1.Clinical observation of a conservative treatment for large keratocystic odontogenic tumors in the mandible: enucleation followed by open packing.

OBJECTIVEThe aim of this retrospective study is to present the long-term effects of open healing of keratocystic odontogenic tumors (KCOTs) in the mandible.
METHODSA retrospective case series study was conducted on 41 patients with large KCOTs (the maximum diameter of the tumors exceeded 5 cm) treated at our institution between September 2003 and April 2011. A conservative surgical treatment was applied. The treatment involved enucleation of the primary lesion through narrow unroofing and open packing of the residual osseous defect with iodoform gauze for secondary healing. Bone regenera- tion and surgical complications were observed. The long-term effects of the treatment were followed up.
RESULTSThe inferior alveolar nerve was exposed in the KCOT bone cavity in all cases, and some nerves adhered to the tumor tightly. The post- operative follow-up time was 81.5 months on the average (36 to 127 months). The packing gauze was changed every two weeks after enucleation, and the total duration time for packing was 8.9 months on the average (3 to 15 months). Notable bone regeneration and satisfactory secondary healing were observed clinically and radiographically. The KCOT-affected teeth were reserved, and their chewing functions were restored. Two cases presented recurrences after the initial treatment. The recurrence rate was 4.9% (2/41). No serious complications were observed.
CONCLUSIONEnucleation associated with subsequent open packing is a reliable treatment for patients with large KCOTs in the mandible.

Objective To explore the abnormal changeof the polysomnography(PSG) in the patientwith traumatifrontal lobe injury .MethodTotally 16 patientwith traumatifrontal lobe injury accompanying sleep disorde(brain traumgroup) and 20 individualof physical examination (control group) were performed the whole nighPSG .ResultCompared with the control group ,the incubative stage of sleep in the brain traumgroup waprolonged ,while the sleep time and slow wave sleep time were shortened .The rapid eye movemen(REM) time and the periodicity of REM had statistical differencebetween the two group,buthe REM incubative stage ,density and intensity of REM had no statistical differencebetween the two goup.Conclusion The changeof PSG in the patientwith traumatifrontal lobe injury are dominated by the extension of the REM time and the shorten-ing of REM periodicity .
3.The effect evaluation of medicine treatment combined with psychological intervention for sleep disorders patients after acute brain stroke

Objective To observe the clinical curative effect of drug combined with psychological treatment on patients with sleep disorders after stroke.Methods A total of 180 cases of eligible patients with sleep disorders were included and randomly di-vided into control group and treatment group.Control group were treated with conventional vascular drug(estazolam,2 mg,before sleep);treatment group increased anxiolytic and sedative hypnotic drugs(venlafaxine,dexzopiclone)compared to control group.Af-ter 14 days,sleep quality,NIHSS were compared both within the treatment group and between the two groups.Results The Bar-thel index,NIHSS and PSQI of two groups had significant differences,and those index of treatment group before and after treat-ment had significant differences too(P <0.05).Conclusion Combined conventional cerebrovascular drugs with Venlafaxine,dexzo-piclone as well as psychological treatment significantly improved sleep quality of patients with acute stroke.

Objective Research on traumatic the related factors of sleep disorder after traumatic brain injury ,in order to pro-vided the rationale for the diagnosis and treatment .Methods The SPIEGEL was used to evaluate the traumatic brain injury patients who were hospitalized .Recording time in sleep disorders in 3 months .Analysis the relations between the sleep disorders and brain injury site by combining with the patients head CT and MRI .Results Seen in 200 cases of patients with sleep disorders of 105 cases (52 .5% );71 cases appeared in patients within 1 week after waking ,accounted for 76 .19% ;The brain stem ,frontal lobe and basal ganglia injury occurred sleep disorders were more likely(66 .7% ,64 .0% ,70% ) .The difference was statistically significant(P<0 .05) .Conclusion Sleep disorder is a common clinical symptom of mild traumatic brain injury .a time to focus on the patients with-in 1 week after waking ,and closely related to brain stem ,frontal lobe and basal ganglia injury .
5.Preoperative neutrophil-lymphocyte ratio predicts clinical outcome in patients with high grade T1 bladder cancer

