1.A Clinical Study on Fractures of the Pelvic Bone

Recently, the incidence of pelvic bone fractures has been increased with increment of the volume of traffic. We reviewed 318 cases of pelvic bone fractures treated at the department of orthopaedic surgery of Hanyang University Hospital during the period from May, 1972 to December, 1980. The age and sex distribution, cause and classification of fractures, type of treatment, associated injuries and complications were recorded and investigated. The results obtained were as follows: 1. There were one hundred and ninety four (61%) male and one hundred and twenty four (39%) female, and one hundred and ninety seven patients (62%) in this entire series were aged from twenties to forties. The most common victims were in active persons. 2. The most common causative injury was traffic accident. 3. Two hundreds and thirteen (67%) cases were stable fracture and one hundred and five (33%) cases were unstable fracture, which were classified by Kanes classification. 4. The fracture of the femur was most commonly associated bone and joint injuries and the injury of the lower urinary tract was also most commonly associated soft tissue injuries. 5. All of the cases, except 3 cases of the avulsion fracture, were treated with conservative treatment. The remaining 3 cases were treated with surgical treatment, 6. The mortality rate was 5.3%, and the most common cause of death was hemorrhagic shock in 70.6% of the entire fatal cases in this series.

3.A Case of Apocrine Hidrocystoma.

Apocrine hidrocystoma is an uncommon benign tumor derived from the secretory segment of apocrine sweat glands and another name for it is apocrine cystadenoma. A 48-year-old female patient had a solitary, translucent red-brownish cystic nodule, 10 × 6 × 7mm in size, on the periorbital area of her left eye. The lesion had enlarged very slowly for 25 years. We excised the lesion and identified it as apocrine hidrocystoma. This is a case of apocrine hidrocystoma with typical clinical and histopathological findings.

5.A Case Report of Heterotopic Pregnancy after IVF-ET.

Heterotopic pregnancy is named when an extrauterine (ectopic) pregnancy coexists with an intrauterine pregnancy simultaneously by many causes such as PID (pelvic inflammatory disease), endometriosis, IUD (intrauterine device), previous pelvic surgery and others. This is very rare in general population, with a range of occurrence estimated between 1:7963 and 1:30000. But recently the incidence has increased as the uses of ARTs (assisted reproductive technologies) including ovulation induction, IVF-ET (in-vitro fertilization and embryo transfer) and GIFT (gamete intrafallopian transfer) increase. Because this has high maternal morbidity, mortality and fetal loss, early diagnosis and proper management is very important. We report a case of heterotopic pregnancy following IVF-ET with a brief review.

6.Resurfacing of Pitted Facial Acne Scars with a Pulsed Erbium: YAG laser.

BACKGROUND: Laser resurfacing has beneficial effects for the treatment of several skin conditions. Recently, the pulsed erbium:YAG laser has been shown to be a highly effective treatment for several kinds of pitted facial scars. OBJECTIVE: The purpose of this study was to assess the efficacy and safety of pulsed erbium:YAG laser skin resurfacing at the setting of low or high energy for pitted acne scars. METHODS: 65 patients with pitted acne scars were included in this study. All patients had skin types III or IV. All patients were instructed to use tretinoin cream 0.05% nightly for 2-4 weeks prior to the laser treatment. The pulsed erbium:YAG laser with a 2 mm handpiece at the setting of 500 mJ/pulse, 3.5-5 W in 45 patients and 1,000 mJ/pulse, 7-10 W in 20 patients was used. Two weeks after laser treatment, topical application of hydroquinone 4%, tretinoin 0.05%, and hydrocortisone 1% cream was recommended for 2-4 weeks. Facial photographs were obtained at baseline and 2 week intervals postoperatively. The results of treatment were evaluated for the changes of skin texture and color at 1 month, 3 months, and 6 months. RESULTS: In 45 cases treated with 500 mJ/pulse energy, pitted acne scars were improved about 46.7 % on average and no erythema and postinflammatory hyperpigmentation was observed after 6 months. In 20 cases treated with 1,000 mJ/pulse energy, pitted acne scars were improved about 64% on average and erythema in two patients and postinflammatory hyperpigmentation in one patient were observed after 6 months. CONCLUSION: The results of laser treatment for pitted facial acne scars at the setting of high energy are better than those of laser treatment at the setting of low energy.
