1.A Case of Elephantiasis Nostras.

Elephantiasis nostras, the result of recurrent skin infection or chronic lymphedema is characterized by marked skin fibrosis and massive enlargement of a body part. We present a case of elephantiasis nostras due to chronic recurrent erysipelas which affected the lower extremities in a 62-year-old man. The patient had had persisting non-pitting edema, and tender multiple verrucous brownish plaques and nodules on both lower legs for 1 year. On histological examination, there was hyperkeratosis and papilomatosis in the epidermis, fibroplasia, dilated thick walled venules and collagen fibers oriented vertical to the skin surface, and hyperplasia of eccrine ducts in the dermis.

2.The Expression of c-erbB-1 and c-erbB-2 in the Various Skin Tumors.

BACKGROUND: c-erbB-1 (epidermal growth factor receptor) and c-erbB-2 oncoprotein have common tyrosine kinase activities, and alteration in their expression has been defined in various visceral tumors. However, relatively little is known about their expression in skin tumors. OBJECTIVE: Our aim was to evaluate the distribution and expression pattern of c-erbB-1 and c-erbB-2 in skin neoplasms. METHODS: We have undertaken an immunohistochemical survey of c-erbB-1 and c-erbB-2 in the tissue specimens of keratoacanthoma (KA), actinic keratosis (AK), squamous cell carcinoma (SCC) and basal cell carcinoma (BCC). RESULTS: Membranous c-erbB-1 expression had a tendency to be down-regulated in some specimens of SCC and dysplastic portions of AK. In invasive lesions of SCC, we observed increased cytoplasmic accumulation of c-erbB-1. Most specimens of BCC showed rather decreased expression of c-erbB-1 compared with other skin tumors. c-erbB-2 oncoprotein showed strong cytoplasmic staining in SCC, especially in the invasive tumor mass, and in some deeply dysplastic or hyperplastic portions of AK, though the difference of intensity was not striking between tumors. BCC revealed relatively weaker expression of c-erbB-2 than other skin tumors, which was similar in pattern to c-erbB-1. CONCLUSION: The expression patterns of c-erbB-1 and c-erbB-2 are altered in various skin neoplasms, and seem to be related to the dysplastic status or differentiation level of tumor cells.

3.Two Cases of Pedunculated Lipofibroma.

Pedunculated lipofibroma is a rare disorder characterized by ectopic deposits of mature adipose tissue in the dermis. It is a dorne shaped or sessile papule which occurs at any site and develops later in life. We, herein, report two cases of pedunculated lipofibroma which have developed on the chin of a 64-year-old man and the buttock of a 47-year-old woman.

5.The Relationship between Work Posture and Cervical Spine Abnormality among Cervicobrachial Disorder Workers in a Microwave-oven Assembling Factory.

This study conducted to evaluate the association of work posture with cervical spine abnormality toward 106 microwave-oven assemblers. A self-administered questionnaire was used to obtain a general characteristics and a work history of workers. The posture of neck and shoulder joint of workers were measured with video tape recording and the simple X-ray of cervical spine was checked-up to observe the X-ray abnormality including degenerative change, curvature abnormality, disc space narrowing or disc canal narrowing. The result were as follows. 1. The prevalence of degenerative change among cervicobrachial disorder workers was 44.3%(47 persons), curvature abnormality was 43.4%(46 persons), disc space narrowing was 21.7%(23 persons), and disc canal narrowing was 21.7%(23 persons). 2. The prevalence of degenerative change at cervical spine was increased with the degree of neck flexion(flexed versus neutral; odds ratio (OR)=2.7), the total work posture of neck(mild or severe awkward versus neutral; OR=1.2, 3.4). 3. The prevalence of degenerative change at cervical spine was increased with the degree of the right shoulder flexion, the heaviness of the materials carried by the right hand and the degree of awkwardness at the total work posture of right shoulder(p<0.05). 4. There was no evidence of association between curvature abnormality, disc space narrowing, disc canal narrowing and work posture. In conclusion the awkward work posture was related with degenerative change of cervical spine among microwave oven assembling workers and the further study in these field must be made to prove the association objectively.

Bowens disease is generally regarded as a premalignant dermatosis. If untreated, 3% to 5% of patients may develop squamous cell carcinoma. However, sebaciou carcinoma arising from Bowens disease is very rare. We presented a case of quarnous cell carcinoma and sebaceous circ s disease in a 68 year-old male. He had multiple bowenoid skin lesi nsties. A bean-sized nodule as developed on the bowenoid lesion of the he had a large yellow crust.ed exudative tumor on the Rt. thigh. We took a biopsy specimen of these three discrete lesions. The nu lipid stain of frozen section revealed Bowens disease, squarnous concllnoma arising from Bowen on the trunk and extremi Rt. lower abdomen. Almost of routine histology and carcinoma, and sebaceous carcinoma, respectively.

9.Relationship between Vimentin Expression and Progression of Uterine Cervix Epithelial Neoplasms.

Vimentin is an intermediate filament protein normally expressed in mesenchymal cells, but the evidence of the aberrant expression of vimentin in epithelial cancer cells suggests that the vimentin expression might be related to local invasiveness and metastatic potential. There have been a few previous studies on the vimentin expression in human cervical carcinogenesis using in vivo and in vitro models. We examined the immunohistochemical vimentin expression in various squamous epithelial neoplasms of the uterine cervix, including the cervical intraepithelial neoplasia group (n=25), the microinvasive squamous cell carcinoma group (n=15), the invasive squamous cell carcinoma group (n=15) and the metastatic squamous cell carcinoma group (n=8). Vimentin positivity was significantly higher in the invasive than in the intraepithelial group, and in the cases with lymph node metastasis than in those without metastasis, suggesting a relationship between the vimentin expression and progression of the uterine cervical epithelial tumors.
