1.Clinical analysis of patients with acute myocardial infarction induced by left main artery occlusion

Objective To study clinical data of patients with acute myocardial infarction (AMI) induced by left main (LM) artery occlusion,and to find out the clinical characteristics.Method From Janurany 1995 to May 2006,there were 15 patients with AMI related with LM obstruction from 1793 AMI patients,in whom primary PCls were performed.All patients were male with a mean age of (60.6?10.4) years old (ranging from 43 to 85 years old).The clinical and interventional data were retrospectively analyzed.Results Cardiogenic shock occurred in 10 patients at admission.Primary PCls were performed with intraaortic balloon pump (IABP) in all patients.Nine patients died during short follow-up,and the mortality was 60%.Three-month follow-up was made in the all survival patients,and one patient died after 4 years.Analysis showed good collateral circulation flow from right coronary artery to left coronary artery before operation affected the short-term prognosis.Conclusions AMI related with left main artery obstruction was critically fatal with high occurrence of cardiogenic shock and high mortality.Prognosis differs at,cording to different degree of collateral circulation.Primary PCI with IABP provides a promising strategy to improve clinical outcomes.

Scm in volume(42/60),multiple site involvement(44/60),blood type"O"(31/41),in comparison with those of survival group,and the difference was statistically significant.C-erbB-2,p16,p53,P-gp,CD_(44) and CD_(25)expression were not significantly different in these two groups. Conclusion The clinical stage, lymph node metastasis,lymphatic tumor emboli and/or neural involvement,infiltration depth,histological dif- ferentiation,tumor volume,involvement extension are important prognostic factors in patients with gastric can- cer,while the significance of cancer-related gene expression in gastric carcinomas needs to be studied further.
3.Establishment and analysis of chronic periodontitis and atherosclerosis model in Wistar rat.

OBJECTIVETo investigate the possible correlation between atherosclerosis and chronic periodontitis by establishing an animal model of chronic periodontitis and atherosclerosis in Wistar rat.
METHODSSixty male Wistar rats were divided into four groups: A (control group), B (chronic periodontitis group), C (atherosclerosis group), D (chronic periodontitis accompany with atherosclerosis group). Every group was accepted the corresponding treatment. Animals were sacrificed after 12 weeks. The periodontal index, levels of serum total cholesterol (TC) and low-density lipoprotein (LDL), the concentration of TNF-alpha and matrix metalloproteinase (MMP-3) were examined. The severity of chronic periodontitis and atherosclerosis was quantified by histopathology. The date were statistically analyzed.
RESULTSThrough detection of periodontal tissue of experimental teeth, serum and histopathology, animal models were successful. Histopathologic observation revealed:obvious inflammation of periodontal tissue was observed in group B and D. Attachment loss level in group B [(137.86 +/- 28.39) microm] and D [(162.36 +/- 22.69) microm] was higher than that in group A [(4.26 +/- 1.07) microm] and C [(68.07 +/- 18.25) microm] (P < 0.05), and that in group C was higher than group A (P < 0.05). Atherosclerotic lesions of abdominal aorta were formed in group C and D. The level of TC, LDL in group C and D was higher than that in group A and B (P < 0.05), and that in group D was higher than group C (P < 0.05). Animals in group B and D showed higher level of TNF-alpha, MMP-3 in serum than that in group A and C (P < 0.05). There was no correlation between the level of MMP-3 and TC (P = 0.971) or LDL (P = 0.604).
CONCLUSIONSChronic periodontitis may be a risk factor and contribute to the pathogenesis of atherosclerosis. MMP-3 may be an independent risk factor of atherosclerosis exclude TC and LDL.

Objective ] With a view to promoting the development of medical science and biological medicine industry in Suzhou , a serum bank as part of Biobank project was initiated by city hall and led by Suzhou Health Bureau . [ Methods] The construction of the serum bank was concerned with 4 aspects: 1 ) house building , 2 ) equipment purchasing , 3 ) organization construction , and 4 ) the collecting, processing and preservation of blood sample and information . [Results] The work of house, equipment and organization met the demands of the project .We collected 60 980 blood samples , with 367 837 specimens produced and saved .The plan was completed in the predetermined time . [ Conclusion] We achieved the goal in the first term of the project which we promised to the government , and obtained some good experience from both the methods and working mode .However , we found some problems in op-eration which would be the foundation for further and future development .

