1.Current status of iodine deficiency disorders in Henan Province in 2011

Objective To analyze the present situation of iodine deficiency disorders (IDD) in Henan Province,and to promote implementation of sustainable control strategies.Methods In 2011,a stratified proportion to population probability sampling (PPS) method was used to survey 1200 children aged 8 to 10 in 30 counties of the province.One primary school was selected in each chosen county.Goiter,intelligence quotient (IQ),urinary iodine and salt iodine level were studied.Meanwhile,12 families per capita salt intake was investigated.In each school,30 5th-grade students and 30 pregnant and lactating women in the school townships and adjacent neighboring townships were selected to carry out questionnaire survey on health education with unified papers.Results ①The goiter rate of children aged 8 to 10 by B ultrasound was 4.5% (54/1201) ; the IQ of 1080 children was 107.75 ± 16.81 ; median urinary iodine level of 358 children was 201.4 μg/L.②The median of salt iodine content was 28.6 mg/kg,the coverage rate of iodized salt was 98.8% (1186/1200),and qualified rate of iodized salt was 93.0% (1116/1200).③The residents average daily salt intake was 10.5 g.④Average score of the questionnaire survey of 1084 5th-grade students was 4.2 points.Average score of 961 housewives was 4.4 points.Conclusions Various technical indicators show that IDD is in a sustained elimination state in Henan Province.Strengthen health education,enhance public awareness of disease prevention is still the important work ahead.
2.Correlation Study Between Serum Level of LncRNA KCNQ1OT1 Overlapping Transcript and Coronary Artery Lesion in Relevant Patients

Objective:To investigate the serum expression of long non-coding RNA overlapping transcription content KCNQ1OT1 (KCNQ1) gene in patients with coronary artery disease (CAD) and its clinical significance.
Methods:A total of 196 patients treated in our hospital were divided into 2 groups:CAD group and Control group, the patients were without CAD. n=98 in each group. Expression levels of serum KCNQ1OT1 and P53 were measured by quantitative RT-PCR;the relationship between KCNQ1OT1, P53 and clinical features in relevant patients were analyzed.
Results:Compared with Control group, CAD group had increased expression of serum KCNQ1OT1 and decreased expression of P53, both P<0.05. Correlation analysis revealed that the expression of KCNQ1OT1 was negatively related to P53 (r=-0.856, P<0.001);multi Logistic regression analysis indicated that serum KCNQ1OT1 was independently related to CAD (P<0.05).
Conclusion:CAD patients had obviously increased serum level of KCNQ1OT1;KCNQ1OT1 was independently related to CAD occurrence.
3.Experimental study of cartilage defect repair using bone mesenchymal stem cells transfer of BMP-2 and TGF-β3 gene mediated by adenovirus vector combining with deminerized bone matrix in pig

Objective To explore the repair result of full-thickness cartilage defects in diannan small-ear pig by bone mesenchymal stem cells (BMSCs) transferred with both transforming growth factor-β3(TGF-β3) and bone morphogenetic protein-2(BMP-2) gene mediated by adenovirus vector and combined with deminerized bone matrix (DBM). Methods 32 full-thickness defects from 16 knees of 8 pigs were randomly divided into 4 groups in the experiments. In group A, the animals′ lateral femoral condyle of right knee joint was repaired with DBM and BMSC infected with both Ad-TGF-β3 and Ad-BMP-2. In group B, the medial femoral condyle of right knee joint was repaired with DBM and BMSC without infection. In group C, the lateral femoral condyle of left knee joint was repaired with DBM. And the group D is control group. Morphology and histology were observed 2, 4, 8 and 12 weeks after operation. Results 12 weeks after operation, the whole defects were repaired in group A, HE staining showed typical cartilaginous structure in the repaired area. In group D, defects were not repaired but filled with fibrous tissue. The O′driscoll scores were 15.65 ± 0.11 (group A), 11.33 ± 0.22 (group B), 6.13 ± 0.15 (group C) and 5.08 ± 0.15 (group D). There was significant difference among the groups (P < 0.05). Conclusions The new type of tissue engineering scaffold that DBM combined with BMSCs transfected with both Ad-BMP-2 and Ad-TGF-β3 could induce cartilage regeneration and repair the defects.
4.External quality control of iodine deficiency disorders laboratory in Henan province from 1999 to 2011

Objective To analyze the examination results of external quality assessment(EQA) at all levels of iodine deficiency disorders(IDD) laboratories in Henan province and the network operation to further standardize and improve the laboratory,and to provide reliable laboratory quality assurance for surveillance and control of IDD.Methods The examination results of EQA at all levels of IDD laboratories in Henan province were statistically analyzed in accordance with the National Reference Laboratory (NRL) of IDD (1999-201 1).Results The survey results showed that the provincial level laboratory was all qualified in testing urinary iodine and salt iodine in the past 13 years.In prefectural level,the laboratory response rates were 100.0%,and through participation in EQA,laboratory capacity had been significantly increased and stabilized.From 1999 to 2001,the passing rate of check up of urinary iodine was 22.2% (4/18),72.2% (13/18),94.4% (17/18),respectively,and the rate was stable at 100.0%(18/18) from 2002 to 2011 except 94.4% (17/18) in 2003.Since 2000,the prefectural level laboratory began to take part in the salt iodine EQA,and the laboratory response rate was 100.0% (18/18) from 2000 to 2011.Except 88.9%(16/18) in 2003,the passing rate of check up of urinary iodine was 100.0%(18/18)from 2000 to 2011.In 2003 and 2004,6 to 7 county-level laboratories participated in the EQA of urinary iodine in Zhengzhou city,respectively,and all qualified.The number of county-level laboratories that participated in the salt iodine quality control network increased from 29 in 1999 to 148 in 2011.Response rate was 94.4%(68/72),96.7%(58/60) and 92.3%(144/156)in 2003,2006 and 2007,respectively,and the rate remained stable at 100.0% in the remaining 10 years.In 1999,the passing rate was 69.0% (20/29),then increased significantly,except 86.7% (26/30) in 2001 and 84.6%(132/156) in 2007,the rates were all above 90.0% in other years,especially in 2000 and 2009,the passing rates were both 100.0%.Conclusions The accuracy of test results of external quality controls and the normal operation of the network at all levels of laboratories is closely related to the IDD laboratory conditions and detection techniques.

