ObjectiveTo identify the framework for assessing health related quality of life(HRQOL) of Kaschin-Beck disease(KBD),in order to reflect the impact of KBD on quality of life in patients with the disease.MethodsQualitative descriptive research was adopted.Semi-open ended questions were developed by using the World Health Organization(WHO) definitions of health and quality of life.Group interview and face to face interviews were conducted on 48 patients with KBD and 29 health care experts on KBD in Linyou and Yongshou counties,Shaanxi province,which were higher prevalence areas of KBD.Content template analysis was conducted and the template was based on the WHOQOL-100's framework.ResultsThe framework of HRQOL for KBD included four domains:physical activity,familial/social support,economic and psychological state.There were also eleven facets which were:pain and discomfort,physical function and activity limitation,diet and sleeping,social relationship,concerns of family responsibilities,social support,economic,housing and the surrounding environment,appearance concerns,mental health,and general state of health.The total entries were 69.ConclusionsThe framework for assessing HRQOL of KBD is established.The framework highlights the impact of KBD on the patients' quality of life with higher specificity.

OBJECTIVETo investigate the prevalence and antibiotic resistance of Shigella isolated from children with diarrhea for the guidance of clinical treatment and prevention and control of bacillary dysentery.

METHODA total of 156 strains of Shigella were isolated from feces of children with diarrhea in Zhejiang Xiaoshan Hospital from January 2008 to December 2010. The antimicrobial resistance of the strains was detected by disk diffusion method and the extended-spectrum beta-lactamases (ESBLs) in these isolates were determined using phenotypic confirmatory test; the isolates of ESBLs producing Shigella sonnei were analyzed by REP-PCR.

RESULTAmong 156 strains of Shigella isolated, the most common groups were Shigella sonnei (130 strains, accounting for 83.3%) and Shigella fleaneri (26 strains, accounting for 16.7%), and 81 (51.9%) strains were identified as ESBLs producers, and the positive rates in 2008, 2009 and 2010 were 32.0%, 41.4% and 59.8%, respectively. The results of antibiotic susceptibility test displayed that the resistance rates of ESBLs producing Shigella to ampicillin, cotrimoxazole, cefotaxime, piperacillin were higher than 90%. However, the resistance rates to cefepime, ceftazidime, levofloxacin and ciprofloxacin were low; The resistance of ESBLs producing strains to piperacillin (100% vs. 77.3%), cefotaxime (100% vs. 0), ceftazidime (14.8% vs. 0), cefepime (28.4% vs. 0), cotrimoxazole (95.1% vs. 86.7%) was significantly higher than that of non-ESBLs producing strains (χ(2) = 20.605, 156.000, 12.037, 24.979, 45.040, respectively; P < 0.05). No isolate was resistant to piperacillin/tazobactam and imipenem. There were 7 genotypes among 74 ESBLs producing Shigella sonnei, respectively type A (50), type B (12), type C (8), type D (1), type E (1), type F (1), and type G (1).

CONCLUSIONThe isolation rate of ESBLs-producing isolate was high in Shigella from pediatric patients with diarrhea, and the number is going up year by year, and these ESBLs producing Shigella sonnei strains in genotype A are dominant in recent years, Piperacillin/tazobactam is the drug of choice for children with ESBLs producing Shigella infection.

OBJECTIVETo investigate the displacement, the position of rotation center and the stress distribution of PDL under different loading force system (Moment/Force, M/F) by simulating clinical loading force system.

METHODSA three-dimensional finite element model of upper central incisor, which consists of 945 isoparametric elements and 1,245 nodes was developed. The displacement, the position of the rotation center and the stress distribution of PDL were analyzed under 13 types of loading force system.

RESULTS1. Different force system led to different types of tooth movement. When M/F= -9.15:1, -10.30 - -10.50:1 and -10.90:1, it brought the result of controlled tipping movement, the bodily movement and the root movement; 2. The graph of the center rotation was a hyperbolic asymptotic line: Mx/Fy = -10.50 (horizontal axis) and L = 6.75 (vertical axis). Moreover, a little change of M/F between -9.15 and -10.90 led to apparent change of the position of rotation center. 3. The maximum strain and stress during the tipping movement were 1.47 x 10(-2) MPa and -2.81 x 10(-2) MPa, and during the bodily movement the results were 1.10 x 10(-2) MPa and -1.86 x 10(-2) MPa, while during the root movement were 0.96 x 10(-2) MPa and -2.58 x 10(-2) MPa.

CONCLUSION1 . Different force system (M/F) leads to different types of tooth movement. 2. It is necessary to adjust the force system accurately to obtain prescient tooth movement, especially when M/F changes between -9.15:1 and -10.90:1. 3. This study suggested that the tooth movement style and the force system (M/F) should be controlled to protect the periodontal tissue.

