1.A Study on Postgonococcal Urethritis.

The subject were 653 male patients with uncomplicated gonorrhea at the VD Clinic of Choong Ku Public Health Center in Seoul from January to November 1985. 653 male patients with uncomplicated gonococcal infection were treated one of the following regimens. Five treatment regimens used were 2, 0 gm spectinomycin single 1M, 4 0 mega unit fortified procain penicillin G+2. 0 gm kanamycin sulfate IM preceded by 1 pgm probenecid PO, 6.0 mega unit fortified procain penicillin G+2 pgm kanamycin sulfate IM preceded by 1. 0 gm probenecid PO, 5, 0 rnega unit aqueous crystaline penicillin+2. 0 gm kanamycin sulfate irn preceded by 1, 0 gm probenecid PO and 9 tablets of cotrimoxazole+ 2. 0 gm kanamycin sulfate IM. There were no significant differences in the incidence of PGU among the five treatment groups. The overall PGU rate was 67.3% 59.2% and 53.6% at 3~5, 7~10 and 14 days after treatment respectively. The incidence of PGU at 7-10 days was not significantly higher than that of at 14 days after treatment. It is suggested that it is best to test PGU at 7 days after treatment. Because the longer one waits, the harder to follow the patients, and in earlier period, post-inflammatory irritation might be too frequent. It is also suggested that at 3 5 days after treatment examination of urethritis might serve to compare the effect of treatment regimens on the incidence of PGU.

2.Clinical Study and Therapeutic Experience in Tinea Versicolor.

Fifty-six patients with tinea versicolor were studied clinically and therapeutically, from July 1984 to August 1985, at the Department of Dermatology, Hanyang University. L atients were treated with 20g sodium thiosulfate solution, 1g isoconazole nitrate cream, lg bifonazole cream and. oral ketoconazole. The result obtained were as follows: 1. The average age of all patients at visit was RO. 1 years, the oldest patient was 72 year-old and the youngest patient was 13 year-old. The male to female ratio was 3. 7: 1. 2. Distribution of lesions were anterior chest(26.8%), back(23.9%), abdomen (19%), axilla (17%), neck(6. 3%), pubic area(4. 2%), upper extremities(1. 4%), hip(0.7%) and lower extremities(0.7%). The incidence of hyperpigmented lesions was 76. 8%, and that of hypopigmented lesions was 23. 2g. 3. Average of treatment duration in each trial group indicated 3. 71-I--0 83 weeks (M+SD) in 20g sodium thiosulfate regimen group, 3. 07+ 0. 73 weeks in 1g isoconazole nitrate, 3.29+1.14 weeks in bifonazole, R. 00+0. 68 weeks in oral ketoconazole. No treatment results show statistically significant differences among the trial groups.

3.Urine culture and serologic test for diagnosis of trichomoniasis in male patients with nongonococcal urethritis.

Trichomoniasis, a common and sometimes distressing condition in women, is caused by infection of the genitourinary tract by the pathogenic protozoan Trichomonris aginalis(T. vaginalis). Men may also be infected, although frequently without any symptoms, and most cases of trichomoniasis are sexually transmitted. The best evidence for an etiologic role for T. vaginalis is in nongonococcal urethritis(NGU). In epidemiologic studies, T,ginalis has been isolated from 1 to 68 percent in men with NGU. The nurnber of NGU is increasing rapidly than gonorrhea in deeloped nations, and in some developing countries, especially in Korea, In view of the difficulties in findng trichomonads by direct microsco by in men, urine culture and serodiagnosis plays an important role in the diagnostic work. Urine culture, indirect fluorescent antibody test(IFAT) and enzyme-linked immunosorbent assay (ELISA) were used to elecect T.vaginalis in 221 patients with male NGU. These patients visited the VD Clinic of ChoongKu Public Health Center in Seoul from May to August, 1989 and from March, 1990 to December, 1991. The results obtained were as follows.: 1. T. vaginalis was grown from only one of the 221 samples on TPS-1 meida. 2. In 208 NGU patients, there were 10(4.8%) positive reactions on IgG antibody in IFAT. 3. In 208 NGU patient, there were 14(6.7%) positive reactions to IgG antibody in ELISA. 4. No statistical significant correlation was found between then sults of IFAT and ELISA in individual patients. In conclusion, 1. Male trichomoniasis in patients with NGU was detected at a, significantly low rate. Due to the small number of patients in the test, further epidemiologic stidies of trichomoniasis in male NGU patient are necessary. 2. Serologic studies (IFAT and ELISA) have detected sorre trichomoniasis in male patients with NGU. These tests seem to be helpful in the diagnosis of male t-ichomoniasis.

