1.Erythrocyte sedimentation rate and C-reactive protein values in patients with hip arthroplasty.

Serum C-reactive protein (CRP) levels and erythrocyte sedimentation rates (ESR) were measured in 46 patients treated with uncomplicated primary hip replacements, 39 total tip replacements and 7 bipolar hip replacements. In uncomplicated primary hip replacements, ESR levels were slightly elevated preoperatively and were variable postoperatively. But CRP was normal before surgery and elevated in postoperative course, but back to normal within three weeks in most cases. Early success of hip arthroplasty is in indicated by normalization of CRP within three weeks, regardless of ESR. Since ESR seems to react somewhat differently from the CRP, both methods are useful in the monitoring of complications after hip arthroplasty.

2.Pathologic Classification of the Resected Gastric Carcinoma.

A total of 212 cases of gastric carcinoma resected at Kang-Dong Sacred Heart Hospital during the period of 4 years from December 1986 to December 1990 were examined according to Borrmann, Mulligan-Rember, Ming and WHO methods based on histopathological investigations. In Mulligan-Rember (M-R) classification, intestinal cell type (IC) was frequently seen in Borrmann type I and II; pyloro-cardiac gland cell type (PC) in II and III, and mucous cell type (MC) in IV and III. Expanding growth pattern was more frequent in IC, infiltrative growth pattern in MC, and PC showed less infiltrative growth than MC. In gross type, the most expanding growth pattern was seen in Advanced gastric carcinoma type (AGC) I and the next one was in AGC II; the ratio of infiltrative versus expanding type was highest in AGC IV and next in AGC resembling early gastric carcinoma (EGC) and AGC III in order. On WHO classification except squamous type, all the papillary type showed expanding growth and infiltrative growth was frequently seen in signet-ring cell, undifferentiated, tubular and mucinous types in order. Lymphoid stroma was more frequently found in expanding type than infiltrative type. The frequency of angioinvasion of tumor cells observed was high in AGC resembling EGC, AGC II, III, IV, I and EGC in order. In WHO classification excluding squamous type, undifferentiated and signet-ring cell types occurred more frequently under the age of 60 and papillary type were more frequnetly seen over the age of 60. But tubular type had no difference between the two age groups. In Ming's classification, expanding type was more frequently seen than infiltrative type over the age of 60.

Teratoma is one of germ cell tumor, common neoplasm in women of reproductive age, but it can arise at any age. Its malignant transformation is rare, less than 2%, frequently at older age. Nearly all the cases are squamous, sarcomatous and adenomatous transformation. Here we present a case of mixed transformation, adeno-squamous cell carcinoma with brief review of the concerned literature.

4.Normal Fetal Echocardiography.

Fetal echocardiography is used by means of decleration of fetal cardiac anaztomy, to estabilish the diagnosis of congenital heart disease in utero. We attemped fetal echocardiography to ninty three pregnant women after intra uterine period 24 weeks, and estimated cardiac circumference, cardiac axis, pulmonary atery root diameter, arortic root diameter, diameter of inferior vena cava, diameter of superior ve studied how these estimates associate with following gestational na cava, and fractional shortening of ventricles. We ages. Cardiac axis was on the average 37.28 degree and cardiac apex was located in anterior left side of chest area. Aortic root diameter was 0.227 GA-0.043mm (GA=gestational age) at systolic phase, 0.203 GA+0.421mm at diastolic phase. Pulmonaly root diameter was 0.271 GA-0.029mm at systolic phase, 0.251 GA-0.067mm at diastolic phase. Thoracic aorta diameter was 0.195 GA+0.109mm at systolic phase, 0.198 GA+0.794mm at diastolic phase. Fractional shortening was 0.24 (1 Standard Deviation=0.11) in right ventricle, 0.23(1 SD=0.154) at left ventricle, and so ratio of right and left ventricle was 1.04(1 SD=0.51). Once normal fetal cardiac anatomy is understood, structural defects and/or alternation of function can be evaluated antenatally.

5.Clinical Significance of PCNA and nm23 Expression in Invasive Colorectal Carcinoma.

PURPOSE: It is known that PCNA (proliferating cell nuclear antigen) is associated with cell proliferating activity and nm23 with so called 'anti-metastatic' activity. We tried to elucidate the relationships of PCNA labeling index and nm23 expression in carcinoma cells with survival rates of patients with invasive colorectal carcinoma. METHODS: Immunohistochemical study was performed using monoclonal antibodies for PCNA and nm23 gene products on 45 cases of paraffin-embedded tissue made of invasive colorectal cancer. RESULTS: Five-year survival rate was lower in the group with high PCNA labeling index than that with low one, but PCNA labeling index was not associated with the tumor stages. Expression of nm23 was not associated with survival rate, tumor stage, site, and PCNA labeling index. CONCLUSIONS: Although PCNA labeling index was not associated with tumor stages, this study suggests that PCNA labeling index will be a good prognostic factors in invasive colorectal carcinoma.

7.A Case of 4P+ Syndrome.

We experinced a case of 4p+ syndrome in male infant. He had multiple anomalies such as flat occiput, hypertelorism, low set malformed ear, lower anterior hair line, depressed nose, broad nasal bridge, bilateral complete cleft lip and palate, short neck, unusual position of fingers, ventricular septal defect and umblical hernia. He menifested growth and developmental retardation. Karyotype with banding revealed an extra short arm of chromosome 4. The mother's karyotype was normal. His father and father's sister had a translocation between the short arm of chromosome 4 and the short arm of chromosome 9; their karyotypes were 46, XY, t(4;9) and 46, XX, t(4;9), respectively. In this case, trisomy 4p was the result of parental balanced translocatiom. As this is the first case in Korea, it is worthwhile to report with reviewing literature.

8.A study on the angle of articular eminence and the inclination of anterior tooth related to facial types.

The purpose of this study was to evaluate the difference and relationship between the slope of articular eminence and the inclination of upper and lower incisor teeth, which are related to the functional stability of occlusion in mandibular protrusion, according to Ricketts' facial types, by the use of lateral cephalograrn and SAM2 articulator in 68-adult normal occlusion without tooth missing, orthodontic treatment and occlusal equilibration. The results of this study were as follows: 1. The angle of articular eminence slope to occlusal plane in brachyfacial type was steeper than that in dolichofacial type, but the angle of articular eminence slope to SN plane and FH plane was not different between facial types. 2. The upper incisor axis in dolichofacial type was steeper than that in brachyfacial type, but lingual surface slope of upper incisor was not different between facial types. 3. In all samples there was a positive correlation between the angle of articular eminence slope and the lingual surface slope of upper incisor to SN plane and FH plane, and in mesofacial type there was a positive correlation between the angle of articular eminence slope and the lingual surface slope of upper incisor to SN plane. FH plane and occlusal plane, and in brachyfacial type there was a positive correlation between the angle of articular eminence slope and the lingual surface slope of upper incisor to occlusal plane. 4. In all samples there was a positive correlation between the angle of articular eminence slope and the angle of Dc-Gn to SN plane, and in mesofacial type there was a positive correlation between the angle of articular eminence slope and the angle of De-Gn to occlusal plane, and in brachyfacial type there was a positive correlation between the angle of articular eminence slope and the angle of De-Gn to SN plane and FH plane. 5. In all samples there was a positive correlation between the angle of De-Gn and the lingual surface slope of upper incisor to SN plane and FH plane, and in mesofacial type there was a positive correlation between the angle of De-Gn and the lingual surface slope of upper incisor to SN plane. 6. In all samples and facial types there was a negative correlation between MP to 1 axis and condylar incisal angle.
