7.Associations of Moyamoya Patients with HLA Class I and Class II Alleles in the Korean Population.

Moyamoya disease is characterized by progressive cerebrovascular occlusion at the peripheral internal carotid artery and development of abnormal collateral circulation at the cerebral basal region. Although abnormal thrombogenesis, inflammation and autoimmune process might be involved in the etiology, the genetic pathogenesis of Moyamoya disease is still unknown. To evaluate the association of Moyamoya disease with HLA alleles in the Korean population, we investigated HLA class I and class II alleles in 28 Moyamoya patients and 198 unrelated healthy controls. The frequency of HLA-B35 allele was significantly increased in the patients compared to the controls (32.1% vs. 10.1%, RR=4.2, p<0.008). Further analysis of HLA-B35 on onset age and sex showed that this allele was significantly increased compared to the controls in both late-onset and female group. Especially, HLA-B35 was the most significantly increased in female of late-onset group compared to the controls. These results suggest that HLA-B35 may be an useful genetic marker for Moyamoya disease, and particularly in females of late onset group in the Korean population.

8.Treatment of Hypertensive Intracerebral Hematoma.

212 cases of hypertensive intracerebral hematoma were treated and followed for more than 3 months. The outcome was analysed in point of comparison between conservative treatment and operative treatment. The precipitating factors on prognosis depend on size and site of hematoma, level of mental deterioration, and blood pressure on admisson. Operative treatment showed better outcome than conservative treatment in cases of severly mentally deteriorated patients, large hematoma, and pontine hematoma.

The purpose of this study is to develop new standards for the disability evaluation with reference to existing laws and other study reports regarding disabilities for the rational evaluation of the diverse kinds of disfigurement in appearance and skin. Three plastic surgery specialists and 3 dermatology specialists developed a new standard for the disability evaluation which is appropriate for circumstances in Korea. Disability rate does not take into account the social occupation, gender or age of the patient, but instead, evaluate the Activity of Daily Living and the social adaptability of the appearance and skin disfigurement regardless of the balance between different disabilities. We tried to include most cutaneous disorders and categorized them into 3 types; congenital (Type 1), acquired (Type 2) as well as any permanent skin impairment sequelae of disease, trauma or treatment process (Type 3). For type 3 disorders, we tried to rate the score according to the size of involved skin lesion. The disability rate is determined by dividing the disability class into 8 steps based on the seriousness of each type of disability.

10.The Outcome of Head Injured Patients in Relation to Glasgow Coma Scale and Secondary Systemic Insults.

A prospective analysis of 106 patients with head injury whose initial Glasgow Coma Scale was 3 to 10, who were managed in a uniform way was analyzed to relate out come to initial Glasgow Coma Scale, combined injuries and secondary systemic insults characterized by serum electrolytes imbalance, arterial gas study, blood pressure, and hemoglobin and hematocrit. The good recovery or moderate disability were achieved by 37% of the patients. The remaining 63% resulted in severe disability, vegetable and death. The poor out come group in comparing with the good outcome group showed lower initial Glasgow Coma Scale and more combined injuries. The five week follow up of secondary systemic insults mean flow showed higher serum Na flow and lower arterial PaO2 flow in poor outcome group.
