1.A Case of Miillerian Adenosarcoma of the Uterus with Squamous Cell Carcinoma of the Uterine Cervix.

Mullerian adenosarcoma is an uncommon variant of MQllerian mixed tumors of the uterus and a mixed, biphasic neoplasia in which the mesenchymal component is malignant and the epithelial component is benign. In contrast to typical malignant Miillerian mixed tumor(MMMT), Miillerian adenosarcomas are usually tumors of low malignant potential. These tumom usually present as pedunculated endometrial masses in postmenopausal women with vaginal bleeding. We experienced a case of Miillerian adenosarcoma of the uterus with squamous cell carcinoma of the uterine cervix and report with a brief review of literatures.

3.Repair of Neglected Rupture of the Achilles Tendon using V-Y Tendinous Flap

The 6 patients with neglected rupture of the Achilles tendon were treated by end to end anastomosis using the V-Y tendinous flap originated by Abraham in 1975. A tendinous flap was developed over the tendon by making an inverted V incision, which was then repaired in a Y fashion. The postoperative results were satisfactory with no functional disability of the ankle and with recovery of muscle strength for periods of 8 months to 15 months, but slight limited dorsiflexion of the ankle in two cases were noticed. The advantages of this method are: 1. Optimum functional recovery. 2. Recovery of muscle strength and tendon contour. 3. Prevention of late stretching or possible rupture of the healed tendon at the rupture site.

4.Computed tomographic evaluation of adrenal gland tumors

CT findings of twenty-four patients with adrenal gland tumors were analyzed to evaluate their location, size,shape, density etc. The post-surgical confirmation was made in 17 cases. The findings were as follows: 1. 4-10cmsized tumor messes were most common (42%). 2. Round or oval shaped forme were most common (59%). 3. Functionaltomors were 71% (17/24) including Cushing's syndrome of 10 cases, pheochromocytomas of 6 cases and aldosteronismof 1 case. 4. Abnormal adrenal glands were delineated in 88%(21/24), most effectively by CT. We concluded that CTwas the valuable study in the evaluation of the adrenal gland tumors and in the determination of surgicaloperation.

6.Antioxidants in Serum and Induced Sputum of COPD Patients.

BACKGROUND: Although an oxidants and antioxidants imbalane has been considered in the pathogenesis of chronic obstructive pulmonary disease (COPD), there is a paucity of reports focussing on the smoking-induced changes of oxidants and antioxidants in COPD. METHOD: The concentration of antioxidants (ascorbic acid, uric acid, retinol, and α- &γ-tocopherol) was measured in the serum and induced sputum of 30 healthy controls and 34 stable COPD patients using high performance liquid chromatography (HPLC). The inhibition of lipid peroxidation as an index of antioxidant capacity was measured in the serum by a TBA assay. RESULTS: The serum concentration of ascorbic acid, α-tocopherol, and retinol were significantly lower in the patients with COPD than in healthy controls (484.8±473.3 vs 1497.8±819.2 pmol/L, p<0.001, 48.38±17.34 vs 73.96±26.29 pmol/L, p<0.001, and 9.51±8.33 vs 15.01±5.88 pmol/L, p<0.05, respectively, mean±SD). However, there were little differences in the ascorbic acid and uric acid concentrations in the induced sputum between the COPD patients and the controls. The induced sputum to serum ratio of ascorbic acid was significantly higher in COPD patients compared with healthy control (0.375 vs 0.085, p<0.05). In the normal controls, the serum ascorbic acid concentration was lower in smokers than in nonsmokers (1073±536 vs 1757±845 pmol/L, p<0.05), but the level was still higher than that of the COPD patients (p<0.05). The serum retinol levels were correlated with FEV1 in COPD patients (r=0.58, p<0.05). The products of lipid peroxidation were increased in normal smokers and COPD compared with normal nonsmokers (115.56±19.93 and 120.02±24.56 vs 91.87±20.71 µmol/µmol Pi of liposome, p<0.05). CONCLUSION: Cigarette smoking may induce the depletion of serum antioxidants and this depletion of antioxidants is suggested to play a role in the pathogenesis of COPD.
