1.External quality control of iodine deficiency disorders laboratory in Henan province from 1999 to 2011

Objective To analyze the examination results of external quality assessment(EQA) at all levels of iodine deficiency disorders(IDD) laboratories in Henan province and the network operation to further standardize and improve the laboratory,and to provide reliable laboratory quality assurance for surveillance and control of IDD.Methods The examination results of EQA at all levels of IDD laboratories in Henan province were statistically analyzed in accordance with the National Reference Laboratory (NRL) of IDD (1999-201 1).Results The survey results showed that the provincial level laboratory was all qualified in testing urinary iodine and salt iodine in the past 13 years.In prefectural level,the laboratory response rates were 100.0%,and through participation in EQA,laboratory capacity had been significantly increased and stabilized.From 1999 to 2001,the passing rate of check up of urinary iodine was 22.2% (4/18),72.2% (13/18),94.4% (17/18),respectively,and the rate was stable at 100.0%(18/18) from 2002 to 2011 except 94.4% (17/18) in 2003.Since 2000,the prefectural level laboratory began to take part in the salt iodine EQA,and the laboratory response rate was 100.0% (18/18) from 2000 to 2011.Except 88.9%(16/18) in 2003,the passing rate of check up of urinary iodine was 100.0%(18/18)from 2000 to 2011.In 2003 and 2004,6 to 7 county-level laboratories participated in the EQA of urinary iodine in Zhengzhou city,respectively,and all qualified.The number of county-level laboratories that participated in the salt iodine quality control network increased from 29 in 1999 to 148 in 2011.Response rate was 94.4%(68/72),96.7%(58/60) and 92.3%(144/156)in 2003,2006 and 2007,respectively,and the rate remained stable at 100.0% in the remaining 10 years.In 1999,the passing rate was 69.0% (20/29),then increased significantly,except 86.7% (26/30) in 2001 and 84.6%(132/156) in 2007,the rates were all above 90.0% in other years,especially in 2000 and 2009,the passing rates were both 100.0%.Conclusions The accuracy of test results of external quality controls and the normal operation of the network at all levels of laboratories is closely related to the IDD laboratory conditions and detection techniques.
2.Current status of iodine deficiency disorders in Henan Province in 2011

Objective To analyze the present situation of iodine deficiency disorders (IDD) in Henan Province,and to promote implementation of sustainable control strategies.Methods In 2011,a stratified proportion to population probability sampling (PPS) method was used to survey 1200 children aged 8 to 10 in 30 counties of the province.One primary school was selected in each chosen county.Goiter,intelligence quotient (IQ),urinary iodine and salt iodine level were studied.Meanwhile,12 families per capita salt intake was investigated.In each school,30 5th-grade students and 30 pregnant and lactating women in the school townships and adjacent neighboring townships were selected to carry out questionnaire survey on health education with unified papers.Results ①The goiter rate of children aged 8 to 10 by B ultrasound was 4.5% (54/1201) ; the IQ of 1080 children was 107.75 ± 16.81 ; median urinary iodine level of 358 children was 201.4 μg/L.②The median of salt iodine content was 28.6 mg/kg,the coverage rate of iodized salt was 98.8% (1186/1200),and qualified rate of iodized salt was 93.0% (1116/1200).③The residents average daily salt intake was 10.5 g.④Average score of the questionnaire survey of 1084 5th-grade students was 4.2 points.Average score of 961 housewives was 4.4 points.Conclusions Various technical indicators show that IDD is in a sustained elimination state in Henan Province.Strengthen health education,enhance public awareness of disease prevention is still the important work ahead.
3.Narcotrend index monitoring can predict the recovery of consciousness in patients undergoing abdominal surgery.

OBJECTIVETo investigate the value of Narcotrend (NT) index monitoring versus standard hemodynamic parameters in predicting the recovery of consciousness in patients undergoing abdominal surgery.
METHODSForty ASA I or II patients undergoing elective abdominal surgery were randomized into two groups to receive sevoflurane-sufentanil anesthesia monitored by NT index or solely by clinical parameters. Anesthesia was induced with the inhalation of 8% sevoflurane and sufentanil target-controlled infusion at 0.2-0.5 ng/ml. The values of NT stage (NTS), NT index (NTI), and hemodynamic parameters (MAP and HR) were recorded during the period of recovery. The prediction probability (Pk) of each parameter was calculated and compared.
RESULTSNTS and NTl were closely correlated to the changes of consciousness during the recovery from general anesthesia. The Pk values of NTS and NTI in predicting eye opening and orientation recovery were 0.95, 0.92, and 0.92, 0.89, respectively, obviously higher than the Pk values of MAP and HR (P<0.05).
CONCLUSIONNT monitoring can be used to effectively predict the recovery of consciousness in patients undergoing abdominal surgery and facilitates a significant reduction of the recovery time and sufentanil dosage during a sevoflurane-sufentanil anesthesia.

