Recently, cell cycle regulators have been suggested as new prognostic factors of the lung cancer. In this study, we evaluated the expression of cyclin D1, p21, and p53 using the X2-test, with regard to the stage of the patients, histologic type, and histologic differentiation in the 135 cases of non-small cell lung carcinomas (NSCLC). To evaluate the confounding effects among cyclin D1, p21, and p53 on X2-test analysis, we used the Mantel-Haenzel test. The NSCLC in this study included 82 cases of squamous cell carcinoma and 53 cases of adenocarcinoma. Each nuclear staining of cyclin D1, p21, and p53 was observed in 65 cases (48.1%), in 54 cases (40.0%), and in 81 cases (60.0%) of NSCLCs, respectively. Only p53 expression was significantly associated with the stage (stage I, II, IIIa) (p<0.05) and squamous cell carcinoma (p<0.05). On the other hand, cyclin D1 expression was significantly associated with the histologic differentiation. The confounding effects among cyclin D1, p21, and p53 revealed that only p21 expression changed the relationship between p53 and stage. In this regard, further study is needed.

Santonin-kainic acid complex was evaluated as a chemotherapeutic of the mass treatment of the Ascaris lumbricoides infection in Korea. The results could be summarized as follows: The negative conversion rate was 82.9% in average in 4 treated groups. Some variations of negative conversion rate among the treated groups were noticed. The egg reduciton rate was 97.7% in average and the results were rather uniform among the three evaluated groups. By the analysis of egg reduction, it seems that the lightly infected cases whose E.P.G. were under 5,000 were resistant to treatment with the less reduced egg output. After the treatment with this complex, the number of egg discharged cases were reduced to 1.84% and the average number of discharge eggs per incompletely treated or untreated cases were reduced to 12.5% compared with the level of before-treatment egg output. The successive observations of the pattern of worm expulsion after drug intake was made. The worms were mostly expelled in the stool within 3 days, and 58.9% of total expelled worms were collected within 24-hour stool. The minimum length of the immature worms expelled was 7.6cm. Among the 659 Ascaris collected in the first-day stool from 91 rural people, 8.5% were in the range of 7.6-12.2 cm-long, immature worms. The sex ratio, male: female= 0.69: 1.

During the period from March 1990 to November 1993, 22 children with diplaced supracondylar fracture of humerus were treated by closed reduction and percutaneous K-wire fixation at department of Orthopaedic surgery in Yeungnam University. All fracture were treated with closed reduction under the general anesthesia and percutaneous pinning. After the fracture was internally fixed, intraoperative anteroposterior reontgenograms of each distal humerus were compared. Only anatomical reduction ensured good result,, because the main cause of late cubitus varus was medial tilting of distal fragment. We could follow up 10 patients more than one year. By Flynn's criteria, satisfactory result were obtained in 9 of the 10. Unsatisfactory one had a limitation in flexion of elbow joint, but had no problem in cosmetic feature. In short term follow up of 7 patients, 5 patients had a satisfactory result, too. There were no neurovascular complication and hospitalization was shortened to 11.4 days. Closed reduction and percutaneous pinning provided stabillity, vascular safty, simplified management, reduced hospital stay. This is safe and reliable technique for obtaining and maintaining an exellent reduction.

BACKGORUND: in spinal anesthesia, we usually walk over the interspinous space with a repetitive insertion-withdrawal-redirection-advance chain, which is annoying and time consuming, let alone the discomfort of patients. The aim of this study was to offer a guideline for the optimal point and angle of needle insertion performing spinal anesthesia and to strengthen the basis of the practice of spinal anesthesia. METHODS: Twenty men were evaluated for this study. While in a head-to-knee position, a simple X-ray was taken and L2-3, L3-4, and L4-5 interspaces were measured. Two tangential lines were drawn between the upper and lower spinous process and the points where these two lines meet the skin were marked. The perpendicular line from the most prominent point of the spinous process to the skin was checked(S). The distances from S to the cephalic point of the tangential line (D1) and to the caudal point of the tangential line (D2) were measured. Perpendicular lines were drawn on the point where tangential lines meet skin and the angles between perpendicular lines and tangential lines were measured (A1, A2). RESULTS: Mean distance (MD, [D2-D1] / 2) and mean angle (MA, [A1+A2] / 2) were calculated as 3.59+/-0.64cm, 11.9+/-4.75 for L2-3, 3.72+/-0.50 cm, 11.18+/-1.65degree for L3-4, and 3.71+/-0.61cm, 11.03+/-4.95degree for L4-5 (mean+/-SD). CONCLUSiONS: At any lumbar level, we could approach the interspinous space from the insertion point of 3.6-3.7cm caudal from the spinous process and with the angle of the cephalic direction of about 11-12degree optimally. it can be the guideline for the insertion of the needle in spinal anesthesia and we can get some safe margins for insertion from the measured data.
