2.A Clinical Analysis of Vitrectomy for the Proliferative Diabetic Retinopathy Patients.

The authors reviewed the charts of 128 patients(160 eyes) who underwent vitrectomy for the proliferative diabetic retinopathy from January 1993 to December 1995 and the results were analyzed in terms of visual efficiency. At the conclusion of the study, visual acuity improved in 96 eyes(60%), unchanged in 33 eyes(21%) and in 31 eyes(19%) became worse. Visual efficiency was increased from 11.9% preoperatively to 37.2% postoperatively in all patients. Patients under the age of 40 showed better results than the patients above the age of 40. The factors which affected the change in visual efficiency were age and preoperative intraocular conditions. The patients who had non-clearing vitreous hemorrhage without proliferative membrane revealed better prognostic results than the other patients. In 50 eyes (31.3%) of cases, maximal postoperative visual acuity was achieved in 4 weeks to 3 months after operation.

3.Relationship among the Expression of Cyclin D1, p21, and p53 Protein, and Prognosis in Non-Small Cell Lung Carcinomas.

Recently, cell cycle regulators have been suggested as new prognostic factors of the lung cancer. In this study, we evaluated the expression of cyclin D1, p21, and p53 using the X2-test, with regard to the stage of the patients, histologic type, and histologic differentiation in the 135 cases of non-small cell lung carcinomas (NSCLC). To evaluate the confounding effects among cyclin D1, p21, and p53 on X2-test analysis, we used the Mantel-Haenzel test. The NSCLC in this study included 82 cases of squamous cell carcinoma and 53 cases of adenocarcinoma. Each nuclear staining of cyclin D1, p21, and p53 was observed in 65 cases (48.1%), in 54 cases (40.0%), and in 81 cases (60.0%) of NSCLCs, respectively. Only p53 expression was significantly associated with the stage (stage I, II, IIIa) (p<0.05) and squamous cell carcinoma (p<0.05). On the other hand, cyclin D1 expression was significantly associated with the histologic differentiation. The confounding effects among cyclin D1, p21, and p53 revealed that only p21 expression changed the relationship between p53 and stage. In this regard, further study is needed.

4.Clinical Effects of E. cole Derived Authentic REcombinant Human Growth Hormone(DA-3002) in Children with Growth Hormone Deficiency.

BACKGROUND: Recently authentic human growth hormone(hGH) has produced in the E coli K-12, W3110 by recombinant DNA tecbnology in Korea In this paper, the clinical efficacy and immunogenicity of this GH was shdied in 38 children with growth hormone deficiency during therapy of 1 year. METHODS: The subjects of this study were aged 4.9-13.9 years, diagnosed by failure of plasma GH to respond to insulin-induced hypoglycemia, arginine and/or L-dopa loading and height below -2 standard deviation of mean for their chronological age. Each patient received GH 0.5-0.7IU/kg/week subcutaneously in 6-7 divided doses. During treatment, vital signs, height, body weight and bone age were checked every 3 months. Complete blood count, urinalysis, blood chemistry and thyroid hormone were checked before and every 6 months. The measurement of serum IGF-1 level and antibody against hGH were performed before and every 6 months during therapy of I year. RESULT: The height velocities significantly increased from 3.3 +/- 1.5cm/year to 10.1 +/- 2.5 and 9.0 +/- 1.8cm/year at 6 and 12 months of therapy, respectively. The height standard deviation score for chronological age were significantly improved from -2.141.50 to -1.74 +/- 1.43 and -1.54 +/- 1.38 at 6 and 12 months of therapy with increasing ratio of bone age to chronological age from 0.72 +/- 0.15 at pretreatment to 0.76 +/- 0.15 at 6 month, 0.79 +/- 0.16 at 12 month of therapy. The plasma IGF-1 level significantly increased during treatment. One of 36 patients(2.8%) showed positive antibody against hGH after 1 year of treatment. During therapy of 1 year, unwanted and remarkable clinical side effect were not observed in all subjects. CONCLUSION: These results indicate that this E. coli derived authentic recombinant growth hormone is very effective in stimulating linear growth in children with growth hormone deficiency.

6.Stress distributions at the periodontal ligament and displacements of the maxillary first molar under various molar angulation and rotation: Three dimensional finite element study.

The purpose of this study was to evaluate the stress distributions at the periodontal ligament (PDL) and displacements of the maxillary first molar when mesially directed force was applied under various molar angulations and rotations. A three dimensional finite element model of the maxillary first molar and its periodontal ligament was made. Upright position, mesially angulated position by 20degrees and distally angulated position of the same degree were simulated to investigate the effect of molar angulation. An anteriorly directed force of 200g, countertipping moment of 1,800gm-mm (9:1 moment/force ratio) and counterrotation moment of 1,000gm-mm (5:1 moment/force ratio) were applied in each situation. To evaluate the effect of molar rotation on the stress distribution, mesial-in rotation by 20degrees and the same amount of distal-in rotation were simulated. The same force and moments were applied in each situation. The results were as follows: In all situations, there was no significant difference in mesially directed tooth displacement. Also, any differences in stress distributions could not be found, in other words, there were no different mesial movements. Stress distributions and tooth displacement of the 20degrees mesially angulated situation were very similar with those of the 20degrees distal-in rotated situation. The same phenomenon was obserned between the 20degrees distally angulated situation and 20degrees mesial-in rotated situation. When the tooth was mesially angulated, or distal-in rotated, mesially directed force made the tooth rotate in the coronal plane, with its roots moving buccally, and its crown moving lingually. When the tooth was distally angulated, or mesial-in rotated, mesially directed force made the tooth rotate in the coronal plane, with its roots moving lingually, and its crown moving buccally. When force is applied to an angulated or rotated molar, the orthodontist should understand that additional torque control is needed to prevent unwanted tooth rotation in the coronal plane.

8.Clinical Evaluation of Traumatic Wound Dehiscence Following Cataract Surgery.

In 38 patients with traumatic wound dehiscence after cataract surgery, we evaluated the causes and degrees of the injury, surgical methods of repair and the final visual outcome from Aug. 1993 to Apr. 1977. Among 38 patients, twenty-five were men and thirteen were women. In 34 patients, wound dehiscence occurred within one month after cataract surgery. The common causes of wound dehiscence were trauma by the finger or fist (9 patients) and by the contusion (7 patients). But in 16 patients, the exact causes of trauma were not identified. Accompanied ocular findings were prolapsed iris (29 patients) , hyphema (9 patients) , and dislocation of intraocular lens (7 patients). All patients received operations for wound closure, iris reposition, anterior vitrectomy, intraocular lens reposition or removal, and iris partial resection. At the end of follow-up, 27 patients achieved corrected visual acuity 20/40 or over. To prevent wound dehiscence after cataract surgery, we suggest that protective eye shield should be applied at least for a month after surgery.
