital three dimensional CT. METHODS: We evaluated the conventional orbital CT and three dimensional orbital CT of the patients visited department of ophthalmology and emergency room to orbital trauma or other orbital diseases. RESULTS: The three dimensional orbital CT was not superior to conventional orbital CT in evaluation of orbital soft tissue disease but useful for evaluation of orbital bony details. CONCLUSION: In cases of coexamination with conventional orbital CT, the three dimensional orbital CT was useful for diagnosis and surgical planning in orbital truamatized patient.

Journal of the Korean Ophthalmological Society 1997;38(4):637-642
Cytomegalovirus retinitis (CMV), an opportunistic infection of a immunocompromised patient, is a necrotizing retinitis occurred in AIDS or the patient using immunosuppressants. A 31-years-old male taken immunosuppressants after renal transplantation, had white granular foci and perivascular sheathings in the uper temporal retina of both eyes. Serologic test demonstrated positive response to bith IgG and IgM antibody against CMV. Argon laser photocoagulation was done and ganciclovir was intravenously treated 300 mg every 12 hours for three weeks without maintenance therapy. Nine months after treatment, there was no recurrence of retinitis. This result suggests CMV retinitis of renal transplanted patient may not need the maintenance therapy of ganciclovir.

Journal of the Korean Ophthalmological Society 2001;42(7):933-938
PURPOSE: This study was to observe the clinical outcome and complications of Medpor(R) orbital implant. METHODS: The clinical outcome and complications were studied retrospectively in 64 eyes with Medpor(R) implantation. Evisceration was done in 34 cases, enucleation in 19 cases, and secondary implantation in 11cases. Mean follow up period was 16 months(4~30 months) and mean age of the participants was 34.9(1~79). RESULTS: Implant infection developed in one case, implant exposure developed in four cases and superior sulcus deformity developed in two cases. Motility Coupling Post(MCP) was inserted in 26 eyes. Motility of the prosthesis was good in most cases. CONCLUSIONS: Medpor(R) orbital implant is being widely used currently. Complications or motility of the prosthesis showed no significant difference from those of Hydroxyapatite. But low cost and its handiness in operation makes it a desirable orbit implant.

Journal of the Korean Ophthalmological Society 2004;45(8):1386-1391
PURPOSE: We report a case of sphenoid and ethmoid mucocele causing orbital apex syndrome, which was successfully treated with drainage under nasal endoscopy. METHODS: A 82-year-old female visited the hospital complaining of decreased visual acuity in her left eye for 15days. Visual acuity of the left eye was hand motion . She also complained of left ocular pain and headache. In ophthalmologic examination, exophthalmos, mild ptosis, extraocular muscle movement limitation and RAPD (relative afferent pupillary defect) of the left eye were found. Orbital CT showed a 3.3 X 2.9 X 4.1 cm sized well-demarcated cystic lesion involving the left sphenoid sinus, left ethmoid sinus, and posterior portion of the right ethmoid sinus. The ethmoid air cell was remodeled by this cystic mass. The medial wall of the left orbit was protruded outward. So, the medial rectus muscle and optic nerve in the left orbit were compressed. We found a bulging yellowish cystic mass supero-posterior to the middle turbinate under nasal endoscopy. We performed incision and drainage under nasal endoscopy. RESULTS: At post-operative 2days, orbit CT showed the removed large mucocele in the sphenoid and ethmoid sinuses. At post-operative 2weeks, corrected visual acuity of the left eye was 0.6. There was no exophthalmos, ptosis, or extraocular muscle movement limitation. CONCLUSIONS: Endoscopic surgery in the treatment of paranasal mucocele can be performed during a short time under local anesthesia and anatomical change of the paranasal sinus can be minimized. We therefore suggest that drainage through nasal endoscopy is effective method in the treatment of paranasal sinus mucocele.

The Mayer-Rokitansky-K ster-Hauser syndrome represents a spectrum of m llerian anomalies, including vaginal agenesis with or without renal anomalies, in genotypically and phenotypically normal female subjects with normal endocrine status. We experienced a case of this anomaly which combined with unilateral renal agenesis and pelvic cystic mass in child.
