1.Case-control study on effects of vacuum drainage on perioperative blood loss after total hip arthroplasty for the treatment of femoral neck fractures.

OBJECTIVETo compare postoperative blood loss under different negative pressures of drainage after total hip arthroplasty for the treatment of femoral neck fractures.
METHODSFrom January 1st to December 30th 2013, 74 patients with femoral neck fractures treated with total hip arthroplasty were randomly divided into two groups: high negative pressure drainage group and low negative pressure drainage group. In high negative pressure drainage group, there were 34 cases including 10 males and 24 females, with a mean age of (75.94 ± 9.02) years old, and the patients were treated with 60 kPa negative pressure of drainage. In the low negative pressure drainage group, there were 40 cases including 13 males and 27 females, with an average age of (74.93 ± 8.90) years old, and the patients were treated with 30 kPa negative pressure of drainage. The amount of total drainage, total blood loss, and hemoglobin change were compared between these two groups.
RESULTSAll the patients got primary healing without infections. In high negative pressure drainage group,the change of hemoglobin was (41.74 ± 15.69) g/L, total blood loss was (1,217.73 ± 459.50) ml and the drainage volume was (312.94 ± 103.44) ml; while in low negative pressure drainage group,the results were (34.90 ± 12.90) g/L, (904.01 ± 381.58) ml and (129.25 ± 44.25) ml separately. All the results in high negative pressure drainage group were higher than those in the other group. Three days after operation, the change of hemoglobin was (46.00 ± 13.29) g/L and total blood loss was (1,304.72 ± 421.75) ml; while in low negative pressure drainage group, the changes of hemoglobin was (43.87 ± 11.39) g/L and total blood loss was (1,196.78 ± 344.20) ml; there were no statistically significant differences between two groups.
CONCLUSIONWhen placing drainage devices after total hip arthroplasty for the treatment of femoral neck fractures, the level of negative pressure should be chosen according to preoperative level of hemoglobin and HCT in patients. For old patients with femoral neck fracture, low negative pressure is more suitable.

2.Psychologic status and their influencing factors in congenital microtia patients and their families.

OBJECTIVETo study the psychologic status and their influencing factors in congenital microtia patients and their families.
METHODSTotally one hundred and two congenital microtia patients (79 men, 23 women, mean age 13.62 +/- 7.2 years) were enrolled. The patients and their families answered the questionnaire written by ourselves to identify the psychosocial problems.
RESULTS(1) 23.5% patients were found to have severe psychosocial problems, such as lack of self-confidence, close and fear and so on. (2) With the growth of age, psychosocial problems of the patients were rated high (P < 0.05). (3) For patients who found their deformations early, psychosocial problems also were rated low. (4) For patients who found their deformations by themselves, psychosocial problems also were rated low. (5) The education and psychosocial impact for parents all affected patients deeply.
CONCLUSIONSTo prevent psychosocial problems, we should operate for patients as early as possible. And correct guidance is very important for youngsters.

3.Effects of tongfu decoction on rats' rapid gastric emptying.

OBJECTIVETo observe the effects of Tongfu Decoction (TFD) on the gastric emptying of normal rats, thus exploring whether it could promote gastric emptying rapidly.
METHODSThirty Sprague-Dawley (SD) rats were randomly divided into 3 groups, i.e., the normal control group, the domperidone group, and the TFD group, 10 in each group. They were respectively administered with normal saline, the domperidone suspension, and TFD by gastrogavage. Thirty min later the gastric emptying of mice was detected by single photon emission computed tomography technology (SPECT) labeled with 99m Tc-DTPA, and the gastric half-emptying time and the gastric emptying rate were calculated.
RESULTSThe gastric half-emptying time was (19.0 +/-1.7) min in the normal control group, (12.9 +/- 3.4) min in the domperidone group, and (12.7 +/- 4.1) min in the TFD group. Compared with the normal control group, the gastric half-emptying time was significantly shortened in the domperidone group and the TFD group (P <0.05). The gastric emptying rate at 15 min was 41.1% +/- 5. 8% in the normal control group, 52.9% +/- 10.9% in the domperidone group, and 56.0% +/- 10.3% in the TFD group, while at 30 min it was 65.6% +/- 2.8%, 72.9% +/- 2.6%, and 72.4% +/- 4.9%, respectively. Compared with the normal control group, the gastric emptying rate at 15 min and 30 min both significantly increased in the domperidone group and the TFD group (P <0.05). There was no statistical difference in the gastric half-emptying time or the gastric emptying rate between the two groups (P >0.05).
CONCLUSIONTFD showed similar effects as domperidone in rapidly promoting gastric emptying, and could shorten the gastric half-emptying time in normal rats.