4.A Case of Scleredema.

Scleredema is a rare scleradematosis of unknown cause involving the face, neck, upper portion of the trunk, and proximal upper extremities. The clinical features of the disease are nonpitting indurated edema or stiffness of the neck, which may be sudden or insidious in onset. Diabetes mellitus and its complications have been frequently described to be associated with this disease. We present a case of scleredema developed on the nape, upper portion of the back and shoulder in 60-year-old man who had been suffered from diabetes mellitus for the past ten years.

5.A Case of 9p Tetrasomy Associated with Hydrocephalus due to Hypertrophy of Choroid Plexus.

9p Tetrasomy is rare chromosomal aberration that was described in approximately 30 previous patients in the world and this is the first described in Korea. Here we report a 3 month-old boy who was referred for genetic evaluation because of facial dysmorphism, such as wide fontanells, hypertelorism, bulbous nose, low set ears, cleft lip and palate. He had also psychomotor retardation and hypotonia. He was diagnosed as tetrasomy 9p syndrome by clinical feature and chromosomal study. Thereafter, increased growing of head size compared with body weight and height was observed and brain MRI shows hydrocephaly associated with remarkable hypertrophy of choroid plexus and mild Dandy Walker syndrome.

7.Study on Perception of Their Body Image and Body Dissatisfaction in Adolescent in Ulsan.

BACKGROUND: In this study, we examined body image perception and dissatisfaction with weight and height in children and adolescents, according to age and body mass index. Additionally, we compared our findings with those of previous studies concerning eating disorders. MATERIALS AND METHODS: In July and August 2008, 1,501 students were sampled from elementary, middle, and high schools in Ulsan and assessed using self-completion questionnaires. We used the Korean version of the Eating Attitude Test-26 (EAT-26) to screen for eating risk groups. Participants were stratified by grade, gender, and body mass index. RESULTS: 1) Dissatisfaction with height, weight, and body image was greater in the older group. 2) Dissatisfaction with weight and body image was greater in the heavier group. 3) EAT-26 scores and the number of individuals at high risk for eating disorders were higher in the female group than in the male group. CONCLUSION: The EAT-26 score in this study was similar to those found in previous Western studies. 2) There were a number of high-risk individuals in the female group, with a tendency toward earlier presentation. Continuous health management and prevention programs are required.

8.Bone Signal Abnormality, as seen on Knee Joint MRI: Relationship between Its Location and Associated Injury.

PURPOSE: The purpose of the study was to evaluate the relationship between the location of bone signalabnormality and associated injury, as seen on MR, in patients with acute knee joint injury. MATERIALS AND METHODS: Thirty-six patients with acute knee injury and bone signal abnormalities on MR were included in this study. Thefemur and tibia were each divided into six compartments, namely the anteromedial, medial, posteromedial,anterolateral, lateral, and posterolateral ; these were obtained in each knee joint. We evaluated the location ofbone signal abnormality and the corresponding arthroscopic or operative findings of injury to ligaments andmenisci. Cases with signal abnormalities involving more than three compartments were excluded. RESULT: Bonesignal abnormalities were demonstrated in 51 compartments. Most(84%, 43/51) were noted in the lateral half of theknee joint, the most common location being the tibio- posterolateral compartment(13/51). The femoro-lateral(11/51) and tibio- anterolateral compartment(8/51) were the next most common locations. All cases(13/13)with bone signal abnormality in the tibio- posterolateral compartment had tears at the anterior cruciate ligament,while 9 of 11 cases(81%) with abnormality in the femoro- lateral compartment had tears at the anterior cruciateligament. Six of eight cases(75%) with signal abnormality in the tibio- anterolateral compartment had tears at theposterior cruciate ligament ; 31 of 43 cases (72%) with abnormality in the lateral half of the knee joint hadtears at the medial collateral ligament. Six of eight cases(75%) with signal abnormality in the medial half of theknee joint had tears at the medial meniscus, but no lateral meniscal tear was found. Among patients with signalabnormality in the lateral half of the knee joint, the tear was lateral meniscal in nine of 43 cases(21%) andmedial meniscal in six of 43(14%). CONCLUSION: The location of bone signal abnormality, as seen on knee MR, inpatients with acute knee joint injury could be an important finding suggesting associated injury.

