1.Effect of chuanxiong and danggui on Na+ and K+ currents of rat hippocampal neurons with hypoxia-reoxygenation

BACKGROUND: The active ingredients of Chinese herbs, such as hongjingtian (rhodiola root, Radix Rhodiolae), srenshen (ginseng, Radix Ginseng), and danggui (Chinese angelica root, Radix Angelicae Sinensis),were found to be effective to inhibit the hypoxia injury.OBJECTIVE: To observe the protecting effect of xionggui diwan (Dripping Pills) on the hippocampal neurons from hypoxia by whole-cell recording patch-clamp technique.DESIGN: An auto-control observation on cells.SETTING: The 88 Hospital of Chinese PLA and Academy of Military Medical Science of Chinese PLA.MATERIALS: The experiment was performed during March to August 2002 in the Institute of Toxicology and Pharmacology of Academy of Military Medical Sciences of Chinese PLA. The neonatal Wistar rats, clean degree, were used. The hippocampal neuron cells out of 9 rats were cultured and divided into the control groups of before and after hypoxia-oxygenation,and xionggui diwan 1 × 10-6 mol/L and 10 × 10-6 mol/L groups. (Xionggui diwan was provided by the 88 Hospital of Chinese PLA and its active ingredient was the volatile ingredient of chuanxiong and danggui.)METHODS: While hypoxia-reoxygenation groups were not given any medicine intervention as control, the treatment of xionggui diwan 1×10-6mol/L was subdivided into xionggui diwan 1 × 10-6 mol/L + non-hypoxia and xionggui diwan 1 × 10-6 mol/L + hypoxia groups, and the treatment of xionggui diwan 10 × 10-6 mol/L was subdivided into xionggui diwan 10 × 10-6 mol/L + non-hypoxia and xionggui diwan 10 × 10-6 mol/L + hypoxia groups. The patch-clamp technique was performed to determine the Na+ and K+ currents of rat hippocampal neurons cultured in vitro before and after hypoxia-reoxygenation.MAIN OUTCOME MEASURES: The Na+ and K+ current amplitude of rat hippocampal neurons cultured in vitro.RESULTS:① The Na+ current amplitude of hippocampal neurons: After hypoxia-reoxygenation, it was significantly lower than that before [(-3.8±1.0, -10.3±0.5) nA, t=3.49, P < 0.01]. ② The K+ current amplitude of hippocampal neurons: After hypoxia-reoxygenation, the activation thresholdof K+ current was changed from -40 mV to -20 mV.③The Na+ and K+ currents of hippocampal neurons before and after hypoxia-reoxygenation with xionggui diwan pretreatment: Before hypoxia-reoxygenation, the ampli tudes of Ⅰ Na and J K of hippocampal neurons at the threshold point were very similar, without significant difference. After hypoxia-reoxygenation,those amplitudes of xionggui diwan 10.0 × 10-6 mol/L group were higher than that of xiongguidiwan 1.0 × 10-6 mol/L group [(-58.5±1.9, -6.9±1.4,-3.8±1.0) nA, t=7.51,P < 0.05].CONCLUSION: It was suggested that xionggui diwan propect the function of hippocampal neurons from hypoxia.

AIM:To investigate the inhibitory effect of dihydro-?-erythroidine on nicotine induced-current in cultured superior cervical ganglion of neonatal rats.METHODS:Pneumatic pressure administration of drug and whole-cell recording techniques were performed to compare induced-current amplitude.RESULTS:Dihydro-?-erythroidine competitively antagonized nicotinic effect,and EC50 was about 0.015mmol/L.CONCLUSION:Dihydro-?-erythroidine is a competitive antagonist of nicotinic receptors in sympathetic neurons.
3.Affect of rhTNFR:Fc on postoperative recovery of patients with inflammatory arthritis