6.Causes of orchiectomy: An analysis of 291 cases.

OBJECTIVETo study the causes of orchiectomy in different age groups.
METHODSWe retrospectively reviewed the clinical data about 291 cases of orchiectomy performed between March 1993 and October 2014 and analyzed the causes of surgery and their distribution in different age groups.
RESULTSThe main causes of orchiectomy were testicular torsion (45.8%), cryptorchidism (32.5%) and testicular tumor (16.9%) in the patients aged 0-25 years, testicular tumor (42.4%), cryptorchidism (25.9%) and tuberculosis (10.6%) in those aged 26-50 years. Prostate cancer was the leading cause in those aged 51-75 years (77.6%) or older (84.0%)), and testicular tumor was another cause in the 51-75 years old men (10.2%). Prostate cancer, testicular tumor, cryptorchidism, and testicular torsion were the first four causes of orchiectomy between 1993 and 2009. From 2010 to 2014, however, testicular tumor rose to the top while prostate cancer dropped to the fourth place.
CONCLUSIONThe causes of orchiectomy vary in different age groups. The proportion of castration for prostate cancer patients significantly reduced in the past five years, which might be attributed to the improvement of comprehensive health care service.

7.Application of computer navigation system in the treatment of post-traumatic reconstruction.

OBJECTIVETo investigate the value of computer aided navigation system (CANS) in the treatment of post traumatic maxillofacial deformation.
METHODSFifty-four patients (M = 37, F = 17) were included in the study, including 31 cases of zygomatic fracture, 7 cases of pure orbital fracture, 11 cases of temporal mandibular joint ankylosis, 1 case of foreign body and 4 cases of defect reconstruction with custom implant. Data acquisition was done through CT scan, and DICOM data was transferred into workstation. Computer assisted design, including osteotomy, reposition, fibula flap design, orbital implant construction was performed using Surgicase CMF and Brain Lab Iplan system. The virtual design was transferred to Brain Lab navigation system, and the osteotomy, reduction, location of bone graft and custom implant were guided by navigation. Postoperative CT scan was required 48 - 72 hours after surgery. Preoperative and postoperative CT images were superimposed automatically in BrainLab Iplan system, and compared both in 3D objects and 2D slices.
RESULTSAll the cases achieved good results without serious complication. The error of important corresponding points in zygomatic fracture reduction, orbital reconstruction and defect reconstruction was 0.2 - 3.5 mm, 0.8 - 2.0 mm and 0.2 - 2.2 mm respectively.
CONCLUSIONSComputer assisted design is of considerable value for the systematic and accurate planning for complicated post traumatic deformation. Virtual plan could be carried out accurately with the assistance of CANS.

8.Microsurgery for intracranial aneurysm:analysis of 438 cases.

OBJECTIVETo summarize the experience of surgical therapy in a series of 438 patients with intracranial aneurysms.
METHODSA retrospective analysis was made on the clinical data of 438 patients, in terms of the perioperative management, timing of surgery, surgical skills, and The HUNT-HESS grade.
RESULTS438 patients with 476 aneurysms underwent microsurgery, in which 450 aneurysms were clipped, 14 were wrapped, 8 were isolated,4 were cut. 32 aneurysms were ruptured (6.72%) during the operation. The relation between Hunt-Hess scale and mortality is significant postpone operation (> or =7 d) has got a better curative effect than the early.
CONCLUSIONSMicrosurgical treatment is a confirmed effective method with intracranial aneurysms. The higher of HUNT-HESS grade will result to the worse clinical outcome.

9.A new algorithm for ultrasound image speckle suppression.

Ultrasound image has a lot of speckle noise, which brings great difficulties to the feature extraction, recognition and analysis. Especially in the edge extraction, the conventional extraction algorithms are difficult to achieve the desired results because of the speckle noise. To solve this problem, an algorithm based on the anisotropic diffusion equation is presented. The new algorithm combines the robust estimation and considers the characteristic of the speckle noise, so it can suppress the speckle noise effectively and be more robust, thus the edge details of the ultrasound image can be preserved even enhanced, which can provide effective safeguard for the following edge extraction. Furthermore, the paper proposes a new method to compute speckle scale coefficients automatically, which reduces the influence of the human beings, and enhances the stability of the algorithm.

10.Segmentation of breast tumor ultrasound images based on an improved C-V model.

This paper proposes an improved C-V model, which can avoid the step of re-initialization and simplify the formation of the initial level set function, thus the speed of segmentation can be accelerated greatly. Furthermore, based on the grayscale distribution characteristics of the breast tumor ultrasound images and on the hypothesis of piecewise constant in the C-V model, a semiautomatic segmentation flow has been presented, in which the rough contour is sketched first, and then a subimage would be obtained for the refined segmentation algorithm. This flow has improved not only the accuracy, but also the efficiency of the segmentation algorithm. The experiments show that the proposed algorithm could extract the contour of the breast tumor from the ultrasound images efficiently and accurately, which is fundamentally important for the following target feature extraction and analysis.
