1.Progress of Heat-CO2 pneumoperitoneum affect gastric cancer metastasis through adhesion molecules

Gastric cancer is one of the most common gastrointestinal malignant tumor in our country,with the continuous development of minimally invasive surgical technology,laparoscopic radical gastrectomy has gradually been developed,but whether CO2 pneumoperitoneum can cause peritoneal metastasis remains controversial.This article has synthesized the recent reports to illustrate the influence of Heat-CO2 pneumoperitoneum on adhesion molecules,and introduce the latest viewpoint of adhesion molecules affect gastric cancer metastasis.This article can provide clues to evaluate the clinical application value of Heat-CO2 pneumoperitoneum.

[Objective] Build gangrene etiology and pathogenesis of frame structure, this paper discusses the etiology and pathogenesis theory. [Methods] Based on the time axis oriented, taking "Chinese Medical Code" as a search tool, "gangrene" as the key word, search Qin and Han Dynasties to the late Qing Dynasty physician medical books, and with the library books proofreading ,this paper will be of the ancient literature about etiology and pathogenesis of clips in series, and carries on the combation and analysis .[Result]To sum up gangrene of etiology and pathogenesis, theory framework is divided into four periods:Begin in the Qin and Han Dynasties, the development in the Wei and Jin Dynasties, rich in Song and Yuan Dynasties, perfect and enrich in the Ming-Qing Dynasty. Its etiology and pathogenesis are concise:yin-yang disharmony, cold coagulation to stasis;toxic heat accumulation. burns essence;Qi and Yin waste, blood stasis inchannels. [Conclusion] Through the gangrene of etiology and pathogenesis of frame structure building, not only can deepen the understanding of the disease, also provide a theoretical basis and ideas for the treatment of the diseases associated with modern medicine.In order to make a modest contribution to the majority of clinical work and scientific researchers.

To study the feasibility, safety and aesthetics of laparoscopic cholecystectomy using a single-port through the umbilicus in patients with chronic cholecystitis with cholecystolithiasis.

From Jan 2007 to Jun 2017, the clinical data of 4790 patients who underwent single-port, 4 426 patients conventional three-ports, and 3 304 patients two-ports laparoscopic cholecystectomy were retrospectively studied.

4.Optimal Ratios and Their Spectral-efficacy Relationship of Baitong Decoction in the Treatment of Ulcerative Colitis

Objective To explore the optimal ratio of Baitong decoction based on efficacy,clarify its spectrum-effect relationship,and identify its potential quality markers.Methods An ulcerative colitis(UC)model in mice was established using dextran sulfate sodium.The efficacy of Baitong decoction with varying drug ratios was assessed by evaluating the apparent score,pathological score and inflammatory factor changes of UC in each group of experimental animals.The fingerprints of Baitong decoction with different ratios were established by high performance liquid chromatography(HPLC),and the relationship between the content of each substance and its efficacy was analyzed by partial least squares regression to determine the potential quality markers of Baitong decoction.Results Baitong decoction was most effective in relieving ulcerative colitis when the mass ratio of Fuzi,Ganjiang and Congbai was 1∶2∶2.The fingerprinting identified 14 common peaks across 7 ratios,with 9 peaks were found to be associated with the remission of ulcerative colitis by partial least squares regression analysis.Conclusion The optimal ratio of Fuzi,Ganjiang and Congbai for treating UC is 1∶2∶2.The spectrum-effect relationship analysis suggested that the quality markers of Baitong decoction may be the substances represented by peak 2(benzoylaconine),3,5,6,8(mesaconine),9(aconitine),10(hypaconitine),13(10-gingerol)and 14.

Objective:
To explore the application of septoplasty and cheiloplasty in complete unilateral cleft lip repairment, and evaluate the effect on nasal shape correction.
Methods:
Twenty-four infants with complete unilateral cleft lip were divided into two groups: the correction group and the control group. Both groups underwent cheiloplasty by Mohler′s technique, septoplasty was performed in the correction group during the primary cheiloplasty. OBJECTIVE:To investigate the “dose-effect-toxicity”correlation of Miao medicine Wikstroemia indica before and after processed on anti-immnue inflammation of mice . METHODS :Mice were divided into blank group ,model group ,ethanol extract of W. indica raw product groups 1-6,ethanol extract of W. indica processed product by “sweat soaking method ”groups 1-6 (hereinafter called “raw groups 1-6”“processed groups 1-6”for short ,drug dosage were 0.13,0.20,0.26,0.52,1.04,2.6 g/kg), positive group (cyclophosphamide,36.4 mg/kg),with 10 mice in each group. Except for blank group ,other groups were given 1% 2,4-dinitrofluorobenzene-acetone-sesame oil mixed solution to induce delayed type hypersensitivity model. After modeling, blank group and model group were given constant volume of 1.0% CMC-Na solution intragastrically ,and administration groups were given relevant medicine 20 mL/kg intrag astrically,oncea day ,for consecutive 5 d. A fter last medication ,ear swelling degree of mice were recorded ;the inhibition rate of swelling degree, half effective dose (ED50) and 95% confidence 158-02-32); interval(CI)of raw and processed products were calculated ; the weight of heart ,liver,spleen,lung and kidney were measured and the indexes of organs were calculated ;ELISA 1161472062@qq.com and modified chemical oxidation method were used to determine the serum levels of inflammatory factors (TNF-α, IL-10) and liver and renal function indexes (ALT,AST, TBIL,BUN,CREA). RESULTS:Compared with blank group ,the degree of ear swelling in the model group was significantly increased(P<0.05). Compared with model group ,ear swelling degree of mice were decreased significantly in different doses groups of ethanol extract of raw and processed products of W. indica (P<0.05). The inhibition rate of swelling increased with the increase of dose ,ED50 and 95%CI of delayed hypersensitivity ear swelling were 0.239 6(0.129 0,0.445 2)g/kg and 0.147 3(0.076 8,0.282 7)g/kg,respectively. Compared with blank group ,liver index and serum TNF-α level of mice were increased significantly in model group ,while lung index and serum IL- 10 level were decreased significantly (P<0.05). Compared with model group ,the levels of liver indexes (positive group ,raw group 3,processed groups 1-6)and serum TNF-α levels(positive group ,raw groups 1-3,processed groups 1-4) were decreased significantly in different administration groups ;while the levels of lung indexes (positive group ,raw groups 3-6 and processed groups 3-6),serum IL- 10 levels(raw groups 1,2,4 and 5,processed groups 2-6),ALT,AST,BUN and CREA levels (raw groups 4-6),TBIL levels (raw groups 3-6 )were increased significantly (P< 0.05). CONCLUSIONS :The ethanol extract of raw product of W. indica has certain anti-inflammatory activity ,and has certain hepatorenal toxicity to mice ,with certain “dose-effect-toxity”correlation. The ethanol ectract of processed product of W. indica has certain anti-inflammatory activity too ,but its hepatorenal toxicity was lower than raw product. The “sweat soaking method ” possesses the function of “retaining efficiency and reducing toxicity ”for processing W. indica .
