1.Surgical Treatment of the Distal Radius Fracture

Fractures of the distal radius are one of the most common injuries met in the orthopedic field. Once it was thought that good function comes despite of poor anatomic restoration in the distal radius fractures. But now there are wide consensus that more aggressive surgical treatments are needed in such complex fractures to fulfill the better clinical results. We reviewed 56 consecutive distal radius fractures of 54 patients treated surgically in Orthopedic Department of Asan Medical Center between October 1989 and July 1994. The mean length of follow-up was 29.2 months. Clinical end results were statistically analyzed according to each anatomi- cal variables and state of distal ulna, and anatomies and results were analyzed according to different surgical modalities in each fracture classes. Radial length and radial inclination was significantly correlated with clinical results. The clinical results were significantly worse when dorsal tilt exceeded 10. In intraarticular type of fractures, residual articular incongruency seems to be the significantly affecting factor of development of arthritis and the clinical result. The associated fracture of distal end of the ulna did not significantly affect the result. In same type of fracture class, achievement and loss of reduction, arthritis, and Demerit score show no significant differences between groups of different surgical modalities. Probably this is due to proper choice of modalities in each different situations.

6.Sclerotherapy Using Abnobaviscum for the Extensive Recurrent Chronic Morel-Lavallée Lesions - A Case Report -

The Morel-Lavallée lesion (MLL) is a closed soft-tissue degloving injury, resulting in characteristic hemo-lymphatic fluid collection between the fascia and subcutaneous layers. The MLL was managed routinely with drainage and compression bandages, but sclerotherapy can be used in patients with chronic lesions refractory to first-line therapy. This paper presents a case of extensive recurrent chronic MLL treated with sclerotherapy using Abnobaviscum, which has been used to treat adhesion in malignant pleural effusion.
7.Factors Associated with Stress Symptoms In Korean Dental Laboratory Technicians.

OBJECTIVES: To investigate the factors associated with symptoms from job stress among Korean dental laboratory technicians. METHODS: We collected the data of 786 technicians for life-style, job stressor, social support, self-esteem, personality, and various dimension of stress symptoms included roomatization, obsessive-compulsive, interpersonal sensitivity, depression, anxiety, and hostility by self-administerd questionnaire. Global severity index of dental technician (GSID) was calculated by total sum of sub-dimension. RESULTS: We used 80 percentile as a cut-off point, and got the 81.5 % of sensitivity, 86.1 % of specificity by receiver operating characteristic curve. The odds ratios of various factors in high group of GSID compared with low group were 1,49(95 % Cl 1.00-2.00) for smoker, 1,53 (1.06-2.21) for unhealthy group, 6,69 (4.74-9.44) for work overload, 8.34(5.82-11.97) for work demand, 9.89 (6.76-74.94) for work process, 8.27 (5.80-11,79) for work environment, 5.82 (4.04-8.39) for interpersonal relationship, 8.34 (5.82-11.97) for occupational conflict, 5.70 (4.04-8.04) for labor remuneration, 7.58 (5.37-10.71) for role conflict, but 0.39 (0.27-0.57) for social support, 0.41 (0.29-0.60) for self-esteem. By multiple logistic regression, factors were significantly associated with GSID were age, working area, role overload, work demand, work procedure, physical environment, occupational conflict, role conflict, social support, personality traits. CONCLUSIONS: Factors associated with symptoms from stress were role overload, work demand, work procedure, occupational conflict, interpersonal relationship, labor remuneration and role conflict in Korean dental laboratory technicians. It is suggested that health promotion program to reduce job stress and to enforce social support and self-esteem should be developed.

8.A histological study of the root resorption applying to intermittent and continuous force for incisor intrusion of dog.