OBJECTIVETo investigate relationship between glucose metabolic disorders and expression of insulin receptor (IR) and tyrosine protein kinase (TPK) in posthepatitic cirrhosis hepatocyte and HBV DNA expression in pancreatic cells.

METHODSTo detect HBV DNA in paraffin-embedded pancreatic and hepatic tissues from 12 posthepatitic cirrhosis patients with positive serum HBV markers by using in situ hybridization (ISH) with a digoxigenin labelled probe. The amount of IR and TPK have been evaluated by immunohistochemical quantitative analysis using image analyzer in hepatocyte of 12 patients positive for HBV markers with posthepatitic cirrhosis in serum. Immunofluorescent histochemical double staining technique was used. HBsAg and IR were observed under confocal laser scanning microscope.

RESULTSEleven of 12 cirrhosis patients? hepatocytes were HBV DNA positive, including 7 patients (7/7) with impaired glucose tolerance (IGT) and 4 patients (4/5) with normal glucose tolerance (NGT). Eight of 12 pancreatic cells were HBV DNA positive, including 7 patients (7/7) with IGT, but only one patient (1/5) with NGT-HBV DNA was found positive in pancreatic cells in significantly more subjects in IGT group than in NGT group (P less than 0.01).IR and TPK amount in hepatocyte of IGT was significantly less than that of NGT patients with posthepatitic cirrhosis (P less than 0.01). IR amount was closely related to the TPK in cirrhosis hepatocyte r=0.82597(P less than 0.01). HBV DNA was mainly localized in the nuclei of hepatocyte and pancreatic acinar and islet cells. Immunofluorescent histochemical double-staining showed that HBsAg was partly localized in the IR positive areas of hepatocytes and pancreatic islet cells.

CONCLUSIONHBV can invade acinar cells of pancreas and islet cells, which might be a direct cause of insulin-dependent diabetes mellitus-like the disorder and insulin absence after HBV infection. Decrease of IR and TPK might be main cause of noninsulin-dependent diabetes mellitus-like disorder after having hepatitis or posthepatitic cirrhosis.

The aim of this study was to explore the regulatory function of interleukin-6(IL-6) on human Th17 cells. Human peripheral blood CD4(+) T cells were purified from healthy donors by anti-CD4 monoclonal antibody (mAb) conjugated microbeads. The experiment was divided into 2 groups. Test group in which CD4(+) T cells (1 × 10(6)/ml) were stimulated by human recombined IL-6 (20 ng/ml) for 4 days; control group in which CD4(+) T cells did not stimulated by IL-6. The concentrations of IL-17 protein in the supernatants were assayed by enzyme-linked immunosorbent assay (ELISA), and quantity of Th17 cells were detected by flow cytometry. The results showed that as compared to control group, IL-17 protein level in the supernatants of CD4(+) T cells significantly increased in IL-6 stimulated group: (337.05 ± 189.09 pg/ml; vs 15.07 ± 12.70 pg/ml) (p < 0.05). Furthermore, the percentage of Th17 cells in cultures of CD4(+) T cells stimulated by IL-6 was significantly higher than that in control group (4.05% ± 0.30% vs. 2.81% ± 0.44%)(p < 0.01). It is concluded that IL-6 promotes the expansion of Th17 cells in vitro.

Purpose The aim was to examine c-MET,ALK,ROS1 variants in advanced non-small cell lung cancer (NSCLC) patients,and to analysis the association of c-MET,ALK,ROS1 variants with the clinical and pathological features.Methods The c-MET,ALK,ROS1 were detected by fluorescence in situ hybridization (FISH) in the 91 cases of NSCLC specimens.The correlation of c-MET gene amplification with clinicopathological features and the ALK,ROS1 fusions was analyzed.Results The positive rate of c-MET gene amplification was 8.79％ (8/91),the positive rates on male and female were 1.82％ and 19.4％,respectively.In ＜ 60-years-old and ≥60-years-old NSCLC patients,the positive rates were 7.5％ and 8.89％,resepectively.The positive rate was higher in stage Ⅲ than stage Ⅳ (9.62％ vs 7.69％),the c-MET gene amplification was detected in 9.2％ adenocarcinoma patients but none in squamous carcinoma patients.The detection rates of ALK fusions and ROS1 fusions were 10％ and 13.3％,respectively.One patient was detected the coexistence of MET with ROS1 fusion.Conclusion The c-MET gene amplification is correlated with gender,but not with age,histological types and clinical stages.C-MET amplification,ALK fusions and ROS1 fusions are almost no coexistence,but not completely mutually exclusive.To they knowledge,this is the first case report the coexistence of MET amplification with ROS1 fusion in NSCLC.