5. Exercise and Alzheimer's Disease: A Review of Research Based on Post ̄translational Modification of Tau Protein

Alzheimer' s disease (AD) is an age-related neurodegenerative disease which seriously damages the physical and mental health of the elderly and causes huge economic pressure to the society. However, the pathogenesis of AD is not completely elucidated. There is still no effective drug to cure AD in clinical practice. Tau protein is a soluble and non-aggregating microtubule-related protein, which can stabilize microtubule structure. The structure and function of Tau protein are abnormal in AD' s brain while under pathological conditions, and the abnormal Tau protein aggregates to insoluble neurofibrillary tangles which damages microtubules and leads to cognitive dysfunction. These changes of Tau protein are regulated by a variety of post-translational modifications, which directly change the properties and functions of proteins by attaching specific chemical moieties to Tau protein's C-terminus or N-terminus. It's confirmed that a variety of Tau post-translational modifications, like phosphorylation, glycosylation, acetylation and sumoylation is abnormal in AD's brain, which is closely related to Tau degradation and the accumulation of toxic substances. In this review, we summarized the latest progress supporting the role of exercise regulated Tau post-translational modification in the prevention and treatment of Alzheimer's disease. Firstly, exercise inhibits tau protein hyperphosphorylation by suppressing the activity of GSK-3β and MAPKs and possibly by up-regulating the activity of PP2A. Secondly, exercise increases tau protein O-GlcNAcylation by up-regulating the expression of GLUT1 and GLUT3, also possibly by regulating the balance of activity of OGT and OGA. Thirdly, exercise decreases tau protein acetylation possibly by inhibiting p300 and activating SIRT1; exercise regulates the acetylation of Tau KXGS possibly by inhibiting HDAC6. Lastly, exercise inhibits abnormal Tau sumoylation possibly by regulating the co-location sites of phosphorylation and sumoylation.
6.The prognostic significance of preoperative plasma level of osteopontin in combination with intercellular adhesion molecule-1 for patients with hepatocellular carcinoma.

OBJECTIVETo investigate the enhanced predictive activity of preoperative plasma osteopontin (OPN) level in combination with intercellular adhesion molecule-1 (ICAM-1) for recurrence and prognosis of patients after resection of hepatocellular carcinoma (HCC).
METHODSA total of 75 patients received liver resection for HCC from August 2001 to December 2001 in authors' institute were enrolled in this study. The preoperative plasma levels of OPN and ICAM-1 were detected by ELISA, and the association of them combination with the recurrence and prognosis of HCC patients was analyzed.
RESULTSOPN and ICAM-1 could be detected in all of the plasma samples of the tested patients. A significantly higher OPN level and ICAM-1 level were found in plasma of patients who were found to have HCC recurrence during the follow-up time compared with those without recurrence (210.40 vs. 154.86 ng/ml, P = 0.001; 1011.23 vs. 747.49 ng/ml, P = 0.027). A significant difference of OS and DFS were found in different subgroups with higher or lower level of OPN (625 vs. 808 days, P = 0.0006; 433 vs. 674 days, P = 0.0003); and a similar situation was found in patients of high- and low- ICAM-1 levels (651 vs. 794 days, P = 0.0269; 489 vs. 642 days, P = 0.0248). The 2-year recurrence rates of the patients with higher and lower plasma levels of both OPN and ICAM-1 were 87.50% and 28.00% (P < 0.001), respectively; and the 2-year OS rates were 37.50% and 88.00% (P = 0.001), and the 2-year DFS rates were 12.50%, and 76.00 (P = 0.001), respectively.
CONCLUSIONSThe evaluation of preoperative plasma level of OPN or ICAM-1 may be helpful to predict the recurrence and prognosis of HCC patients in advance. The assessment of OPN level in combination with ICAM-1 could stratify patients into groups with different potentials of HCC recurrence and different outcomes more accurately than OPN or ICAM-1 individually.

