Chinese Medical Equipment Journal 2003;0(10):-
Objective To contrast the image qualities and the display percentage of vessel branches in cerebral angiography by two techniques, namely, Digital Subtraction CT Angiography (DSCTA) and Dual Energy Subtraction CT Angiography (DECTA). Methods 40 patients were divided averagely into Group A and Group B. All of them underwent head CTA exam, employing Somaton Definition Dual-Source CT of Siemens. The patients in Group A were under the mode of single tube and single energy by DSCTA, meanwhile the patients in Group B were under the mode of dual tube and dual energy by DECTA. Then the image quality of two groups and the display percentage of main vessels and branch were contrasted. Results 18 patients out of 20 in Group A succeeded in subtraction while 2 cases failed because of patients′ motion. The rate of success was 90%; 20 patients in Group B did subtraction successfully and the achievement ratio was 100%; there was no great difference on cerebral main vessels and branches which displayed in image between two groups. But the image quality of group A was superior to that of Group B. Conclusion As the development of dual-source CT′s hardware and software, DECTA Cerebral Angiography will be applied widely in clinic.

Objective To compare the efficacy of balloon dilation and YAG laser endouretertomy in treatment of secondary ureteric stricture . Methods 32 patients with secondary ureteral stricture were randomly divided into balloon dilatation group and holmium laser cut group, 16 cases in each, respectively. The two kinds of treatment efficacy was compared after two years postoperative follow-up. Results The clinical results of the two groups showed no statistical differences at the third, sixth and twelfth month follow-up (P > 0.05), and there were no statistical differences in the overall effective rates (P > 0.05). The clinical results of the two groups showed no statistical difference at the twenty-fourth month follow-up (P < 0.05), and the overall effective rates had statistical difference (P < 0.05). Conclusions Balloon dilation and YAG laser endouretertomy in treatment of secondary ureteral stricture have the same short-term clinical effects. But the middle-term efficacy of YAG laser endouretertomy is superior to that of balloon dilation.

Objective To compare the clinical value of 2 oral contrast agents for magnetic resonance cholangiopancreatography(MRCP)and to investigate the safety, potency ratio of negative gastrointestinal contrast agents and to improve MRCP image quality. Methods Before MRCP,30 patients took oral ferric ammonium citrate(FAC)solution as group A and another 30 patients took Gd-DTPA dilution as group B.Then all patients underwent MRCP with T2-haste-fs-thick-slab sequence.Image assessment was done before and after oral taking of negative gastrointestinal contrast agents. Results After oral taking of negative gastrointestinal contrast agents, the interference of liquid in stomach and duodenum was suppressed and even effectively eliminated on MRCP.MRCP image quality was improved.Though the display of bile duct and pancreatic duct was of less image artifact,it reached the quality for diagnosis. No significant difference of MRCP image quality was found between those took FAC solution or Gd-DTPA solution. Conclusion FAC and Gd-DTPA both are effective negative gastrointestinal contrast agents.Gd-DTPA solution is of better function and higher quality-price ratio.

Objective To investigate the effects of tagged fluid and substraction in CT virtual colonoscopy (CTVC) on the conspicuity of polyps and to confirm the optimal attenuation value and the optimal viewing window. Methods Polyps measured 3-10 mm were created in fresh porcine colon in vitro and submerged in saline and mixed with positive contrast medium for the CT values of 200 HU, 400 Hu, 600 HU, and 800 HU. Polyps were measured before and after substraction and compared with those in the control group. The effects of different viewing windows and different attenuation values on the measurement and conspicuity of polyps were analyzed. Results The optimal attenuation value of tagged fluid was 800 HU and the optimal viewing window was colon (-150 HU, 1 500 HU) and bone (500 HU, 2 500 HU). Conclusion The combination of tagged fluid and CT substraction can improve the conspicuity of small polyps covered by colonic fluid.

Objective To assess the clinical application value and advantages of DSCT Angiography on artery injury in Extremities. Methods Seven suspected patients with artery injury in extremities underwent DSCT Angiography, including three cases on artery injury of upper extremity and four cases on artery injury of lower extremity. All the seven patients were performed with DSCT Angiography after the operation. The data of thin slice cross section were reconstructed by MPR, MIP and VR, employing Somatom Definition, German. Contrast-concentration was 350 or 370mg i/ml, total amount was 80-100ml and injection rates was 4.0 ml/s. Results All the images of angiography displayed many clear blood vessels and smooth vessels verge, contrasting with surrounding issues well. They also displayed no ladder artifact or truncation artifact. The image quality was good enough to meet the requirements of clinical diagnosis. Meanwhile the coincidence between preoperative diagnosis and postoperative diagnosis was 100%. Conclusion To suspected patients with artery injury in extremities, DSCT Angiography may be the first choice. Three-dimensional reconstruction angiographys by means of different reconstruction techniques are very helpful in diagnosing and formulating treatment perscription.

OBJECTIVE:To observe the clinical efficacy and safety of different doses of atorvastatin in the treatment of chron-ic subdural hematoma(CSDH). METHODS:One hundred and tweaty-eighie CSDH patients selected from our hospital during Jun. 2013-May 2015 were divided into observation group(n=62)and control group(n=66)in accordance with random number table. Both groups were given conventional treatment of brain cell nutrition. Control group received Atorvastatin tablet 20 mg,po,qd;observation group received Atorvastatin tablet 40 mg,po,qd. Both groups were treated for 6 months. Clinical efficacy,CSS and ADL score, hematoma volume, the levels of serum inflammatory factors (hs-CRP,MMP-9,IL-6,TNF-α) were observed in 2 groups. ADR was recorded during treatment and recurrence rate was also recorded. RESULTS:2 patients withdrew from observa-tion group and 6 from control group. Finally,120 patients met the criteria were included, with 60 cases in each group. Total re-sponse rate of observation group(88.3%)was significantly better than that of control group(73.3%),with statistical significance (P<0.05). After 1,3,6 months of treatment,CSS score, hematoma volume, the serum levels of hs-CRP,MMP-9,IL-6 and TNF-α in 2 groups were significantly decreased, while ADL score was increased significantly;the improvement of above indexes in observation group was significantly better than in control group, with statistical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). The recurrence rate of control group(13.3%)was significantly higher than that of observation group(3.3%),with statistical significance(P<0.05). CONCLUSIONS:Daily dose of shows ator-vastatin 40 mg better therapeutic efficacy and lower recurrence rate in the treatment of CSDH with good safety.

Objective To study the diagnosis and treatment of portal vein complications after orthotopic liver transplantation. Methods The clinical data of 173 patients receiving orthotopic liver transplantation in our hospital from 2002 to 2005 were retrospectively analyzed. Results The incidence of portal vein complications was 3.5% (6 cases). The incidence of portal vein stenosis was 1.2% and that of portal vein thrombosis was 2. 3%. Three cases had previously been treated for portal hypertension and three cases had had a history of portal vein thrombosis before liver transplantation. All the complicated patients recovered and were discharged after successful treatment. There was no complication related mortality. Conclusions A history of previous treatment for portal hypertension, portal vein thrombosis is a risk factor predisposing the patients to portal vein complications after orthotopic liver transplantation. Color Doppler sonography is a sensitive and specific method for monitoring the portal vein complications following orthotopic liver transplantation. The angiography of portal vein is essential for diagnosis of the complications. Thrombolysis treatment is unsatisfactory for advanced stage portal vein thrombosis. Balloon dilation and stenting are both a safe and effective management modality for simple portal vein stenosis.
