1.Effects of trimetazidine on B-type natriuretic peptide and cardiac troponin I in patients undergoing off-pump coronary artery bypass graft

Objective To investigate the myocardial protective effects of trimetazidine by observing the changes of peripheral B-type natriuretic peptide (BNP),cardiac troponin I (cTnI) level in patients undergoing off-pump coronary artery bypass graft (OPCAB), and the clinical significance of peripheral cTnI and BNP in cardiac surgery. Methods One hundred and three OPCAB patients were divided into trimetazidine group (52 cases) and control group (51 cases) by random digits table. The serum levels of BNP and cTnI preoperatively,postoperatively of 24 h, 72 h and 7 d were detected. Results The serum levels of BNP [(224.5 ± 12.0), (331.2 ±22.6), (82.4 ±3.3) ng/L] and cTnI [(0.21 ±0.04), (1.32 ±0.49), (0.26 ±0.04) μ g/L] in trimetazidine group were lower than those in control group [(294.7 ± 11.8 ), ( 383.9 ± 28.3 ),( 112.4 ± 12.5 ) ng/L and ( 1.20 ± 0. 13 ), (2.35 ± 0.54), (0.75 :± 0.21 ) μ g/L] postoperatively of 24 h, 72 h and 7 d (P< 0.05 ). The serum levels of BNP and cTnI increasedd at 24 h after operation. The peak level was found at 72 h and remained higher level until 7 d after operation. The baseline levels of BNP were positively correlated with cTnI (r = 0.635,P < 0.05), but negatively correlated with left ventricular ejection fraction (LVEF) (r =-0.674,P < 0.01 ). Conclusion Trimetazidine can obviously reduce serum levels of BNP and cTnI in patients undergoing OPCAB. So the united application of serum BNP and cTnI may be as a monitor marker to reflect the cardiac function in patients after OPCAB.
2.The expression and relationship of CXCR4 and microvessel density in laryngeal squamous cell carcinoma.

OBJECTIVE:
To investigate the expression of chemokine receptor 4 (CXCR4) in laryngeal squamous carcinoma and its correlation with clinicopathology character and the building of microvessel density (MVD). To evaluate its role in the carcinogenesis and progression in laryngeal squamous carcinoma.
METHOD:
The expression of CXCR4 in fresh laryngeal squamous carcinoma tissues and adjacent normal tissues from 42 patients were examined by RT-PCR, immunohistochemical staining and Image-pro-plus software. The numbers of regeneration blood vessels in the laryngeal squamous carcinoma was counted by antibody against factor V associated antigen and immunohistochemical analysis.
RESULT:
The positive expression rate of CXCR4 in tumor samples was significantly higher than that in normal ones (P < 0.01). In tumor samples, the expression of CXCR4 were not associated with age, sex, tumor site and T stage (P > 0.05), while it were higher in tumors of grade III, IV than in grade I, II of pathology classification (P < 0. 01). The expression of CXCR4 were significantly higher in tumors with cervical lymph node metastasis than that in tumor without cervical lymph node metastasis (P < 0.01). The expression of CXCR4 protein and CXCR4 mRNA were at the same level. The expression level of CXCR4 in the laryngeal squamous carcinoma tissue was positively correlated with vascularization.
CONCLUSION
The higher expression of CXCR4 may play a key role in the invasion and metastasis of laryngeal squamous carcinoma, and were correlated with micro-vascularization.

3.Periodontal tissue in a bio-implant by periodontal ligament cells sheet and bone marrow stromal cells sheet.

OBJECTIVETo fabricate the bio-implant supported by regenerated periodontal tissue utilizing periodontal ligament cells (PDLC)-bone marrow stromal cells (BMSC) sheet and natural root.
METHODSPremolars of 2 beagle dogs were extracted to prepare the implanted area. Autologous tooth roots were carved into cylinders. PDLC and BMSC separated from beagle dogs were cultured into cell sheets in medium. Tooth roots were wrapped by one type of cell sheets or both to fabricate bio-implant and divided into four groups, tooth roots were wrapped by PDLC sheets and BMSC sheets successively (2 samples each dog), tooth roots were wrapped by PDLC sheets alone (2 samples each dog), tooth roots were wrapped by BMSC sheet alone (2 samples each dog), tooth roots without cell sheet (1 sample each dog). The implants were implanted into the mandibles. The mandibles were dissected 12 weeks later, sliced and stained by HE and Masson dyes for histological examination.
RESULTSIn the PDLC cell sheet/root implants, histological examination revealed that new periodontal-like tissue including cementum, periodontium and alveolar bone was regenerated.In the BMSC implants, tooth ankylosis was observed.
CONCLUSIONSPDLC sheet and natural root can be used to fabricate bio-implant. PDLC sheet could promote periodontal regeneration.

4.Clinical application of modified radical neck dissection by gasless unilateral axillary approach in papillary thyroid cancer

Objective:To investigate the effectiveness, safety, and advantages of modified radical neck dissection by gasless unilateral axillary approach (GUA-MRND) in the surgical management of selected patients with papillary thyroid cancer.Methods:We retrospectively analyzed patients with papillary thyroid cancer who underwent GUA-MRND (endoscopic group, n=16) versus unilateral open modified radical neck dissection (MRND) (open group, n=32) during the period from Jan. 2019 to Jun. 2021, including the differences in surgical efficiency, complication rate, and incisional satisfaction.Results:Compared MRND with GUA-MRND, the patients were younger ( P<0.05) , operative time and postoperative drainage anterior ( P<0.01) were slightly inferior in the latter, but it had obvious advantages in cervical swallowing discomfort and incision satisfaction evaluation ( P<0.05) . There was no significant difference in the incidence of temporary recurrent laryngeal nerve injury, intraoperative and postoperative bleeding, hematoma, infection, lymphatic or chylous leakage and supraclavicular numbness after surgery ( P>0.05) . The number of dissected lymph nodes in area II in the GUA-MRND was lower ( P<0.05) , but it was significantly higher ( P<0.01) in area III. And the average regional cleaning efficiency in the GUA-MRND was level Ⅲ (35.5%) , level Ⅵ (28.59%) , level Ⅳ (23.21%) , level Ⅱ (7.18%) and level Ⅴ (7.12%) , suggested that GUA-MRND had higher efficacy for level III, level Ⅵ and Level IV. Conclusion:GUA-MRND is safe, effective, and has high cosmetic satisfaction in the treatment of selected patients with lateral cervical lymph node metastases from papillary thyroid cancer.