[Objective]To study the intervention effect of mild hypotherrnia on cerebral edema in rats after cerebral infarction.[Methods]Twelve male SD rats were randomized into control group (n--6) and mild hypotherma treatment group (n=6).The rat models of focal cerebral infarction were established with obstructing the middle cerebral artery of rats by suture emboli.Rats of the mild hypotherma treatment group were treated by mild hypotherm for 3 h after the models were established.MRI was performed at 6 h,and 1,3,5 and 7 d after the success of model making.Cerebral infarction volume,absorption rate of edema,and signal intensity ratios (SIRs) ofTIWl,T2WI,FLAIR and DWI sequencas in the infarction zones,and relative variance ratio of SIR (△SIR) were measured and calculated.[Results] In the mild hypotherma treatment group,the cerebral infarction volumes 3,5 and 7 d after the models were established were significantly decreased as compared with those in the control group (P＜0.05);the absorption rate of edema (the 7th d against the 3th d of infarction) in the the mild hypotherma treatment group was significantly higher than that in the control group (P＜0.05).Three,5 and 7 d after infarction,the SIRs of TIWI were all obviously higher than those in the control group,and the SIRs of T2WI and FLAIR were all significantly lower than those in the control group (P＜0.05).The SIR of DWI in the mild hypotherma treatment group was obviously lower than that in the control group 7 d after infarction (P＜0.05).The △SIRs of T2Wl,FLAIR and DWI in the mild hypotherma treatment group were significantly higher than those in the control group (P＜0.05).[Conclusion] Mild hypothermia has a significant inhibition effect on cerebral edema in rats after cerebral infarction.

BACKGROUNDTo investigate the frequency of circulating HBV specific T helper cell and evaluate its association with serum levels of HBV DNA before and during lamivudine treatment in patients with chronic hepatitis B.

METHODSThe frequency of circulating HBV specific T helper cells in response to HBcAg in 25 chronic HBV-infected patients was determined by Elispot assay; serum HBV DNA was quantitated by real-time PCR.

RESULTSThe frequency of HBV specific T helper cell before antiviral treatment (47.30 +/- 25.50 SFCs /1 x 10(6) PBMC) was significantly higher than that at the third month of therapy (23.10 +/- 18.45 SFCs /1 x 10(6) PBMC, P < 0.05). All 8 patients observed dynamically had decreased frequency of HBV specific T helper cell at the third month of therapy; six patients with serum HBV DNA level reduced had higher frequency of HBV specific T helper cell before treatment than 2 patients without serum HBV DNA level decrease.

CONCLUSIONHBV specific T helper cell response at the time of hepatitis flare in chronic hepatitis B patients was significantly augmented compared to that at the time of catabasis.

OBJECTIVETo study the effect of gonadotroph-releasing hormone (GnRH) agonist (GnRH-a) and GnRH antagonist (GnRH-ant) against cyclophosphamide (CTX)-induced gonadotoxicity in female rats.

METHODSThirty-six female SD rats were divided randomly into 6 groups to receive treatment with normal saline (NS), CTX, GnRH-a+NS, GnRH-a+CTX, GnRH-ant+NS, GnRH-ant+CTX, respectively. The rats were sacrificed between the first and second week after termination of the medication to compare the weight of the ovaries, the number of the primordial follicles and the follicle growth. The expressions of bcl-2 and bax mRNA in the ovaries were examined using RT-PCR.

RESULTSThe number of the primordial follicles was significantly greater and that of the growing follicles significantly lower in GnRH-a+NS and GnRH-a+CTX groups than in the GnRH-ant+CTX and CTX groups (P<0.05). The rats in GnRH-a+NS and GnRH-a+CTX groups had the lowest ovarian weight among 6 the groups (P<0.05). The bcl-2 mRNA level in the GnRH-ant+NS group was significantly higher than that in the other groups (P<0.05). The Bax mRNA in the GnRH-a+NS and GnRH-a+CTX groups was significantly higher than that in the NS group (P<0.05), but close to that in the CTX group (P>0.05); bax mRNA expression in the GnRH-ant+NS group was significantly lower than that in the NS group (P<0.05), but in GnRH-ant+CTX group, its expression was close to that in the NS group (P>0.05).

CONCLUSIONSIn female rats exposed to CTX, the GnRH analogs provides ovarian protection against CTX-induced gonadotoxicity by regulating the expression of the Bax mRNA in the ovary. GnRH-a may decrease the sensitivity of the follicles to CTX-induced gonadotoxicity by promoting follicle apoptosis and inhibiting follicle proliferation, and GnRH-ant increases the sensitivity to the CTX through a reverse effect on the follicles.