Objective To assess the value of preoperative neutrophil-lymphocyte ratio ( NLR) for predict the prognosis in patients with high grade T1 bladder.Methods From January 2004 to December 2014, the data of 307 patients diagnosed as bladder cancer of Stage 1 and high grade after undergoing TURBT were analyzed, including gender, age, smoking status, tumor number and size, hydronephrosis, intravesical instillations and preoperative blood transfusion of 307 patients diagnosed as bladder cancer of stage 1 and high grade after undergoing TURBT were analyzed retrospectively.All patients were primary urothelial carcinoma.According to preoperative NLR,patients were divided into the low NLR group( NLR≤2.42,n=197) and the high NLR group(NLR >2.42,n =110).Recurrence-free survival (RFS) and progression-free survival ( PFS) were calculated according to the Kaplan-Meier model and compared by the log-rank model.Cox regression models were used for multivariate analyses of the association between NLR and bladder cancer, then the prognostic factors affecting RFS and PFS were evaluated.Result of these 307 patients, the low NLR group accounted for 64.2%(197/307), and the high NLR group accounted for 35. 8%(110/307).The mean follow-up period was 71(range, 1-123)months.The recurrence rate in the low NLR group and the high NLR group recurrence rate were 19.2%( 38/197 ) and 34.5%( 38/110 ) respectively, RFS were 73.0(range, 2-123)months and 67.5(range, 1-122)months respectively.The progression rates were 4.1%(8/197) and 10.9%(12/110) respectively.The recurrence and progression rates in the high NLR group is higher than those in the low NLR group(P<0.01 and P=0.008), and RFS was shorter( P=0.002).Multivariable Cox regression analysis showed that NLR>2.42(P=0.007,HR=1.912)and hydronephrosis (P<0.01, HR =2.485 ) are associated with higher risk of recurrence.Conclusion Elevated preoperative NLR is an independent predictor of RFS and PFS in patients with high grade T1 bladder cancer.
6.Application of Delayed Surgical Managements in Patients with Stensen's Duct Injury

The study aimed to retrospectively evaluate surgical treatment outcomes after delayed parotid gland and duct injuries.Nine patients subjected to parotid gland and duct injuries with 1-to 3-month treatment delay were retrospectively evaluated with special reference of etiology,past medical history,and injury location.Conservative treatment,microsurgical anastomosis,and diversion of salivary flow or ligation were chosen for delayed parotid gland and duct injuries concerning to their site of injury,time of repair and procedures.Assistant treatment as pressure dressing was adopted thereafter.All patients experienced an uneventful recovery at the time of finalizing the study.Two patients received Stensen's duct ligation,5 received microsurgical anastomosis and 2 accepted salivary flow diversion for 5 patients with sialoceles and 4 patients with fistulas,and no re-occurrence was found.Facial paralysis occurred after surgery in 4 patients,and 3 of them recovered after the nerve nutrition treatment.Our study suggested that appropriate surgical treatment is efficient for the re-establishment of the tissue function and facial aesthetic for delayed injury of the parotid and its duct.
7.Study on relationship between length of homologous sequences and chromosomic recombination rate in Saccharopolyspora erythraea.

In order to study the relationship between lengths of homologous fragments and chromsomic recombination rate in Saccharopolyspora erythraea, three homologous sequences, with mutant loci and different flanking sequences, (26bp + 27bp), (500bp + 576bp) and (1908bp + 1749bp), were synthesized by chemical reaction or PCR amplification, and cloned into pWHM3 to construct homologous recombination plasmids, pWHM1113, pWHM1116 and pWHM1119. When the plasmids were transformed into protoplast of Saccharopolyspora erythraea A226 under PEG mediated, on an average 30, 69 and 170 transformants grew on each plate for the three plasmids respectively, but chromosomic integration frequency were 0, 2% and 19% among corresponding transformants. Both pWHM1116 and pWHM1119 could take double crossover recombination, and exchange the mutant loci in the chromosome. It was concluded that when the flanking sequences were equal or more than (500bp + 576bp), they could take effective single and double recombination with Saccharopolyspora erythraea chromosome.

8.Effects of electromagnetic fields of different frequencies on proliferation and DNA damage of gallbladder cancer cells.

OBJECTIVETo study the effects of low-power electromagnetic fields of different frequencies on proliferation and DNA damage of gallbladder cancer cells.
METHODSThe cell growth curve was drawn and single cell gel electrophoresis performed to evaluate the proliferation and DNA damage of gallbladder cancer cells respectively after the cells were exposed to electromagnetic fields of different frequencies.
RESULTSAfter exposure to low-power electromagnetic fields of different frequencies (0.1-40 MHz), the cells displayed significant changes with obvious cell proliferation inhibition and DNA strand breakage.
CONCLUSIONLow-power electromagnetic fields within the range of 0.1-40 MHz may impair the DNA strand and cause inhibition of proliferation of the gallbladder cancer cells, and these effects are related to the frequency of the electromagnetic fields but not in a linear fashion.

10.Flavonoid glycosides from dried and nearly ripe fruits of Evodia rutaecarpa.

OBJECTIVETo study the chemical constituents from the dried and nearly ripe fruits of Evodia (Euodia) rutaecarpa.
METHODThe compounds were separated and purified by solvent and chromatographic methods. Their structures were identified by spectroscopic techniques.
RESULTFifteen compounds were separated from the normal butanol extracts of the 70% aqueous ethanol extract of the dried and nearly ripe fruits of E. rutaecarpa. Among of them, four compounds were reported in the essay and identified as diosmetin-7-O-beta-D-glucopyranoside (1), isorhamnetin-3-O-rutinoside (2), diosmin (3) and chrysoeriol-7-O-rutinoside (4).
CONCLUSIONCompounds 1, 3 and 4 were separated from the dried and nearly ripe fruits of E. rutaecarpa for the first time.