Objective To explore the outcome of brain derived neurotrophic factor(BDNF) and parvalbumin (PV) positive neurons after hyperbaric oxygen intervention for the valproic acid(VPA) autism rats in hippocampal CA1 region.Methods The animal model of autism was established by the methods of Schneider and Przewlocki.Animal model groups of autism were obtained in male offspring of the Wistar rats that received intraperitoneal injection of 600 mg/kg VPA on the 12.5 day after pregnancy according to the eye opening time,behavior,weaning weight and the learning and memory abilities which were evaluated by the Y electricity maze test on the 28th day after birth.Forty-eight male VPA autism rats were randomly divided into high-pressure high-oxygen group,high-pressure air group and normal pressure high-oxygen group and normal pressure air group(n =12,each group).Normal control group was obtained in male offspring of Wistar rats that received intraperitoneal injection of equivalent physiological saline in the same period (n =12).Using the immunohistochemistry methods and image analysis to examine the number of BDNF and PV positive neurons in hippocampal CA1 region of different groups.Results The number of BDNF positive neurons in normal pressure air model group was more than that in the normal pressure air control group,with statistical difference between them(5.00 ± 1.60 vs 3.00 ± 1.04,t =3.633,P =0.001).The number of the PV positive neurons in the normal pressure air model group was more than that of the normal pressure air control group,with statistical difference between them (5.33 ± 0.99 vs 2.83 ± 1.29,t =5.369,P =0.000).The number of the PV positive neurons in the high-pressure high-oxygen group was less than that in the normal pressure air model group,with statistical difference between them (3.33 ±0.99 vs 5.33 0.99,t =4.975,P =0.000).The number of the PV positive neurons in the high-pressure highoxygen group was less than that in the high-pressure-air model group (3.33 ± 0.99 vs 4.67 ± 1.92,t =-2.138,P =0.044).Conclusions The pathogenesis of autism may be related to the expression level of BDNF and PV in the hippocampal CA1 region and this may serve as the mechanism of hyperbaric oxygen intervention of autism animal model for effective treatment.
6.Effect of UC-MSCs on inflammation and thrombosis of the rats with collagen type II induced arthritis.

OBJECTIVETo investigate the immunoregulation effects of umbilical cord mesenchymal stem cells (UC-MSCs) on the rats with collagen II induced arthritis (CIA).
METHODSThe rats were first immunized by intradermal injection of chicken collagen type II emulsified with complete Freund's adjuvant (CFA) to monitor their swelling of foot, hair color and action state. After injected UC-MSC by caudal vein, the rats were scored with the arthritis index (AI) once a week. Then, the concentration of interleukin (IL-6), tumor necrosis factor-α (TNF-α) in serum and D-dimer (D-D), antithrombin-III (AT-III), thrombomodulin (TM) in plasma were detected by ELISA.
RESULTSObvious swellings of the feet were found in the experiment group compared with normal one. ELISA analysis showed that the concentrations of IL-6, TNF-α, D-D and TM in plasma of the experiment group as of (200.48 ± 15.04) ng/L, (450.25 ± 45.39) ng/L, (274.26 ± 67.93) ng/L and (9.18 ± 0.84) µg/L, respectively were higher than of(167.62 ± 0.97) ng/L, (371.44 ± 21.26) ng/L, (193.95 ± 8.22) ng/L and (6.30 ± 0.32) µg/L respectively in normal group (P < 0.05), but the concentration of AT-III \[(89.57 ± 6.40) ng/L\] was lower than normal group \[(112.82 ± 1.74) ng/L\] (P < 0.05). The levels of cytokines through the UC-MSCs treatment were significantly different from the model group (P < 0.05). After 9 weeks, these cytokines in the UC-MSCs group were mostly the same as the normal group.
CONCLUSIONThe thrombophilia status of the CIA rats was caused by immune injury. The UC-MSCs reduced the production of inflammatory cytokines and regulated and repaired the balance of coagulation and anticoagulation system of the body to cure the immune-related thrombophilia.