Objective To study knowledge of healthy volunteers in-volved in Phase ⅠClinical Studies to clinical researches and related fac-tors.Methods The public awareness of research for therapeutic ad-vancements through knowledge and empowerment (PARTAKE) question-naires, designed by Duke University , were administered to 100 partici-pants in phaseⅠclinical study in Peking university third hospital.Re-sults The following perceptions were reported: research benefited our society (96.0%) , human experiment was necessary for developing new therapeutic method (91.0%), confidentiality and voluntary participating were adequately approved ( 90.0%) , all the undesirable effects were caused by the research ( 18.0%) , and volunteers would be paid for any undesirable adverse reactions completely ( 81.0%).Conclusion Re-sults suggest that the healthy volunteers have a good command of clinical research generally.They are aware of protections to participants , but they do not have a full understanding of compensation for damages .
5.Detection of guanylate cyclase C mRNA and cytokeratin 20 mRNA in peripheral blood and analysis of prognosis in early to moderate colorectal cancer patients.

OBJECTIVETo investigate the associations of guanylate cyclase C (GC-C) mRNA and cytokeratin 20 (CK20) mRNA with metastasis and prognosis in early to moderate colorectal cancer patients.
METHODSGC-C mRNA and CK 20 mRNA in peripheral blood of 74 colorectal cancer patients without distant metastasis were detected by fluorescent quantitative PCR (FQ-PCR). Based on their clinicopathological and postoperative follow-up data, the relationship and clinical significance of these data with metastasis hazards and prognosis factors were analyzed.
RESULTSThe positive rate of GC-C mRNA in 74 colorectal cancer patients was 33.8% (25/74), and CK20 mRNA was 31.1% (23/74). The 1-, 2-, 3- year disease-free survival rates of patients were 94.6%, 82.4% and 78.4% respectively. There were significant differences in positive rates of GC-C mRNA and CK20 mRNA, tumor differentiation, mesentery lymph node metastasis, tumor embolus in vessel and postoperative chemotherapy associated with 3-year disease free survival rate by Kaplan-Meier analysis (all P<0.05). While mesentery lymph node metastasis and tumor embolus in vessel were independent risk factors of 3-year disease-free survival (P<0.05). CK20 mRNA and tumor embolus in vessel were independent risk factors of 3-year disease-free survival by analysis stratified with clinical stage (P<0.05).
CONCLUSIONSDetection of CK20 mRNA and GC-C mRNA in peripheral blood may be important for early detection of early metastasis of colorectal cancer.

6.Surveillance and pathogenic analysis on non-typhoidal Salmonella in Guangdong province, 2007

Objective To understand non-typhoid Salmonella in diarrhea patients from Guangdong province in order to timely discover the outbreaks caused by them as well as to grasp the serotypes, antibiotic resistance and pulse-field gel electrophoresis (PFGE) types of those strains isolated from this surveillance program. Methods Salmonella strains from diarrhea patients were detected and all the positive strains were tested by serum agglutination, antibiotic susceptibility and PFGE. Results 71 nontyphoid Salmonella strains were isolated from 1128 stoop samples, with a positive rate of 6.29 %. All the strains were divided into 29 serotypes, with Salmonella serotype enteritidis and typhimurium showing the most common serotypes. Most of the strains were sensitive to cephalosporins and quinolones. The antibiotic resistance rates of S. typhimurium were higher than S. enteritidis and S. stanley. Other than S. enteritidis, all the serotype strains did not have the same type of PFGE. 17 S. enteritidis strains digested by Xba Ⅰ were divided to 8 PFGE types while the PFGE 4 type appeared the most common one. 12 S. enteritidis strains were typed again by Sfi Ⅰ and Not Ⅰ , and there were still 3 groups of strains showing the same PFGE pattern. Conclusion Most of the infection caused by non-typhoidal Salmonella was sporadic in Guangdong province in 2007. Cephalosporins and quinolones seemed the best in curing the infection of non-typhoidal Salmonella at the clinics.
7.Surgery for jugular foramen schwannomas via a pure endoscopic transoral approach

