1.Primary Intrapericardial Lipoma Simulating Pericardial Effusion -Report of A Case-.

Primary pericardial tumors are rare than those originating within the myocardium or endocardium and, moreover, primary benign pericardial tumors are much rare in occurrence. Fine(1986) was able to collect only 43 cases of lipoma. Most cases are diagnosed only at autopsy. One fourth of the cardiac lipoma arise subepicardially, where they suggest the presence of pericardial effusion with compression or displacement of the heart. A case of primary intrapericardial lipoma simulating pericardial effusion is presented. This tumor originated in left anterior aspect of pericardium. The tumor was measured 30x30x5cm in size and 2,200mg in weight. Total excision of the mass was accomplished by posterior lateral thoracotomy incision, resulting in complete cure. This is the first case of primary benign lipoma of the pericardium in literatures in our knowledge.

Granulosa cell tumors are uncommon neoplasm of ovary that are characterized by their long natural history and for their tendency to recur years after an aparrent clinical cure. Endometrial cancer occurs in association with these tumor in at least 5% of cases and 25%-50% are associated with endometrial hyperplasia. In this study, we present a case of granulosa cell tumor of right ovary with endometrial cancer after left oophrectomy.

3.Choreoathetosis after cardiopulmonary bypass with deep hypothermia.

A 9-year-old boy showed orofacial dyskinesia and psychic symptoms shortly after open heart surgery with deep hypothemia and cardiopulmonary bypass for congenital cyanotic heart disease. The choreothetosis progressed to be generalized and accompanied by mental deterioration. This involuntary movement partially responded only to repetitive administration of sedatives. After 3 month the patient expired with sudden development of ventricular tachycardia and cardiogenic shock.

4.Vein Ligation Combined with Endovenous Laser Treatment in Klippel-Trenaunay Syndrome: A Case Report.

Klippel-Trenaunay syndrome is a rare congenital anomaly combined with capillary, venous malformation and limb hypertrophy, which presents with port-wine stain caused by capillary malformation and varicosities, leg swelling, stasis dermatitis, thrombophlebitis and/or deep vein thrombosis induced by deep vein atresia or hypoplasia, and deep vein obstruction due to fibrous band or persistent marginal vein. Treatment is usually conservative as surgery is technically challenging and the excision of varicose vein is incomplete. However, there are several reports, even when varicosities are recurrent, of clinical improvement following excision of the varicose vein in almost all cases. Here the case of a 12 year-old patient, presenting with severe leg pain due to chronic venous hypertension, treated with vein ligation combined with endovenous laser treatment (EVLT).

6.The Expression of Nuclear Factor-kappaB in the Placental Tissue with Preeclampsia.

OBJECTIVE: The aim of this study is to ascertain the differences in NF-kappaB (Nuclear Factor-kappa B : p50) activity between the placental tissues of preeclampsia and normal pregnancy, and to certify that the circulating lipid peroxides is increased in preeclamptic women. METHODS: Placental tissues were obtained from preeclamptic (n=33) and normal pregnancies (n=21) with no other medico-surgical illness or obstetric complications, delivered by cesarean section without labor. The activities of NF-kappaB and IkappaBalpha (Inhibitory factor kappaBalpha) on syncytiotrophoblast, cytotrophoblast, endothelium, extravillous cytotrophoblast, and decidua were separately measured by immunohistochemical staining using tissue microarray technique. Malondialdehyde assay was used to evaluate the oxidative stress, measuring lipid peroxide levels on each sample. Mann-Whitney test was done for statistical analysis of the data. RESULTS: Nuclear staining of NF-kappaB (p50) was seen more intensively within the extravillous cytotrophoblast of preeclampsia group compared with the control group (p<0.05). The immunoreactivity of NF-kappaB (p50) was also detected in cytotrophoblasts, syncytiotrophoblasts, endothelium, and decidua, but showing no statistical difference between two groups. IkappaBalpha was strongly expressed in both groups but there was no statistically significant between two gropups. Preeclamptic group showed significantly increased circulating lipid peroxide levels compared to normal pregnancy group (1.22+/-0.79 nmol/mL vs 0.41+/-0.12 nmol/mL, p<0.05). CONCLUSION: The expression of NF-kappaB is significantly increased in extravillous cytotrophoblast of preeclamptic women compared to normal pregnancy, and may be associated with increased levels of circulating lipid peroxide. These findings might help us to understand the pathologic mechanism of preeclampsia and further study should be done for effects of NF-kappaB on implantation.

