1.Value of different MRI sequences in the diagnosis of breast lesions

Objective:To explore the value of different MRI sequences in the diagnosis of breast lesions. Methods:Using Philips Achieva 1.5T Nova Dual MRI with SENSE~ breast coil, MRI scanning was performed on 21 patients clinically suspected suffering from breast cancer. MRI sequences included T1WI/TSE,T2WI/TSE (turbo spin echo), short TI inversion recovery(STIR),diffusion weighted whole body imaging with background body signal suppression (DWIBS) and dynamic contrast-enhanced MRI scanning THRIVE. MRI findings were compared with histopathological results. Results:The detection rates of breast lesions of the five sequences were 53.85%, 65.38%, 80.77%, 88.46% and 100%. The sensitivity, specificity and accuracy of dynamic contrast-enhanced MRI scanning were 100%, 77.78%, and 92.31% respectively, which were better than other sequences in the differential diagnosis of the lesions. Conclusion:Dynamic contrast-enhanced MRI scanning could accurately show the numbers of the lesions, describe the morph and size of the lesions and provide more information for the surgeons. DWIBS and STIR could detect the breast lesions more sensitively than T1WI/TSE and T2WI/TSE sequences.
2.De Novo urinary and male genital cancers in kidney transplant recipients

Objective To explore the outcomes of kidney transplant recipients who developed urinary and male genital cancers after transplantation. Methods Data of 31 kidney transplant recipients developed de novo urinary and male genital cancer were compared with data of 31 patients in general population with the same age and same tumor stage. Results Compared with the general population, the overall survival was significantly worse in the transplant recipients (P=0. 02) , 5-year survival rates for each group were 50% vs 68%. Multivariate analyses demonstrated cancer stage to be a negative risk factor for survival for transplant recipients with de novo urinary and male genital cancer, and surgery and functioning graft to be the positive survival predictors. Conclusions Transplant recipients experience worse outcomes than the general population from urinary and male genital cancers. Cancers in transplant recipients are more biologically aggressive at the time of diagnosis.
3.Percutaneous vertebroplasty combined with radiofrequency ablation for spinal metastases:a meta-analysis of clinical efficacy and safety

Objective To investigate the clinical efficacy and safety of percutaneous vertebroplasty(PVP)combined with radiofrequency ablation(RFA)for spinal metastases.Methods A computerized retrieval of academic papers concerning the case-control studies(CCS)and randomized controlled trials(RCT)of PVP combined with RFA for spinal metastases from the databases of CNKI,Wanfang,Chinese Biomedical Literature Database(CBM),PubMed,Cochrane Library,Web of science and other databases was conducted.A control group(receiving PVP treatment alone)and a test group(receiving PVP combined with RFA treatment)were established.Newcastle-Ottawa scale(NOS)score was used to evaluate case-controlled trial literature,and the Cochrane risk bias assessment tool was used to evaluate the RCT literature.Results A total of 12 articles(10 CCS articles and 2 RCT articles)including 1 051 patients were included in this analysis.Meta-analysis showed that the recurrence rate of spinal metastases(OR=0.17,95%CI=0.08-0.34,P<0.001),incidence of complications(OR=0.29,95%CI=0.21-0.41,P<0.001)and pain visual analog scale(VAS)score(WMD=-1.21,95%CI=-1.64--0.78,P<0.001)in the test group were significantly lower than those in the control group.The Karnofsky functional status(KPS)score(WMD=14.69,95%CI=-12.25-17.14,P<0.001)in the test group was remarkably higher than that in the control group,but the long-term efficacy in the test group(OR=1.55,95%CI=0.90-2.68,P=0.12)was not obviously better than that in the control group.Conclusion For the treatment of spinal metastases,PVP combined with RFA has significant clinical short-term efficacy and safety,although its long-term efficacy is not obvious.Due to the limited quantity and quality of the included literature,larger-scale and higher-quality clinical trial studies need to be done before the above conclusions can be further clarified.
4.Changes of peripheral blood CD4(+)CD25(+) regulatory T cells and TREG-related Foxp3 gene expression at the onset of chronic allograft nephropathy.