Objective To report a new approach,endoscopic transoral approach for the resection of jugular foramen schwannoma.Methods Nine patients with jugular foramen schwannoma ( three males and six females,ranging in age from 15 to 61 years old ) were treated by direct surgery via a pure endoscopic transoral approach to the jugular foramen. Eight patients complained of hypoglossal nerve palsy with hemiatrophy of the tongue; six cases complained of vagus nerve palsy. Three cases complained of glossopharyngeal nerve palsy,one case complained of facial nerve palsy and hearing loss.Results The nerves in this area were preserved and radical intracapsular removal of the tumor was performed via endoscopic transoral approach in the nine cases.Tumor removal,as assessed by intraoperative endoscopic inspection,postoperative magnetic resonance imaging and clinical evaluation,revealed all tumors were completely removed.One patient suffered from temporary swallowing difficulties and temporary right vagus palsy Ⅰ day after surgery.There were no others intraoperative and postoperative complications.All patients were followed up for 4 -29 months,no recurrences were occured in all these patients and the muscle bulk,motor and the pre-postoperative swallowing fuction,the vagus palsy,the facial nerve palsy and hearing loss had improved in these patients.Conclusion The endoscopic transoral approach and intracapsular removal of the tumor provided for successful minimally invasive surgery in the jugular foramen schwannomas.

9.Efficacy of Lugua polypeptide injection on active rheumatoid arthritis.

OBJECTIVETo investigate the therapeutic effect of Lugua polypeptide on active rheumatoid arthritis (RA).
METHODSFifty patients with active RA were selected for the study and were randomly divided into study group and control group. Patients in study group were treated with Lugua polypeptide intravenously at a dose of 16 mg per day and those in control group were given Celecoxib 200 mg twice a day for successive 2 weeks. Two groups were given the same basic treatment. Tenderness and swelling of joints, morning stiffness, erythrocyte sedimentation rate, C-reactive protein,rheumatoid factor and so on were recorded before and after treatment.
RESULTSThe above index on joints in study group was significantly improved compared with that in control group and the level before treament. No apparent side effects were observed.
CONCLUSIONLugua polypeptide is effective and safe on active RA. It is a promising agent in the treatment of RA.

10.Investigation on reasons of unacceptable external quality assessment results for blood gas and acid-base analysis in China

Objective To investigate the reasons of unacceptable results and corrective measures adopted in external quality assessment (EQA)for blood gas and acid-base analysis.Methods The reasons of unacceptable results and corrective measures for three EQA testing events of blood gas and acid-base analysis in 2016 were reported through EQA system based on web which was developed by National Central for Clinical Laboratories.The responses were divided into seven major groups,including EQA samples,errors in reporting results,methodology,equipments,techniques,EQA evaluations and unexplainable results after survey.Results The disqualified rates of EQA survey on blood gas and acid-base analysis were ranged from 0.5% to 13.1% and reporting rates of disqualification causes were ranged from 45.8% to 69.0% (except for the groups less than 20 laboratories).In the reasons for unacceptable results technological defects (35.9% to 37.0%)were mainly associated with inappropriate specimen handling and/or storing,reagents and calibration problems.The defects of equipments (24.4% to 27.9%) included mainly the malfunction and failure to adhere to scheduled instrument maintenance procedures.The errors in reporting results (12.2% to 19.7%) were mostly transcription errors and reporting wrong codes.The unexplainable results after survey account for 8.7% to 9.6%.The methodological defects (8.1% to 11.8%) were largely attributed to inadequate training and quality control method.The defects of EQA evaluations (0.8% to 3.3%)were all due to inappropriate grouping.The categorizations of the problems in the three EQA testing events were similar.The most corrective measures were appropriate,in which re-education and training for staff and improvement in instruments,reagents,internal quality control,calibration and process of reporting results were included.Conclusion The analysis and classification for reasons of unacceptable EQA results should be helpful for laboratories in identifying opportunities for improvement and adopting corrective measures in time.