7.Unusual Giant Mycotic Aneurysm at the Middle Cerebral Artery: Case Report.

Aneurysms at the distal branch of the middle cerebral artery may be mycotic or congenital in origin. A 34 year-old male patient was admitted with a seizure and a 7-day history of upper respiratory tract infection. The neurological examination showed no abnormality except for headache and mild dysarthria. There was no intracardiac lesion suggesting infective endocarditis. CT, MRI and cerebral angiograms revealed a partially thrombosed giant aneurysm at the distal branch of the middle cerebral artery. Pathologic examination revealed acute inflammatory cells and focal necrosis in the aneurysmal wall as well as recent thrombus. Here, we are reporting an unusual case with a giant distal middle cerebral artery aneurysm with intramural inflammation, suggesting a congenital giant aneurysm with secondary infectious inflammation.

8.Transvginal Sonography in the Prediction of Preterm Labor.

OBJECTIVE: Preterm labor and delivery is probably the largest problem in modern perinatology. The early diagnosis of preterm labor is crucial for prevention of preterm birth. To predict the onset of preterm labor, we examined the diagnostic performance of serial cervical assessment by transvaginal sonography. METHODS: In this prospective study, we performed transvaginal sonography at approximately 30 and 34 weeks of gestation in women with singleton pregnancies. Cervical parameters evaluated included endocervical length, the presence of funneling, funnel length and funnel width. We then assessed the relation between cervical parameters and the risk of spontaneous preterm labor. RESULTS: We examined 258 women at approximately 30 weeks of gestation and 247 of these women again at approximately 34 weeks. Spontaneous preterm labor occurred in 9 of women examined at 30 weeks(Group 1) and in 13 at 34 weeks(Group 2). The endocervical length was normally distributed at 30 and 34 weeks(mean +/-SD, 38.26+/-6.82mm and 35.63+/-7.35mm, respectively). The endocervical length decreased significantly from 30weeks to 34 weeks(p=0.0001). Both groups showed significantly shorter endocervical length(p <0.05) and groupl more presence of funneling than group of term pregnancy(p<0.05). Receiver-operator characteristic curve and multiple logistic regression analyses indicated that endocervical length <-30mm at 30 weeks and <-25mm at 34 showed highest diagnostic index in predicting the onset of preterm labor(p=0.0001). Conclusions: Serial transvaginal ultrasound assessment of endocervical length during early third trimester is a useful predictor of preterm labor and delivery in low-risk patients.

9.Adventitial Cystic Disease of the Popliteal Artery: A case report.

Adventitial cystic disease of the popliteal artery is a rare disorder that causes localized stenosis or occlusion by compression of the vessel lumen. The disease produces lower extremity claudication, typically in young and middle-aged men. We report a case of the adventitial cystic disease of the popliteal artery in a 55-year-old man with symptom of left calf claudication. Diagnosis was done by ultrasound, angiography, and magnetic resonance imaging. The patient was treated with resection of the diseased popliteal artery followed by interposition of saphenous vein graft. Postoperatively, the symptom and sign resolved completely.

10.Comparison of Lacrimal Scintigraphy and Contrast Dacryocystography in Epiphora.

PURPOSE: Conventional contrast dacryocystography(C-DCG) has been used mainly for anatomical assessment of the lacrimal drainage apparatus, due to its limited information on the dynamics of the lacrimal system, and thus correlation role in epiphora. The purpose of study was to improve the diagnostic value in epiphora by utilizing RI dacryocystography(RI-DCG) with quantitative criteria. MATERIALS AND METHODS: The patient for RI dacryocystography set erect with the head fixed in front of gamma camera fitted with a standard 6 mm pinhole collimater. Both eyes of 85 patients(170 eyes) were scanned with 99rnTc-pertechnetate followed by conventional dacryocystography. The data from gamma camera were recorded simultaneously on a computer system for subsequent quantitative analysis. RESULTS: In 94 of 96 eyes without epiphora, RI-DCG was correlated with normal C-DCG and % of emptying was at least over 50%. In 58 of 74 eyes with epiphora, RI-DCG was correlated with abnormal C-DCG and % of emptying was at most below 50%. In 16 eyes with functional block, quantitative RI-DCG showed abnormal % of emptying. CONCLUSION: Nuclear DCG with quantitation had higher diagnostic yield in functional block and correlated higher with epiphora. Thus nuclear DCG with quantitation would be a valuable functional test after postprocedures such as dacryocystoplasty.