9.Use of Quality of Life Instruments in Original Articles of the Korean Journal of Dermatology.

BACKGROUND: The measurement of quality of life (QOL) is increasingly important in patients with skin diseases. However, no study has objectively analyzed these trends in the Korean literature. OBJECTIVE: The aim of this study was to analyze original articles that measure QOL in the Korean Journal of Dermatology and to evaluate the use of QOL instruments. METHODS: We collected 14 original articles that measure QOL from the Korean Journal of Dermatology (1960 to 2008) and analyzed these articles for annual changes of the number of articles, the kind and frequency of QOL instrument, and the kind of skin disease in the articles. We also analyzed 66 original articles from the Journal of the American Academy of Dermatology (JAAD) (1979 to 2008) in the same way. RESULTS: In general, the number of articles that measure QOL in the Korean Journal of Dermatology increased. The articles for psoriasis and acne vulgaris accounted for 21.4% of the articles each. Dermatology-specific instruments were the most common QOL instruments used. These results were similar for the JAAD. Of the dermatology-specific instruments, the skindex-29 was used most frequently in the Korean Journal of Dermatology, and the Dermatology Life Quality Index in the JAAD. CONCLUSION: Measuring the impact of skin disease on QOL is increasingly important. Domestic assessment of QOL in skin disease using novel QOL instruments should increase to conform to international standards to allow Korean dermatologists to provide better care to their patients.

10.Prediction of the alveolar bone level after the extraction of maxillary anterior teeth with severe periodontitis.

PURPOSE: After extraction, the alveolar bone tends to undergo atrophy in three-dimensions. The amount of alveolar bone loss in the horizontal dimension has been reported to be greater than the amount of bone loss in the vertical dimension, and is most pronounced in the buccal aspect. The aim of this study was to monitor the predictive alveolar bone level following the extraction of anterior teeth seriously involved with advanced chronic periodontitis. METHODS: This study included 25 patients with advanced chronic periodontitis, whose maxillary anterior teeth had been extracted due to extensive attachment loss more than one year before the study. Periapical radiographs were analyzed to assess the vertical level of alveolar bone surrounding the edentulous area. An imaginary line connecting the mesial and the distal ends of the alveolar crest facing the adjacent tooth was arbitrarily created. Several representative coordinates were established in the horizontal direction, and the vertical distance from the imaginary line to the alveolar crest was measured at each coordinate for each patient using image analysis software. Regression functions predicting the vertical level of the alveolar bone in the maxillary anterior edentulous area were identified for each patient. RESULTS: The regression functions demonstrated a tendency to converge to parabolic shapes. The predicted maximum distance between the imaginary line and the alveolar bone calculated using the regression function was 1.43+/-0.65 mm. No significant differences were found between the expected and actual maximum distances. Likewise, the predicted and actual maximum horizontal distances did not show any significant differences. The distance from the alveolar bone crest to the imaginary lines was not influenced by the mesio-distal spans of the edentulous area. CONCLUSIONS: After extraction, the vertical level of the alveolar ridge increased to become closer to the reference line connecting the mesial and distal alveolar crests.
