1.Clinical Significance of Determination of Serum CoX-2,IL-8 and GM-CSF Levels in Pediatric Patients with Bronchial Asthma

Objective To explore the changes of serum COX-2,IL-8 and GM-CSF levels in asthmatic children and their clinical significance.Methods The serum COX-2,IL-8 and GM-CSF levels of 86 asthmatic children,including 42 cases in the acute attack group and 44 cases in the remission group,and 40 healthy controls were detected by sandwich ELISA.Results The serum COX-2,IL-8 and GM-CSF levels in the asthmatic group significantly increased(P<0.05).The serum COX-2,IL-8 and GM-CSF levels in the acute attack group were higher than those in the remission group(P<0.05);the serum COX-2,IL-8 and GM-CSF levels in both acute attack group and remission group were higher than those in control group(P<0.05).Conclusions These cytokines participated in the pathogenesis of bronchial asthma.Monitoring the changes of their serurm levels was helpful for the management of the diseaSes.
2.Death patterns of the mouse fibroblast cell L929 induced by ferrite stainless steel in magnetic attachment

BACKGROUND: It has been reported that 26-1 ferrite stainless steel has excellent biological characteristics, but the report of its biocompatibility is rare, especially the evaluation of biocompatibility by apoptosis mechanism. OBJECTIVE: To study the pattern of call death induced by magnetic-attached 26-1 ferrite stainless steel and to evaluate whether 26-1 ferrite stainless steal meats the biocompatibility in clinical application. DESIGN, TIME AND SETTING: A contrast observational study at the level of molecular biology was performed at Central Laboratory of China Medical University from June 2004 to June 2005.MATERIALS: 26-1 ferrite stainless steel (Fe - 26% Cr - 1% Mo), Tilite [70% Ni - 16% Cr - (4%-6%) Ti - ether alloy elements] and Co-Cr alloy (Co - 30% Cr - 5% Mo) were used in this study. Embed and cast metal materials then were cut into roll discs whose diameters were 12 mm.METHODS: ① The fibroblast L929 cells were co-cultured with the three materials, while cells in the negative control group were cultured in RPMI1640 media. At 18 and 24 hours after culture, types of cell death were quantitatively detected using fluorescein isothiocyanate and propidium iodide (PI) staining. ② Apoptosis rate and cycle distribution of L929 cells were detected using flow cytometry and Pl staining at 24 hours after respectively culturing in 25%, 50%, and 100% leaching liquor. MAIN OUTCOME MEASURES: Ratio between apoptosis and necrosis of L929 cells induced by different materials; apoptotic rate and cycle distribution of L929 cells.RESULTS: All the three materials could decrease the survival rate of L929 cells: in particular, there was no significant difference in necrosis rate between different concentration material groups and negative control group, suggesting that call apoptosis was the major factor to induce cytopathic effect, but there was no significant difference between the three material groups. Apoptosis of L929 cells indicated a great fashion of time- and concentration-dependence. There was no significant difference in apoptosis rate of L929 cells betwean the three material groups.CONCLUSION: Cell apoptosis is a major pattern induced by 26-1 ferrite stainless steel which meets the biocompatibility as a clinical material and can be safely applied in dinic.
3.Repair for chronic radioactivity ulceration on chest wall with transverse rectus abdominis musculocutaneous flaps(TRAM)

Objective To study and solve the repairing and reconstruction characteristic for chronic radioactivity ulceration on chest wall.Methods Using transverse rectus abdominis musculocutaneous flaps (TRAM) to repair and reconstruct 12 cases serious chronic radioactivity ulceration after radical operation of mastocarcinoma.Including 6 cases using single pedicle TRAM flaps, 6 cases using double pedicle TRAM flaps.Results All 12 cases were applied successfully with 100% survived and were followed-up for 1 to 4 years.There were better colour, texture, elasticity on flap and obviously improvement for cicatricial tissue round ulceration.Conclusion It is better choice for repairing chronic radioactivity ulceration on chest wall with transverse rectus abdominis musculocutaneous flaps and it is also reliable method for flap circulation using double pedicle TRAM flaps by vascular anastomosis.
4.Management of disorder of temporomandibular joint syndrome using elastic resin occlusal pad: a clinical observation

Objective To investigate the possibility of occlusal pad in treating disorder of temporomandibular joint syndrome.Methods We treated 25 patients with disorder of temporomandibular joint syndrome using occlusal pad made of elastic resin,and evaluated the effects.Results Total effective rate was 96% .Clicking and pain dissappeared in all patients.Conclusion Occlusal pad made of elastic resin could be used for treating disorder of temporomandibular joint syndrome in clinic.
5.Laparoscopic Repair of Esophageal Hiatal Hernia

