1.Predictive Value of NVDA Score on Minimum Lumen Area in Patients With Coronary Artery Intermediate Lesions

Objective: Based on scores by number of vessels diseased and age (NVDA), the minimum lumen area (MLA) of left anterior descending (LAD) proximal or middle intermediate lesions were examined by intravascular ultrasound (IVUS) to analyze the clinical characteristics, to ifnd the factors affecting lumen area and to establish a scoring system for predicting MLA in relevant patients.
Methods: A total of 90 patients were enrolled including 58 male and 32 female with the age of (41-77) years. The demographic information, medical history and laboratory results were studied by simple linear regression analysis to screen relevant factors affecting MLA; multi regression analysis was conducted to establish a regression equation for predicting MLA and to calculate the risk factor coefifcient for obtaining relevant scoring system.
Results: NVDA score≤4 was deifned as negative result with speculated MLA≥3.0mm2, while NVDA score>4 was deifned as positive result with speculated MLA<3.0mm2. The sensitivity, speciifcity and negative predictive value of NVDA scoring system for predicting MLA were 83.35%, 75% and 90% respectively.
Conclusion: NVDA scoring system had the better accuracy, sensitivity and speciifcity for predicting MLA in coronary artery intermediate lesions, it had certain value for guiding coronary interventional therapy in relevant patients.
2.Short-term outcome of rotational atherectomy followed by drug-eluting stenting in heavily calcified ;coronary long lesions

Objective To investigate the safety and short-term outcome of rotational atherectomy followed by drug-eluting stenting in heavily calcified coronary long lesions. Methods From Jan 1, 2011 to May 31, 2016, 109 cases with 114 heavily calcified coronary lesions which were treated with rotational atherectomy followed by drug-eluting stenting in Peking University People's Hospital were included. They were divided into diffuse lesion group ( lesion ≥25 mm, 68 cases, 72 lesions ) and focal lesion group (lesion ﹤25 mm,41 cases, 42 lesions). All patients were followed up in hospital. Procedure parameters, procedural complications ( dissection, perforation, slow flow/no flow and procedural related myocardial infarction),procedural success and major adverse cardiovascular events (cardiac death, non-fatal myocardial infarction and stent thrombosis) were analyzed. Results The procedural success rate was 98. 5%(67/68) in diffuse lesion group and 100% ( 41/41 ) in focal lesion group ( P=0. 453 ) . Complication rates did not differ between the two groups (41. 2% and 34. 1%, P=0. 673). Major adverse cardiovascular events rates were 41. 2% and 31. 7%, P =0. 484. Conclusions Treating coronary lesions ≥25 mm in length with rotational atherectomy followed by drug-eluting stenting does not impact the short-term outcome when treating carefully and correctly . Procedural success rate and in-hospital outcome is satisfactory.
3.Integrated intravascular ultrasound and fractional flow reserve to guide single-stent crossover ;technique for left main and bifurcation lesions

Objective To investigate the relationship between the Minimum Lumen Area (MLA) and plaque burden(PB) of the left circumlfex (LCX) ostial and the occurrence of myocardial ischemia after a single stent crossover for the treatment of left main (LM) bifurcated lesions. Methods Intravascular ultrasound (IVUS) assessment was performed on 5 patients, each where coronary angiography showed left main bifurcated lesions, and to measure the MLA and PB of the LCX. Following a single stent crossover fractional lfow reserve (FFR) evaluation were performed in all 5 patients to investigate the relationship between the MLA and PB of the LCX ostium and the occurrence of myocardial ischemia. Results The mean MLA of the left main coronary of the 5 patients was (4.99±2.3) mm2 with the minimum lumen diameter (MLD) being (2.26±2.8) mm. The average MLA of the left anterior descending (LAD) ostial or proximal was (4.01±2.0) mm2, mean plaque burden (PB) at the LAD ostial or proximal was (68.15±10.1)%. Average MLA of LCX was (4.94±0.4) mm2 with a plaque burden of (66.00±6.0)%. Single stent crossover technique was used to treat the bifurcated lesions. Among the 5 patients, only 1 of them was treated with a double stent deployment where the LCX ostial FFR was<0.75 (FFR=0.42). The remaining 4 patients showed LCX ostial FFR of>0.75. Conclusions Integrated IVUS and FFR would achieve threshold measurements of MLA at LCX’s ostium which could predict ischemia after one stent strategy.
4.Effect of high temperature requirement protease A1 lentivirus expression vector infection on biological behavior of human vascular smooth muscle cells