Objective To investigate the affect of rhTNFR:Fc on the postoperative recovery of patients with inflammatory arthritis after arthroplasty. Methods Patients with inflammatory arthritis undergoing arthroplasty were included and divided into rhTNFR:Fc group (rhTNFR:Fc only or combined with conven-tional DMARDs) and conventional DMARDs group (monotherapy with or combination of conventional DMARDs). We retrospectively analyzed the incidence of postoperative infection, wound healing time, the febrile period (body temperature ≥37.5 ℃) and the duration of antibiotics treatment after arthroplasty. x2 test and t test were used for statistical analysis. Results Sixty-seven patients were included, 18 in the rhTNFR: Fc group and 49 in the conventional DMARDs group. One postoperative infection occurred in rhTNFR :Fc group but none in the DMARDs group. There was no significant difference by Fisher's exact test (P>0.05). The febrile duration was (4±3) days in the rhTNFR :Fc group and (3±3) days in the conventional DMARDs group, the difference was not statistically significant (P>0.05). The wound healing time was (14.0±3.1) days in the rhTNFR :Fc group and (14.7±2.9) days in the conventional DMARDs group, which was not statistically different(P>0.05). The duration of antibiotics treatment after operation was (14.8±9.3) days in the rhTNFR: Fc group and (10.3±2.7) days in the conventional DMARDs group, the difference was statistically significant (P<0.05). Conclusion Using rhTNFR:Fc during perioperative period in patients with inflammatory arthritis does not increase the risk of infectious complications or extending wound healing time and the febrile duration.
4.The clinical and laboratory features of patients with Sjgren′s syndrome associated with systemic lupus erythematosus

Objective To investigate the clinical and serologic features of patients with Sj(?)gren′s syndrome (SS) associated with systemic lupus erythematosus (SLE), and to compare with patients of primary Sjgren′s syndrome (pSS) and systemic lupus erythematosus. Methods Two hundred and eighteen unselected SLE patients were evaluated for the presence of SS. The SLE patients with SS (SLE-SS) were compared with SLE patients without SS on clinical and laboratory parameters. Results SS was identified in 22 SLE patients (10.1%). The SS preceded the development of SLE in 8 (36%), the SS secondary to SLE in 7 (32%), and in 7 (32%) patients, the SS and SLE occurred at the same time. Compared with the SLE patients without SS group, patients with SLE-SS were older, with a higher frequency of Raynaud′s phenomenon, pulmonary involvement, anti-SSA, anti-SSB, and rheumatoid factor, but with a significantly lower frequency of renal involvement. Compared with the pSS patients group, SLE-SS patients were younger, with a higher frequency of Raynaud′s phenomenon, arthritis, mucocutaneous involvement and anti-SSA. Conclusions SLE-SS patients have distinct clinical and laboratory features that differ from SLE or pSS patients.
5.Epidemiological characteristics and diagnosis of reported cases with suspected occupational diseases in Huzhou City from 2016 to 2020

Objective:
To investigate the epidemiological characteristics of suspected cases with occupational diseases and analyze the quality of occupational diseases report in Huzhou City from 2016 to 2020, so as to provide insights into supervision and early warning of occupational diseases.
Methods:
The epidemiological data pertaining to suspected cases with occupational diseases reported in Huzhou City from 2016 to 2020 were retrieved from the Occupational Disease and Health Risk Factors Monitoring Information System of China Disease Prevention and Control Information System, including gender, age, type of job, industry category, enterprise size and diagnosis. The percentages of identification, diagnosis and definitive diagnosis were calculated, and the distribution and diagnosis of suspected cases with occupational diseases were descriptively analyzed among different regions, industry categories and enterprise sizes.
Results:
A total of 264 398 people underwent occupational health examinations in Huzhou City from 2016 to 2020, and 322 suspected cases with occupational diseases were detected (12.18/104), including 179 cases with suspected pneumoconiosis (55.59%), 78 cases with suspected occupational ear, nose, throat and oral diseases (24.22%), 54 cases with suspected occupational poisoning (16.77%), 6 cases with suspected occupational skin diseases (1.86%), 3 cases with suspected occupational diseases caused by physical factors (0.93%) and 2 cases with other suspected respiratory diseases (0.62%). The 322 cases with suspected occupational diseases included 290 men (90.06%) and 32 women (9.94%), and had a mean age of (53.28±9.20) years. A total of 207 cases underwent occupational diseases diagnosis (64.29%), and 155 cases were definitively diagnosed with occupational diseases (74.88%). Among different counties (districts) in Huzhou City, the highest number of suspected cases with occupational diseases was reported in Changxing County (101 cases, 31.37%), with a diagnostic rate of 88.12%, and among all enterprises, the highest number of suspected cases with occupational diseases was reported in small enterprises (220 cases, 68.32%), with a diagnostic rate of 68.64%, while among all industry categories, the highest number of suspected cases with occupational diseases was reported in manufacture industry (228 cases, 70.81%), with a diagnostic rate of 56.14%. In addition, there was no suspected case with occupational diseases detected in agriculture, forestry, animal husbandry, fishery, wholesale/retail trade industry, or public administration, social security or social organizations.
Conclusion
Pneumoconiosis was the predominant type of suspected occupational diseases in Huzhou City from 2016 to 2020, and a low diagnostic rate was found. Improved supervision of occupational diseases control is strongly recommended among small enterprises to standardize the reporting system of suspected occupational diseases.