7.Semen quality of the men from six districts in Chongqing.

OBJECTIVETo investigate the quality and spatial distribution features of semen and to evaluate the reproductive health of the males in the Chongqing section of the Three-Gorge Reservoir area.
METHODSWe collected semen samples by masturbation after 2 -7 days of abstinence from the men in Nan'an, Shapingba, Zhongxian, Wanzhou, Yunyang and Wushan of Chongqing, which are geographically and demographically representative of the Three-Gorge Reservoir area. We analyzed the semen quality of all the samples and evaluated the reproductive health of the men.
RESULTSThe mean value of the five semen parameters of the male subjects from the six districts was within the normal range, including semen volume, sperm concentration, total sperm count, rapid progressive motile sperm, and total motile sperm. Those from Shapingba, Yunyang and Zhongxian exhibited abnormal sperm motility. According to the WHO criteria, normal value of all the semen parameters was found in less than 50% of the semen samples from the six districts, in 47% of those from Yunyang, and only 16% of those from Wanzhou. Spatial distribution maps of the semen parameters revealed significant spatial differences in seminal quality among the six districts, the highest in Yunyang, and the lowest in Wanzhou and Wushan that are located in the middle and lower reaches of the Three-Gorge Reservoir area.
CONCLUSIONThe mean value of semen parameters was low in a large proportion of men in the Chongqing section of the Three-Gorge Reservoir area, with spatial differences along the Changjiang river.

8.Personalized biomechanical modeling of prostate deformation based on elastography for MRI three-dimensional transrectal ultrasound(TRUS)image registration

Objective To evaluate the clinical significance of personalized biomechanical modeling of prostate deformation based on ultrasound elastography for magnetic resonance imaging(MRI)-transrectal ultrasound(TRUS)image registration.Methods A total number of 5 patients and 1 commercial prostate phantom were imaged via transrectal ultrasound elastography,3D-TRUS and MRI from June 2016 to December 2016.A personalized biomechanical model via the patient-specific ultrasound elastography was made for the deformable registration of prostate MRI and 3D-TRUS images.The registration accuracy was evaluated by the target registration error(TRE)and also the t-test was conducted to validate the statistical significance of our results.Results All the 5 sets of patient data as well as the phantom data were successfully registered.The TRE value of the phantom data was 1.65 mm.The mean TRE value of 5 patients was 1.31 mm,compared with the 2.52 mm TRE value of the registration method without patient-specific biomechanical properties via elastography,was approximately 48% lower(P <0.05).Conclusions Personalized biomechanical modeling of prostate deformation based on ultrasound elastography for MRI-TRUS image registration possesses important clinical significance and is a promising way to provide more quality guidance and improve the accuracy of prostate biopsy.

OBJECTIVETo ascertain the causation of a pregnant woman with undefined pneumonia reported from the People's Hospital of Tongling city in Anhui province on November 2005.
METHODSEpidemiological and clinical information of the case was collected from the keypersons close to the case and referring to the medical record. A medical observation was carried out on the close contacts of the case and sick or dead poultry. Tracheal aspirates being collected were tested by both RT-PCR and real-time PCR to detect viral nucleic acids of A/H5N1, and were inoculated into special pathogen free (SPF) embryonated hens' eggs.
RESULTSThe pregnant woman was found to have been contacted with the sick/dead poultry directly on the 4th day before onset of illness. All the 122 close contacts were healthy after a 10-day medical observation. The major clinical features of the case were viral pneumonia with rapidly developed leukopenia and lymphopenia. The progress to acute respiratory distress syndrome and multiple organ dysfunction syndromes was found at clinical presentation. HA and NA gene of A/H5N1 virus were positive. The 8 gene fragments of A/Anhui/1/2005 (H5N1) isolated from the tracheal aspirates had not carried genes from a human virus through reassortment, and the receptor-binding site of the hemagglutinin was polybasic cleavage site.
CONCLUSIONThis was the first documented case of H5N1 infection in pregnant woman. The immunotolerant state of pregnancy might have predisposed to the fatal outcome of the patient.
