Objective To analyze the occurrence rate,positive rate,and other related factors influencing interpectoral lymph nodes in breast cancer patients,and to explore the significance and indications of the surgical dissection of interpectoral lympy nodes.Methods Clinical and pathological data from 148 breast cancer patients were retrospectively analyzed.All patients were subjected to modified radical mastectomy,and interpectoral lympy nodes were pathologically examined.The occurrence rate and metastasis of interpectoral lympy nodes were recorded,and the relationship between the interpectoral lympy nodes positive rate and tumor size,axillary nodes,clinical stages,neo-adjuvant chemotherapy,hormone receptors,human epidermal growth factor receptor-2 (Her-2) expression,and molecular subtypes of breast cancer were determined.Results The occurrence rate,overall metastasis rate,and the positive rate of interpectoral lympy nodes in patients with axillary lymph node metastasis were 13.5％(20/148),4.7％(7/148),and 10.9％ (7/64),respectively.I nterpectoral lympy nodes metastasis was significantly correlated with axillary node metastasis and clinical stages (P＜ 0.05).However,interpectoral lympy nodes metastasis was not significantly related with hormone receptor and Her-2 expressions(P ＞ 0.05).Interpectoral lympy nodes metastasis rate was not affected by neo-adjuvant chemotherapy.Patients with interpectoral lympy nodes metastasis were characterized by larger tumors,more positive axillary lymph nodes,and later chnical stages.Conclusions Interpectoral lympy nodes metastasis usually occurs in patients with larger tumors,more positive axillary lymph nodes,later clinical stages,as well as those with locally advanced cancer that meets the standard of neo-adjuvant chemotherapy.These indications suggest that the surgical dissection and pathological examination of interpectoral lymph nodes should be routinely performed.

Prediabetes is an abnormal condition between normal glucose metabolism and diabetes mellitus. Impaired glucose tolerance (IGT) is an indicator of high-risk state of prediabetes. Positive interventions of IGT, including life style changes and pharmacological intervention, can effectively postpone and reduce the development of prediabetes into type 2 diabetes mellitus, suggesting that IGT is a key point of diabetes prevention. Currently, pharmacological intervention for prediabetes is still at early stage. In this review, we summarizes recent clinical and preclinical studies on pharmacological intervention for prediabetes, and studies in the development of animal models with IGT and the application of new techniques. We also discuss the prospects of drugs for diabetes prevention, especially with the traditional Chinese medicine.

It is of capital importance to establish a reliable experimental animal model of intracranial aneurysm which should be extremely similar to human intracranial aneurysm in histological and hemodynamic aspects.For recent years,the elastase-induced aneurysm model by means of endovascular balloon occlusion has been widely used abroad.This paper aims to review the preparation of animal mode,the advantages and disadvantages of such a model as well as the recent progress in its applications.

Objective To summarize the experience of laparoscopic cholecystectomhy(LC) in the treatment of acute cholecystitis in senile patients. Methods A retrospective review was made on 279 cases of LC in senile patients,involving 263 cases of acute calculous cholecystitis,8 cases of gallbladder adenoma,5 cases of polypoid gallbladder lesions,and 3 cases of acute cholecystitis with unknown causes.Out of the 279 cases,there were 25 cases of gallbladder hydrops accompanying stones impacted in the neck and 23 cases of atrophic cholecystitis. Results The success rate of LC was 87.5%(244/279),and LC was converted to open surgery in 35 cases.No serious complications or fatal cases happened. Conclusions For senile patients,acute cholecystitis is often complicated with other diseases,which increases peri-operative risks.To prevent and minimize the occurrence of complications,strict adherence to surgical indications,proper management of co-morbidities,and broadening indications of conversions to open surgery are all considered essential.

Objective To explore the pathogenesis of femoral head ischemic necrosis(FHIN),and to observe the clinical therapeutic effects of extracorporeal shock waves therapy(ESWT) for FHIN in soldiers.Methods With exclusion of the femoral head necrosis sAbsequent to femoral neck fracture,108 cases of FHIN in ARCO stage Ⅰ-Ⅲ period,induding 86 males and 22 females,with aged ranged from 17 to 25 years,were involved in the present study,and the treatment modality was chosen by patients voluntarily.In 50 patients,ESWT combined with drilling decompression procedure(ESWT group) was chosen,and 9-12kV(0.16-0.22mJ/mm2) of extracorporeal shock wave was applied on the second day after drilling decompression.X-ray and MRI of bilateral hip joints were taken and Harris hip score(HHS) was estimated at 6 and 12 months after treatment.58 patients chose the treatment of simple drilling decompression served as controls.The results of the two groups were analyzed by Student's paired t test.Results No aggravation of FHIN occurred in the two groups.The average ratio of necrosis zone in ESWT group was diminished by 22.9%,32.8% and 34.3%,respectively,in the stages of ARCO Ⅰ,Ⅱ and Ⅲ,and the improvement was better in ARCO Ⅱ and Ⅲ stages than that in control group(P

OBJECTIVE:To provide reference for improving the national essential medicine system and satisfy the needs of ra-tional use from the public METHODS:The current situation and existing problems in the national essential system were described by literature research and comparative analysis,and recommendations were presented. RESULTS:The implementation of essential medicine system has certain effects in China,but there still remains some problems,involving too much variety of essential medi-cines,high price and others,and limitation of the use of medicine and distribution in primary medical institutions. CONCLU-SIONS:Government should further simplify the essential medicine lists,reduce prices,strengthen policy support for primary medi-cal institutions,deepen the national essential medicine system and protect the public's rational drug use.

Objective To explore the morphological changes of human mesenchymal stem cells(hMSCs) after exposure to extracorporeal shock wave(ESW).Methods An optimal experimental energy dose of ESW was applied to hMSCs of passage one.The morphological changes were examined by using inverted phase contrast microscope(IPCM),transmission electron microscope and cytochemical staining.Results ESW of 5kV and 100 times could raise the viability and proliferation of the cells.In the hMSCs of passage 3-5,the multiplicative division was speeded up,and the cells in the nuclear fission phase were increased.In the hMSCs of passage 7-9,the volume of the cells increased,and the endoplasmic reticulum was expanded.The Golgi's apparatus was developed and the matrix of the cells was bunchy.In the hMSCs of passage 11,the multiplicative division was also detected.Conclusion The proliferation and differentiation of the hMSCs can be improved by the treatment of ESW.

Objective To explore the role of Survivin and Fas in the tumor cells′ invasion and metastasis and to evaluate whether they can be used as a reliable predictor for diagnosis and a new treatment pattern for therapy.Methods Thirty paraffin-embeded material of nephroblastoma were collected.The control group was composed of 15 specimens of normal kidney tissue acquired from paratumor region of the material.Immunohistochemistry methods was used to detect the stain of Survivin and Fas proteins in normal kidney and nephroblastoma specimens,compared them with clinical stage,pathology type and the outcome.Results Positive expression rate of Survivin in tumor specimens and significantly higher than that in control group(53.3% vs 0,P