The purpose of this study was to investigate the root resorption pattern in incisors in dog under intrusive orthodontic loadings of various magnitude and duration. Intrusive forces were generated by closed coil springs. Force magnitudes were 15-30gm, 50-60gm and 80-110gm. Durations were continuous and intermittent. Intermittent duration was applied at intervals of 12 hours. The readjustment of the force was done every 4 days. The forces were maintained for 30 days. All specimens were decalfied, embedded in paraffin and stained with hematoxylin-eosin stain. Observations were made with light microscope. The following results were obtained; 1. The continuity of root surface was ceased in all, except intermittent forced teeth with 25gm. 2. The root resorptions, cementoclasts and cemental lacunae, were increased around periapical regions and the destructive scope of bone was deeply extended as the exerted orthodontic forces increased. 3. It was inspected that, under the same forces, root resorption and bone destruction were more deeply appeared in the continuously forced teeth than the intermittently forced ones. 4. All of the alveolar bone, showed direct and undermining bone resorptions.

9.The effect of pregnancy induced hypertensive disorders on perinatal death in placental abruption.

OBJECTIVE: To evaluate the clinical profiles and outcomes of patients with placental abruption and the effect of hypertensive disorders on perinatal death. METHODS: It was a retrospective study over 9 years from 1 January 1995 to 31 December 2003. We reviewed the data of women (N=104) presenting placental abruption among 10,940 women who were delivered at this hospital. RESULTS: The incidence of placental abruption was 0.95% or one in 104 deliveries. Only 47.6% of these could be diagnosed before delivery. The most common symptom was vaginal bleeding (71.1%) and intrauterine fetal death had already occurred in 26.9%. Perinatal mortality rate was 32.6% including still birth (26.9%) and neonatal death (5.7%). There was no maternal death. 28.8% of placental abruption were associated with preeclampsia and other hypertensive disorders but in most cases (68.3%), the contributing factors could not be found. When the hypertensive disorders were associated, perinatal mortality rate was 33%, which was not significantly different when compared with perinatal mortality (32%) without hypertensive disorder (p>0.05). But the neonates from the hypertensive women were more growthly impaired than those from normotensive women (p<0.05). CONCLUSION: Hypertensive disorders was an important factor in women with placental abruption but in most cases the contributing factors could not be found. The hypertensive disorders did not aggravate the perinatal mortality but increased the rate of intrauterine growth retardation in placental abruption.

10.The Relationship between Serum Ferritin and High Sensitivity C-reactive Protein among Adults in a Health Promotion Center .

BACKGROUND: More attention is given to oxidative hypothesis which causes atherosclerosis to be recognized as inflammatory response. The relationship between serum ferritin which catalyzes lipid peroxidation and high sensitivity C-reactive protein which reflects vascular inflammation was investigated among adults in a health promotion center. METHODS: The study group consisted of 297 men and women (men 86, women 211) who visited the health promotion center of a hospital in Seoul to have a health checkup from October 1, 2004 to April 1, 2005. These subjects answered the questionnares and were measured in the following; blood tests, brachial-ankle pulse wave velocity and several anthropometric measurements. Statistical analysis was performed on 111 subjects after exclusion of those subjects who were taking antihypertensive agents or antidiabetic agents, and who had acute inflammatory diseases, acute liver diseases, anemia, and who had a WBC > or =11,000x10(3)/mm3 or a serum ferritin > or =200 ug/L or a ABI (Ankle Brachial Index) <0.9. RESULTS: The average serum ferritin concentration of men against women was 132.57+/-43.12 ng/ml to 78.23+/-38.10 ng/ml which means that men have about 1.7 times as high concentration than women (P<0.001). Serum ferritin was significantly correlated with high sensitivity C-reactive protein (r=0.332). Even in multiple stepwise regression analysis, there was a independent relationship between serum ferritin and high sensitivity C-reactive protein (beta=0.138, P=0.010). When we analyzed with distinction of sex, this relationship in women was constant (beta=0.131, P=0.031), but serum ferritin in men just showed the trend of correlation with BMI (beta=9.510, P=0.059). CONCLUSION: There is a significant relationship between the increase of serum ferritin and high sensitivity C-reactive protein in healthy women; furthermore, studies in men need to be confirmed.