Objective To investigate the clinical effects of laparoscopic repair of esophageal hiatal hernia using Bard CruraSoft PTFE/ePTFE Mesh combined with Toupet partial fundoplication.Methods From August 2006 to April 2007,13 patients with esophageal hiatal hernia(typeⅠin 6 and type Ⅲ in 7)were treated by laparoscopy in our hospital.Under a laparoscope,esophageal hiatal hernia was separated by ultrasonic scalpel,and then repaired using Bard CruraSoft PTFE/ePTFE Mesh with Ethicon Endopath Multifieed Stapler.Afterwards,Toupet partial fundoplication was performed.Results The operation was completed in all the cases without converting to open surgery.The mean operation time was 142 min(115-185 min);mean intraoperative blood loss was 75 ml(25-120 ml);mean time to the first flatus and oral feeding was 32 h(26-37 h);and mean postoperative hospital stay was 4 d(3-6 d).The patients were followed up for 4-11 mon(mean 6.5 mon).The symptoms disappeared in 1 month.Three months after the operation,barium examination found no recurrence of the hernia in the 13 cases.Conclusions Laparoscopic repair of esophageal hiatal hernia with mesh combined with Toupet partial fundoplication is a safe and minimally invasive operation.The method is worth being widely used.
6.Preliminary application of LigaSure vessel sealant system in hemorrhoidectomy for patients with Grade 3 and Grade 4 hemorrhoids

Objective To evaluate the use of LigaSure vessel sealant system on hemorrhoidectomy for the patients with grade 3 and grade 4 hemorrhoids. Methods Sixty five patients with hemorrhoids (41 males and 24 females), among whom 46 cases were grade 3 and 19 cases were grade 4, underwent hemorrhoidectomy under local anesthesia with the use of LigaSure vessel sealant system. The time consumed for operation and the blood volume lost during and after operation were recorded. Postoperative pain was assessed by means of visual analog score (1-10). The patients were followed up for 2 months. Results The operations were performed successfully in all patients. The time consumed was 5-25 minutes (9.7?4.6 minutes). The pain scores at 4 hours and 24 hours after operation were 1-7 (2.7?1.7) and 0-5 (1.9?1.3), respectively. On the third day after operation, only 4 cases had a pain score of 3 to 5, in the remaining 61 cases the score was under 3. On the seventh day after operation, only 5 patients had pain scores of 1 to 3. No patients complained pain about 2 months later. The volume of blood loss in 9 cases was less than 15ml (15.8%), and in the remaining 56 cases there was no apparent blood loss during the operation. Only one patient was re-operated for bleeding after operation, and in 18 (27.7%) patients there was only oozing of blood. Mild incontinence occurred in 6 patients shortly after operation, and no severe complications occurred. Conclusions LigaSure vessel sealant system shows the advantage of good haemostatic effect, less time consuming for operation, and less pain after operation for the treatment of patients with Grade 3 and Grade 4 hemorrhoids.
7.Study on the relationship between degree of venous injury and the indwelling time by injection of chemotherapy drugs with intravenous catheter system

Objective To observe the relationship between degree of venous injury and the indwelling time by injection of chemotherapy drugs with intravenous catheter system. Methods Totally 43 New Zealand rabbits were selected as the study sample. Among them, three rabbits were chosen by lot as a blank control, the others were evenly divided into two groups randomly. The veins at the edges of the two ears were taken out as the tested blood vessels for the infusion with intravenous catheter system. The two groups were injected respectively with physiological saline and chemotherapy drugs once daily, and then blocked with heparin salt water. After intraperitoneal anaesthesia, pathological sections were prepared by taking live samples from the ear vein where intravenous catheter system puncturing on the 2nd, 4th, 6th,8th and 10th day of transfusion. And inflammation and thrombosis in the vein had been observed under the light microscope. Results In the same indwelling time, the inflammatory response of the two groups was compared: no difference on the 2nd day; difference was seen on the 4th、6th and 8th day; but again no difference on the 10th day. Thrombosis compared: no difference on the 2nd, 4th day, but difference was seen on the 6th, 8th and 10th day. Conclusions Chemotherapy drugs has a strong irritant on blood vessels.The indwelling time should not be too long, generally advisable 2 days,even if no abnormal response is observed locally by the naked eye.
8.Effects of Qibai Pingfei Capsules on Up-regulating Expression of 5-HT1AR mRNA, DA-1 and DA-5 in Hippocampus of Rat Model with COPD Accompanied with Depression

