1.Photoelastic analysis of the Stress distribution on an intervertebral disc.

To observe the change in the status of stresses according to three different postural angulation of an intervertebral disc with or without nucleus pulposus, 6 specimens of a 3-dimensional photoelastic model of the spine were made of epoxy. The nucleus pulposus portion was replaced with silicon in three models, and the three were made without silicon. Through axial application of a vertical compressive load of 8 kg, the peculiar patterns of the isochromatic fringes were observed. Stresses on the intervertebral disc were analyzed according to three different postural angulations of the intervertebral disc with the nucleus pulposus and without the nucleus pulposus. The results of these study are as follow: 1. In an erect neutral posture with the nucleus pulposus, the stress concentration was much increased at the posterior portion rather than at the anterior portion. Also, the high stress was concentrated at the medial and central portion. In an erect neutral posture without the nucleus pulposus, the stress concentration was much increased at the anterior portion rather than at the posterior portion and the stress distribution seemed to be locally concentrated. 2. In a maximal flexed posture, the stress concentration was much increased at the posterior portion rather than at the anterior portion. Comparing the presence of the nucleus pulposus with the absence of the nucleus pulposus, the stress concentration was lower at the anterior portion in the presence of the nucleus pulposus than in the absence of the nucleus pulposus. However, the stress distribution at the posterior portion was nearly same in the two groups. According to the analysis of the stress distribution diagram, as a whole, the stress pattern around the disc was evenly distributed. 3. In a maximal extended posture, the higher concentration of the stress distribution at the anterior and medial portion rather than in the posterior and lateral portion was observed. The stress concentration was higher in the presence of the nucleus pulposus than in the absence of the nucleus pulposus. 4. Comparing the maximal flexed posture with the erect neutral posture, the stress concentration in the flexed posture was much decreased in the posterior portion rather than in the erect neutral posture, and an even distribution of the stress pattern in the flexed posture was observed. 5. In the presence of the nucleus pulposus, at the anterior and posterior portion, the stress concentration in the flexed posture was much decreased compared with the extended posture. In the absence of the nucleus pulposus, at the anterior and posterior portion, the stress concentration in the extended posture was much decreased compared with the flexed posture.

2.A Case of allergic Contact Dermatitis from Glutaraldehyde.

Glutaraldehyde is commonly used as a biocide to disinfect delicate instruments such as dental instruments, to treat warts, hyperhidrosis and bullous diseases and as a preservative in various materials. Contact sensitivity to glutaraldehyde may not be detected, because it is not included in the standard screening chemicals recommended in the screening tests. We report a case of allergic contact dermatitis caused by Wydex(2% glutaraldehyde). A 54-year-old female made a visit to our departrnent with eczematous dermatitis of both hands, neck and face. She worked as a cleaner in delivery room and newborn nursery, and had developed the dermatitis 1 month after Wydex' containing 2% glutaraldehyde had been to clean the rooms. She had no family or personal history of atopy or contact dermatitis. Patch tests with Korean Standard(KS-1000) and Wydex were performed, and patch tests with 2% glutaraldehyde, 0.2% gluteraldehyde and rubber mix followed. The positive results were, Wydex 2% ++, 0.2% +, 0.02% + and. glutaraldehyde 2% aq. +, 0.2% aq. + on day 2 and day 4. We confirmed that the skin lesions had been caused by Wydex . The symptoms were eradicated after she changed her work environment and there has been no evidence of recurrence.

3.A Case of Hidradenoma Papilliferum of the Nipple.

6.An immunohistochemical study for several skin conditions in patients with viral hepatitis B.

The present study was carried out to investigate the HHsAg in the normal skin, dis- eased skin and skin specimens of window period of HBsAg positive patients in their serum with normal liver function. This study was done by immunoperoxidase techniques using the monoclonal antibody to hepatitis E3 surface antigen and by electron microscopy. Immunoperoxidase staining was dane on twenty specimeris of normal skin Group .D twenty specimens of diseased skin (15 cases of dermographism, 2 cases of purpura, 1 case of follicuht,is and 1 cases of morbilliform eruption) (Ciroup II ) and three specimens of normal skin af window period (Group III ) of HBsAp positive patients in their serum. Twelve cases of Group I, eleven cases of Group IL (1(J cases of dremographism and 1 rase of purpura) and two cases of Group II were stained positively for HBsAg. Positive sites were keratinocyte of epidermis, sweat gland, blood vessel and hair follicle. Electron microscopy failed to reveal viral particle, The above resuts suggest the possibility of transmission of viral hepatitis B thraugh the skin and skin appendages.

7.Treatment of bone tumors with a custom-made prosthetic replacement.

Congenital Hyperextension of the Knee is a extremly rare deformity, in which the tibia is displaced anteriorly in relation to the femur. It is usually classified into three subtypes according to severity of the status: genu recurvatum, subluxation and dislocation. (Curtis and Fisher) We experienced a genu recurvatum type of congenital hyperextension of the knee in June, 1976.

9.The Role of Computed Tomography of the Bladder Cancer with Perivesical Fat Infiltration.

Total cystectomy was performed to the 40 patients with urinary bladder tumor who were found perivesical fat infiltration by computerized tomography. 25 patients were previously untreated, and 15 patients had undergone transurethral resection or biopsy of the tumor before computerized tomography. The histopathological stage was compared to the preoperative computerized tomography stage. In pathologic specimen, perivesical fat infiltration was found in 62.5% of the cases, whereas perivesical fat infiltration was not found in 37.5% of the cases. In untouched tumors or after transurethral resection computerized tomography accuracy demonstrated no significant difference.

10.Operative treatment of supracondylar-Condylar Fractures of the Femur

The previous reports about Supracondylar-Condylar Fracture of the femur which extends to articular surfaces revealed poor results in general, regardless of the methods of treatment. But those results are thought to be much alleviated if they are treated according to the principles of ASIF. Fifteen cases of Supracondylar-condylar fractures of the femur from 13 patients were treated by open reduction and rigid internal fixation during last 3 years at Department of Orthopedic Surgery, Early mobilization of knee joint was permissible in most of the cases without loss of fixation or other complications during convalescence Subsequently, greatifying results were obtained as observed in varying period of follow up days.
