Objective To explore the efficacy of thoracoscopic treatment of giant pulmonary bulla with complex conditions. Methods Video-assisted thoracoscopic surgery (VATS) was performed in 32 cases of giant pulmonary bulla with complex conditions in our department from September 2011 to November 2015. According to the dyspnea index of the American medical research council, there were 22 cases with distress index Ⅳ and 10 cases with distress index Ⅴ. Results All the patients underwent thoracoscopic surgery smoothly. The drainage tube was retained for 4-7 d in 17 cases, 8-14 d in 14 cases and 48 d in 1 case. The follow-up after operation was 12-18 months (mean, 16. 0 ± 3. 2 months). The dyspnea index was maintained at gradeⅠin 10 cases and gradeⅡin 22. All the patients were able to take care of themselves in daily life. Conclusion Giant pulmonary bulla with complex conditions can be treated under thoracoscopy with good surgical outcomes.

The main species and research status were introduced for the haemostatic dressing of the prehospital first aid in foreign countries and China. The advantages and disadvantages, problems and futural development were analyzed for kinds of haemostatic dressing. It's pointed out that the haemostatic dressing tended to be multi-functional and complicated in the future.Dressing and haemostatic system had to be designed based on traumatic conditions in case multi function could not be realized.

Objective To investigate the dietary pattern in college freshmen students and to analyze the influencing factors on their dietary patterns. Methods A questionnaire survey on situation of dietary pattern and influencing factors was conducted among 1319 freshmen students. Results Four major dietary patterns were noticed and they were: Ⅰ , high consumption in hamburger, fried food, nuts, biscuit, chocolate, cola, coffee, sugars, Ⅱ, high consumption in pork, mutton, beef, poultry meat, animal liver, Ⅲ, high consumption in fresh fruits, eggs, fish and shrimps, kelp laver and sea fish, milk and dairy products, beans and bean products, Ⅳ, high consumption in rice and grain, fresh fruits, fresh vegetables, pork. Risk factors on dietary pattern were presented as follows: ( 1 )boys: having the food pattern Ⅰ and Ⅱ showed a strong positive association with the place where they live (OR= 1.67, 95%CI: 0.87-3.19; OR= 1.51,95%CI: 0.79-2.88), eating place (OR=1.63,95%CI: 1.O3-2.59; OR= 1.83, 95%CI: 1.04-3.23), level of mother' s education (OR=2.52,95%CI: 1.07-5.95; OR=3.38, 95%CI: 1.50-7.63), family income (OR=2.24, 95%CI: 1.30-3.88;OR=3.06, 95% CI: 1.77-5.29) and the status of passive smoking (OR= 1.80, 95%CI: 0.70-4.59;OR=1.83, 95%CI: 0.75-4.45). Inverse correlations was found on the level of mother's education (OR=0.56,95%CI: 0.17-1.79). The food pattern Ⅳ showed a strong positive association with place of eating(OR= 1.83,95%CI: 1.04-3.23) but having an inverse correlation with the level of mother's education (OR=0.56, 95%CI: 0.17-1.79). (2)girls when compared with boys, the food pattern Ⅰ showed minor association with the places of living and eating; while the food pattern Ⅱ and Ⅲ had minor association with the status of passive smoking. Conclusion Socio-demographic factors and lifestyle had influenced on the dietary patterns among college freshmen students who should be guided to have a reasonable, balanced diet in the college.

Objective To design a new type of external fixator for bone fracture and verify its rationality and feasibility. Methods The frame and screw of stainless steel achieved fracture fixation in vitro with a threaded locking structure. At the same time the increasing thickness of body frame, the inclined nail holes and the raised bottoms were designed to greatly enhance the strength and overall stability of body frame.FEM (finite element method),measuring the relative displacement and stress distribution after axial load, was used to verify the rationality of the design. An animal experiment with sheep was used to verify the feasibility of fracture treatment. Results Simulated result of FEM indicated that the biggest relative displacement between the separated bones was 0.04 mm,which was much less than the minimum value 1 mm required for fracture healing. The maximum stresses applied on the frame of the fixator, fixator screw, and bone were 35,26,and 6 MPa, respectively, which was much less than the allowable stress. In the animal experiment, fracture site was fixed firmly after operation and was well cured 3 months later. Conclusion The design of this new device is feasible and it can be used as a new method of fracture treatment.

The advantages and disadvantages of different treatment methods of traditional tibial fracture were summarized. The research progress on the treatment of tibial fracture with internal fixation plates as external fixators was introduced from two aspects of biomechanics and clinical application. It was proved that internal fixation plates as external fixators could reduce the structural stability, but this technique was still controversial to meet the mechanical requirements of fracture healing on biomechanics.Internal fixation plates as external fixators,it had the advantages of minimal invasion, high healing rate, less complications and favorable activities. It's pointed out clinical trials on its feasibility and availability as well as simulating body mechanics environment should be carried out to provide basis for relevant biomechanics researches.

OBJECTIVETo study and compare the effect of neutrophil elastase inhibitors (GW311616A and sivelestat) on the proliferation and apoptosis of U937 cells.
METHODSInhibitory effects of GW311616A and sivelestat on the proliferation of U937 cells were assayed by MTT assay. The morphologic changes of U937 cells were detected by transmission electron microscope, and apoptosis was observed by AnnexinV-FITC/PI staining. The changes of cell cycle and apoptosis were detected by flow cytometry. The expression of NE in U937 cells was observed by indirect immunofluorescence, the variations of content and activity of NE in U937 cells were measured through ELISA assay and colorimetric method.
RESULTSMTT showed that both NE inhibitors could inhibit the proliferation of U937 cells in a dose dependent manner. The IC50 of GW311616A and sivelestat were 150 and 214 μmol/L respectively. The inhibition effect of GW311616A was significantly higher than of sivelestat (P<0.01). Typical apoptosis morphological changes of U937 cells was observed through electron microscope. AnnexinV-FITC/PI staining showed that U937 cells could be induced to undergo apoptosis by the two inhibitors, the apoptosis ratio of 150μmol/L GW311616A group (13.60%) was significantly higher than that of 150μmol/L sivelestat group (3.69%)(P<0.01). The result of flow cytometry indicated that the apoptosis ratio of 150 μmol/L GW311616A group was 14.61%, U937 cell cycle was mainly blocked in G2/M phase; meanwhile 150 μmol/L sivelestat group as 4.25% with cell cycle in S phase. The fluorescence intensity of GW311616A group obviously decreased than of sivelestat group. And the two inhibitors could reduce the content and activity of NE in U937 cells, but the effect of GW311616A was significantly higher than of sivelestat (P<0.01).
CONCLUSIONGW311616A and sivelestat could inhibit the proliferation and cause apoptosis of U937 cells. Furthermore, GW311616A was more effective and harmful to cells than sivelestat.
