Objective:To explore the trajectory of frailty changes in patients receiving maintenance hemodialysis and analyze its influencing factors. Methods:Maintenance hemodialysis patients at the Blood Purification Center of the Minda Hospital of Hubei Minzu University from January to October 2022 were selected by convenience sampling. The Clinical Frailty Scale (CFS) was used to survey patients at the start of dialysis and one, three, six, and 12 months of dialysis. Latent class growth modeling was employed to identify the potential classes of frailty trajectory changes and analyze the influencing factors of these classes. Results:A total of 255 questionnaires were distributed, with 212 valid responses, yielding an effective response rate of 83.14%. The changes in frailty among the patients receiving maintenance hemodialysis were categorized into three groups: moderate frailty worsening group ( n=64), frailty improvement group ( n=110), and stable pre-frailty group ( n=38). The average probabilities of patients belonging to each latent class were 0.948, 0.938, and 0.950, respectively. Age, gender, body mass index, anemia, continuing to work after illness onset, and comorbidities were identified as influencing factors of frailty trajectory changes in these patients (all P<0.05) . Conclusions:There are three types of frailty trajectory changes in patients receiving maintenance hemodialysis from the start of dialysis to 12 months of regular dialysis. Targeted interventions can be implemented based on the identified influencing factors.

AIM:To investigate the visual quality and stereopsis after the implantation of PanOptix trifocal intraocular lens(TFNT00).METHODS: A prospective clinical study was conducted. A total of 36 cataract patients(50 eyes)who underwent phacoemulsification combined with TFNT00 implantation in Jinan Mingshui Eye Hospital from November 2022 to April 2024 were selected. They were followed up until 3 mo after the operation. The uncorrected distance visual acuity(UCDVA), uncorrected intermediate visual acuity(UCIVA), uncorrected near visual acuity(UCNVA), objective scatter index(OSI), modulation transfer function cut off(MTF-cut-off), Strehl ratio(SR)and 100%, 20%, 9% contrast visual acuity(CVA)were observed. The binoptometer was used to collect the patients' far and near stereopsis acuities. The defocus curve was drawn after the operation; the Chinese version of the VF-14 Visual Function Index Scale was used to evaluate the visual quality and satisfaction after the operation.RESULTS: There were statistically significant differences in the UCDVA, UCIVA and UCNVA of the patients at different time after the operation(all P<0.05). The transition of the defocus curve was gentle between +0.5--3.0 D after the operation. The OSI value at 3 mo postoperatively after the operation was significantly lower than that before the operation(P<0.01), and the MTF-cut-off, SR, 100% CVA, 20% CVA and 9% CVA were significantly improved than those before operation(all P<0.01). The far and near stereopsis acuities of 34 patients were abnormal before the operation. The far stereopsis acuities of the patients who underwent bilateral eye surgeries were all normal after the operation, and the near stereopsis acuity of 12 patients was normal. Among the patients who underwent unilateral eye surgeries, the far stereopsis acuities of 13 patients were normal, and the near stereopsis acuities of 11 patients were normal. The far and near stereopsis acuities of the patients who underwent bilateral eye surgeries were significantly better than those patients who underwent unilateral eye surgeries.CONCLUSION: The PanOptix trifocal intraocular lens can provide patients with good full-range visual acuity. The stereopsis of the patients with bilateral implants is better than that of the patients with unilateral implants. The overall visual quality and satisfaction of the patients after the operation are relatively high.

OBJECTIVE: To synthesize and identify rotundic acid esters derivatives, and to evaluate in vitro antitumor activity. METHODS: Using rotundic acid as raw material, rotundic acid esters derivatives were synthesized by 28-esterification, 3β and 23-position hydroxyl group combined with acid anhydride. The structure of rotundic acid esters derivatives were identified by HRMS, 1H-NMR, 13C-NMR and physicochemical properties. The in vitro antitumor activities were evaluated by MTT assay against paclitaxel (positive control), rotundic acid and rotundic acid esters derivatives with human cervical cancer cells HeLa, human malignant melanoma cells A375, human lung adenocarcinoma cells SPC-A1 and human liver cancer cells HepG2 (take IC50 as an indicator). RESULTS: Totally 6 kinds of rotundic acid esters derivatives were synthesized, i.e. rotundic acid methyl ester(compound 1), 3, 23-O-di(acetyl) rotundic acid methyl ester(compound 2), 3, 23-O-di(propionyl) rotundic acid methyl ester(compound 3), 3, 23-O-di(butyryl) rotundic acid methyl ester(compound 4), 3, 23-O-di(phthaloyl) rotundic acid methyl ester(compound 5) and 3, 23-O-di(succinyl) rotundic acid methyl ester(compound 6). Compared with paclitaxel, the antitumor activity of compound 5 and compound 6 to Hela, A375, HepG2 and SPC-A1 was similar to it, and there was no significant difference between them (P>0.05); the IC50 of rotuntic acid and other compounds decreased with statistical significance (P<0.05). CONCLUSIONS: In the study, 6 kinds of rotundic acid esters derivatives are synthesized and compound 5 and compound 6 have significant antitumor activity in vitro, which can provide reference for further study.