OBJECTIVETo investigate the changes of CD4(+)CD25(+) regulatory T cells (TREG) and TREG-related foxp3 gene expression in the peripheral blood at the onset of chronic allograft nephropathy (CAN) after kidney transplantation.
METHODSTwenty-five patients with initial onset of CAN were examined for CD4(+)CD25(+)high/CD4(+) ratio and the expression of Foxp3 gene in the peripheral blood using the flow cytometry, and the data were compared with those of 30 kidney recipients with normal graft function, 20 patients with chronic renal function (CRF), and 20 normal subjects. All the recipients had no more than 1 HLA mismatch and received the same inductive and maintenance drug treatment.
RESULTSThe recipients with CAN had significantly lower CD4(+)CD25(+)high/CD4(+) ratio and Foxp3 gene expression compared with those with normal graft function (0.71∓0.33 vs 1.17∓0.25 and 62.75∓10.80 vs 70.42∓6.8, respectively, P<0.01). The recipients with normal renal graft function showed no significant difference in CD4(+)CD25(+)high/CD4(+) ratio and Foxp3 gene expression from the normal control subjects.
CONCLUSIONThe peripheral blood CD4(+)CD25(+)high/CD4(+) ratio and Foxp3 expression in the kidney recipients with CAN are significantly lower than those of recipients with normal renal graft function, which does not correlate with elevated creatinine level, suggesting a role of TREG in the occurrence and development of CAN.

5.Effects of aerobic exercise on cognitive function in male patients with chronic schizophrenia

BackgroundThe schizophrenia is majorly treated with drug and through physical therapy. However, both treatments would lead to adverse reactions, which could affect therapy adherence and treatment efficacy. Previous studies have shown that aerobic exercise can help alleviate cognitive function in patients with Alzheimer's disease and depressive disorder. At present, little research has been done on such alleviation in schizophrenia patients. ObjectiveTo explore the effect of aerobic exercise on cognitive function in male patients with chronic schizophrenia, so as to provide references for relevant treatments. MethodsA total of 76 male patients with chronic schizophrenia hospitalized in the Fourth People's Hospital of Wuhu between December 2022 and April 2023 were selected as the study subjects and, in accordance with random number table, divided into study group (n=36) and control group (n=40). Both groups received conventional drug treatment. On this basis, the study group received a 60-minute aerobic exercise 5 times a week for 8 weeks as intervention. Before and after intervention, assessment of cognitive function was performed by using MATRICS Consensus Cognitive Battery (MCCB) and Stroop Color Word Test (SCWT). ResultAfter intervention, compared with the control group, the study group spent less time on finishing the Trail Making Test and scored higher in both the spatial span test and maze test (Z=-2.070, -2.306, -2.375, P<0.05). Repeated measure ANOVA results showed that the time main effect of Hopkins Verbal Learning Test score was statistically significant in the two groups after intervention (F=39.067, P<0.05). So was the interaction effect between the time and group of the Brief Visuospatial Memory Test and Verbal Fluency Test scores (F=10.092, 9.252, P<0.05). After intervention, the scores of the Brief Visuospatial Memory Test and Verbal Fluency Test in the study group were higher than those in the control group (t=6.689, 4.249, P<0.05). As for the study group itself, the scores were higher than those before intervention (t=23.746, 23.842, P<0.05). After intervention, the numbers of correct reading in color test and word test in the study group were more than those in the control group (Z=-2.358, -2.771, P<0.05). The interaction effect between the time and group of the reaction time in color test, word test and color word interference test were statistically significant in both groups (F=23.383, 19.888, 19.662, P<0.05). After intervention, the reaction time in color test and color word interference test of the study group was shorter than those of the control group (t=4.895, 6.163, P<0.05). As for the study group itself, the reaction time were shorter than before intervention (t=54.318, 42.425, 42.141, P<0.01). ConclusionAerobic exercise may help alleviate the cognitive problems in male patients with chronic schizophrenia in terms of information processing speed, working memory, reasoning/problem solving ability, word learning and memorizing, visual learning and memorizing, and executive function.