7.Melamine related urinary calculus and acute renal failure in infants.

OBJECTIVETo summarize clinical characteristics, diagnosis and treatment of infants with urinary calculus and acute renal failure developed after being fed with melamine tainted formula milk.
METHODSData of infant patients with urinary calculus and acute renal failure due to melamine tainted formula milk admitted to the Beijing Children's Hospital affiliated to the Capital Medical University and the Xuzhou Children's Hospital in 2008 were used to analyze the epidemiological characteristics, clinical manifestations, image features as well as effects of 4 types of therapies.
RESULTSAll the 34 infants with urinary calculus were complicated with acute renal failure, their blood urea nitrogen (BUN) was (24.1 +/- 8.2) mmol/L and creatinine (Cr) was (384.2 +/- 201.2) micromol/L. The chemical analysis on the urinary calculus sampled from 14 of the infants showed that the calculus contained melamine and acidum uricum. The time needed for the four types of therapies for returning Cr to normal was (3.5 +/- 1.9) d for cystoscopy group, (2.7 +/- 1.1) d for lithotomy group, (3.8 +/- 2.3) d for dialysis group, and (2.7 +/- 1.6) d for medical treatment group, which had no statistically significant difference (P = 0.508). Renal failure of all the 34 infants was relieved within 1 to 7 days, averaging (3.0 +/- 1.8) d.
CONCLUSIONMelamine tainted formula milk may cause urinary calculus and obstructive acute renal failure. It is suggested that firstly the patients with urinary calculus complicated with acute renal failure should be treated with dialysis or medication to correct electrolyte disturbances, in particular hyperkalemia, and then relieve the obstruction with available medical and surgical methods as soon as possible. It is observed that the short term prognosis is satisfactory.

8.Diagnosis and treatment of melamine-associated urinary calculus complicated with acute renal failure in infants and young children.

BACKGROUNDInfants in some areas of China developed urinary lithiasis after being fed with powdered milk that was tainted with melamine in 2008 and very small proportion of the infants developed acute renal failure caused by urinary tract calculus obstruction. The aim of this article was to summarize clinical characteristics, diagnosis and treatment of infants with urinary calculus and acute renal failure developed after being fed with melamine tainted formula milk.

9.Nonspecific Effect of Stress on Brain Gray Matter Volume in Drug-naive Female Patients with First Depressive Episode.

BACKGROUNDThis study aimed to observe the differences in brain gray matter volume in drug-naive female patients after the first episode of major depression with and without stressful life events (SLEs) before the onset of depression.
METHODSForty-three drug-naive female patients voluntarily participated in the present study after the first major depressive episode. The life event scale was used to evaluate the severity of the impact of SLEs during 6 months before the onset of the major depressive episode. High-field magnetic resonance imaging (MRI) scans were obtained, and the VBM and SPM8 software process were used to process and analyze the MRI.
RESULTSCompared to that in patients without SLEs, the volume of brain gray matter was lower in the bilateral temporal lobe, right occipital lobe, and right limbic lobe in the SLE group. However, the gray matter volume did not differ significantly between the two groups after the application of false discovery rate (FDR) correction.
CONCLUSIONSAlthough the results of the present study suggest the absence of significant differences in brain gray matter volume between female drug-naive patients after the first episode of major depression with and without SLEs after FDR correction, the study provides useful information for exploring the definitive role of stress in the onset of depression.

10.Establishment of a rat model of bone marrow mesenchymal stem cell transplantation for repairing full-thickness skin defect.

OBJECTIVETo establish a rat model of full-thickness skin defect to receive bone marrow mesenchymal stem cell transplantation for wound repair.
METHODSA full-thickness skin defect measuring 4 cmx4 cm in 36 F344 rats, which were divided into 3 groups with the wound covered with alloskin graft, acellular dermal matrix, or petrolatum gauze. In vitro cultured BMSCs in the 5th passage were transplanted into the skin defect, and the time of wound dressing dissociation and number of transplanted Brdu-positive cells in the wound were observed 14 days later.
RESULTSThe alloskin graft resulted in significantly longer time before dressing dissociation, with greater number of Brdu-positive cells in the wound than the other two wound dressings (P<0.001). The acellular dermal matrix showed better effect than petrolatum gauze in terms of the dressing dissociation time and the viable transplanted cell number in the wound.
CONCLUSIONAlloskin graft can be ideal for covering the wound surface to protect the transplanted BMSCs in rats.