7.Use of Antimicrobials in Non-surgical Departments

OBJECTIVE To investigate and analyze the application of antibiotics in non-surgical departments in our hospital.METHODS To make a survey of the use of antimicrobials in non-surgical departments from a total of 548 randomly selected medical records during Jan-Mar 2007,the data of reasonable use of antibacterials were obtained after evaluation and analysis.RESULTS Of the 548 patients in our study,229(41.79%)used antimicrobials.The use of antimicrobials for infection prevention was in 46 cases(20.09%),for infection treatment was in 183 cases(79.91%).From them 95 cases(41.48%)used a combination of two and more antimicrobials.The most widely used antibacterials were penicillins,cephalosporins,and quinolones.CONCLUSIONS The use of antimicrobials is generally reasonable,and meets the safe,effective,and economical goal in our hospital.
8.Debridement and interbody fusion by a single posterior approach for the treatment of lumbosacral regional spinal tuberculosis

Objective To evaluate the clinical effects of a single posterior translaminal or transforaminal debridement and internal fixation with bone graft for the treatment of lumbosacral regional spinal tuberculosis. Methods From January 2004 to October 2009, 32 patients with lumbosacral regional spinal tuberculosis were treated by a single posterior debridement and internal fixation with bone graft, including 21 males and 11 females with the mean age of 46 years (range, 32-65). The lesions located: 3 in L3,4, 19 in L4,5, 10 in L5S1. 20 cases were pedormed operation via translaminal approach, 12 via transforaminal approach. The modified Prolo scale, visual analogue scale (VAS), kyphosis angle and the vertebral body loss were measured before and after surgery, and in the final follow-up. Results All the patients were followed up for average 16 months (range, 12-24). The pre-, postoperative and final follow-up mean kyphosis angle were -24.3°,-34.8°, and -32.4° in the translaminal group, -25.4°, -35.2°, and 33.6° in the transforaminal group,respectively. According to the modified Prolo scale, the excellent and good result was 93.8%. VAS decreased significantly from 7.6 points to 2.9 after surgery. Conclusion Debridement and internal fixation with bone graft via a single posterior approach for the treatment of lumbosacral regional spinal tuberculosis was an easy and effective technique. The appropriate selection of indications, careful imaging evaluation and thorough surgical debridement played an important role in the treatment of lumbosacral regional spinal tuberculosis.
9.Anti-bacterial Drugs Applying in Liver Transplanting Patients During Perioperative Period:A Survey and Analysis

OBJECTIVE To find out the situation of the application of anti-bacterial drugs in liver transplantation perioperative patients and to improve the quality of their usage.METHODS According to the criteria of DDD and DUI recommended by WHO,making a random sampling survey of the application of anti-bacterials in 206 liver transplantation perioperative patients who discharged hospital during Jan-Dec 2006,and then to analyze the rationality of their application.RESULTS Among the 206 patients,100.0% patients had received anti-bacterial drugs involving 13 kinds,137 patients in them used one,31.1% used two and 2.4% used three,and there were twelve in thirteen kinds of drugs whose DUI was no more than one.CONCLUSIONS The survey has proved that the liver transplantation perioperative patients have received rational therapy.
10.Antibacterial Usage among Outpatients in Our Hospital

OBJECTIVE To understand the antibacterial usage among outpatients in our hospital and provide reference for their rational use in clinic treatment.METHODS Totally 18 232 prescriptions in 2007 were investigated and analyzed.RESULTS The rate of antibacterial usage was 21.9% which took up 6.25% of total cost.As for active treatment single drug consisted of 91.21%,two-drugs 9.09%,and three-drugs of 0.55%.CONCLUSIONS The antibiotic service is basically rational.