Objective To construct the lentivirus vectors of high temperature requirement protease A1 (HTRA 1) gene as well as it's mutant gene (1091 T>C HTRA 1),and investigate the effect of their infection on proliferation,migration and apoptosis of human brain vascular smooth muscle cells (HBVSMC).Methods The lentivirus vectors of wild type HTRA1 and mutant HTRA1 were constructed,cultured and identified,and then,infected into HBVSMC.Empty lentivirus vectors were used as controls.CCK-8 method was used to detect the cell proliferation for a consecutive five d.Transwell assay was used to detect the cell migration,and cell apoptosis was detected by flow cytometry.Results As compared with those from the control group,cells from wild-type HTRA 1 group showed no statistically different proliferation rate (P>0.05),while cells from mutant HTRA 1 group had significantly reduced proliferation rate since the third d (P<0.05).Migration rate of cells from control group,wild type HTRA 1 group and mutant HTRA 1 group was 0.474±0.079,0.612±0.037,and 0.283±0.064,respectively,with significant differences (P<0.05).Apoptosis percentage of cells from control group,wild type HTRA 1 group was 3.68%±0.23% and 3.13%±0.07%,with significant difference (P<0.05),while that of mutant H TRA 1 groupwas3.70%±0.20%,showingnosignificantdifferenceascomparedwiththatfromcontrolgroup (P>0.05).Conclusion HBVSMC infected by HTRA 1 mutant gene display attenuation of proliferation and migration activity,but have no statistical change in apoptosis.
5.Methodological analysis of sequential sector scan through oral fissure for fetal cleft palate screening

Objective:To explore the method of sequential sector scan with 2D ultrasound through oral fissure (SSTOF) and its utilization in the cleft palate screening.Methods:Based on features of oral anatomy and ultrasonic beam, SSTOF was designed to screen cleft palate and the accuracy had been verified using specimens of aborted fetuses. This study recruited 7 154 women with singleton pregnancy who were screened for fetal malformations during 20-28 gestational weeks in the First Affiliated Hospital of Anhui Medical University from May 2020 to October 2020. In medical addition to routine screening, these subjects also underwent SSTOF to further verify its feasibility and imaging performance. Follow-up was performed by telephone and medical record review.Results:Clear images of the upper palate were acquired in five specimens of induced fetuses using SSTOF. Except for 56 cases lost to follow-up, a total of 7 098 fetuses were finally enrolled, of which 6 885 acquired satisfactory images using SSTOF, 213 did not due to inappropriate position, with a success image rate of 97%. SSTOF found cleft palate in 31cases, which were all confirmed after birth or induction, noting an accuracy rate of 100%.Conclusion:Sequential sector scan through oral fissure has a high clinical value on cleft palate screening in the second trimester with advantages of clear image, easy operation, and access to section views.
6.Effects of mind map combined with physical nursing in the prevention and treatment of stroke for elderly patients with hypertension in community

Objective To discuss the effect of mind map combined with physical care in the prevention and treatment of stroke for elderly patients with hypertensionin community.Methods A total of 100 cases of elderly patients with hypertension from March to September 2015 who met the inclusion criteria were included in the study. According to random numbers generated by the computer, patients were randomly divided into intervention group and control group, 50 cases respectively. The two groups were followed up routinely. Patients in the intervention group were guided by the mind map combined with physical care. The mind map was made and designed to assess the patient's constitution. Individualized health guidance was developed according to the patient's physique identification. The intervention time was 24 weeks. The self-management behavior,systolic blood pressure, diastolic blood pressure, biased syndrome, and stroke morbidity before and after the intervention were compared between the two groups.Results Before the intervention, there were no significant differences in self-management behavior, systolic blood pressure, diastolic blood pressure, biased syndrome and stroke morbidity between the two groups (P>0.05). After the intervention, dietetic adjust and nursing, sports health, emotion transfer, daily life transfer, regularly blood pressure checking in the intervention group were significantly better than those in the control group; and the scores of self-management behavior increased compared with those before the intervention; BMI, systolic blood pressure, diastolic blood pressure, and biased syndrome score were significantly lower than those in the control group and before the intervention; Control rate of blood pressure was 78% in the intervention group, which was significantly higher than that in the control group (46%) (P<0.05). The incidence of stroke was 20% in the intervention group, which was lower than that in the control group (40%), with no statistically significant difference between the two groups (P>0.05). Conclusions Mind map combined with physical care can improve the self-management behavior of patients with hypertension in the community, and reduce diastolic blood pressure, systolic blood pressure and biased syndrome. However, the effect in the prevention of stroke requires further investigations.
7.The diagnostic value of genetic testing in familial hypercholesterolemia in patients with premature myocardial infarction

Objective:To evaluate the diagnostic value of gene testing in familial hypercholesterolemia (FH) in patients with premature myocardial infarction(PMI).Methods:This study was a single center cross-sectional study. A retrospective analysis was made on PMI patients who visited the People′s Hospital of Peking University from May 1, 2015 to March 31, 2017. Clinical data of patients was collected and gene testing of FH related genes low density lipoprotein receptor (LDLR), proprotein convertase subtilisin/kexin type 9 (PCSK9), apolipoprotein B(APOB) and low density lipoprotein receptor adaptor protein 1(LDLRAP1) was carried out. Clinical diagnosis of FH patients was performed using Simon Broome criteria, DLCN criteria, and FH Chinese expert consensus.Results:There were 188 males (83.6%) among 225 PMI patients, and the age of the first myocardial infarction was (46.6±7.2) years old. Ten patients carried FH pathogenic or possibly pathogenic mutations (4.4%). Compared with Simon Broome standard, DLCN standard and FH Chinese expert consensus, gene testing increased the diagnostic rate of FH by 53.3%, 33.3% and 42.1% respectively.Conclusion:Gene testing is helpful to improve the diagnosis of FH, and it is important to start the standard treatment of FH as early as possible in patients with premature myocardial infarction.