Objective To investigate the effects of Qibai Pingfei Capsules(QPC) on the expression of monoamine neurotransmitters of 5-hydroxytryptamine receptor(5-HT) and dopamine(DA) receptors of DA1 and DA5 in the rats with chronic obstructive pulmonary disease (COPD) accompanied with depression, and to explore the possible therapeutic mechanism. Methods Ninety male Wistar rats were randomly divided into 6 groups, namely normal group, COPD depression group, QPC group, Venlafaxine group, and QPC plus Venlafaxine group. The rat model of COPD accompanied with depression was established by compound methods of smoking, food deprivation, water deprivation, forced swimming, shaking, and clipping tail. And then the effects of QPC on the lung function and the general characteristics were observed. Furthermore, the mRNA level of 5-HT1AR was determined by real-time quantitative reverse transcription PCR(RT-qPCR), and protein levels of DA-1 and DA-5 in rat hippocampus were assayed by Western blot analysis. Results Compared with the normal group, forced expiratory volume in 0.3 second(FEV0.3), the forced volume capacity(FVC) and FEV0.3/ FVC in the COPD depression group were significantly reduced (P < 0.01). Compared with the COPD depression group, the values mentioned above were restored to different extents in the groups of QPC, Venlafaxine, and QPC plus Venlafaxine(P<0.01). Compared with the normal group, the expression levels of 5-HT1AR mRNA, and DA-1 and DA-5 protein in the COPD depression group were significantly down-regulated, and their levels were also slightly decreased in the treatment groups(P<0.05). Compared with the COPD depression group, the expression levels were up-regulated to various degrees in all of the treatment groups (P < 0.05). Conclusion QPC can increase the mRNA levels of 5-HT1AR and protein levels of DA-1 and DA-5 in the hippocampus of COPD with depression.
9.Prognostic factors for diabetic foot ulcer

Objective To identify the factors related to prognosis of diabetic foot ulcer (DFU).Methods A total of 186 patients with type Ⅱ DFU from a single diabetic foot center was included in this prospective study.Follow-up of the final outcome (healing,major amputation or death) was made in 6 months.Influence of patient demographics and clinical data on outcome was detected using multivariate Logistic regression analysis.Results Follow-up was performed in 172 patients,of whom 147 were cured (55 cases cured after minor amputations),3 underwent major amputations,6 died,and 16 were not yet cured at the final follow-up.In multivariate Logistic regression analysis,the outcome was independently correlated with ischemia (P <0.01),infection (P < 0.05),ulcer number (P < 0.01) and peripheral neuropathy (P < 0.05) ; the risk of poor outcome increased with ischemia [odds ratio (OR) =10.8],infection (OR =211.4),ulcer number (OR =39.5),and peripheral neuropathy (OR =181.1).Conclusion Prognosis of DFU is associated with ischemia,infection,ulcer number,and peripheral neuropathy.
10.Effect ofXiao-Qing-LongDecoction on AngⅡ, ALD, Na+-k+-ATPase Contents and Expressions of AT1 and AT2 mRNA in Cor pulmonale Rats

This article was aimed to study the effect ofXiao-Qing-LongDecoction (XQLD) on plasma AngⅡ, ALD, Na+-k+-ATPase content, and plasma AT1 and AT2 mRNA expressions in Cor pulmonale rats, in order to further explore the mechanism of ventilating lung qi for diuresis. A total of 60 Wistar rats were randomly divided into the normal group, model group and XQLD, with 20 rats in each group. The enzyme-linked immunosorbent assay (ELISA) method was used to determine the AngⅡ, ALD and Na+-k+-ATPase content. The fluorescence quantitative PCR was used to detect the AT1 and AT2 mRNA expression. The results showed that compared with the normal group, the AngⅡ, ALD and Na+-k+-ATPase contents in the model group were significantly increased (P< 0.05, orP< 0.01). Compared with the model group, the AngⅡ, ALD and Na+-k+-ATPase contents in the XQLD group were obviously decreased (P< 0.05). Compared with the normal group, AT1 mRNA expression was increased; and AT2 mRNA expression was decreased in the model group (P<0.05, orP< 0.01). Compared with the model group, AT1 mRNA expression was decreased; and AT2 mRNA expression was increased in the XQLD group (P < 0.01). It was concluded that XQLD can effectively regulate the AT1 and AT2 mRNA expressions, influence ALD content to ventilate lungqi for dieresis.
