1.Observation on clinical efficacy of warm needling moxibustion plus Mulligan dynamic joint mobilization for cervical radiculopathy

Objective: To observe the clinical efficacy of warm needling moxibustion plus Mulligan dynamic joint mobilization for cervical radiculopathy. Methods: A total of 60 cases with cervical radiculopathy were randomly divided into group A, group B and group C by random digital table, 20 cases in each group. The patients in three groups were treated with similar warm needling moxibustion. Group A only received warm needling moxibustion; group B was treated with warm needling moxibustion plus Mulligan dynamic joint mobilization; group C was treated with warm needling moxibustion plus cervical traction. Before the treatment and after 12 sessions of treatment, the patients in the three groups were assessed for score of pain and range of motion (ROM). Results: By 12 sessions of the treatments, the scores of pain were obviously decreased in the three groups, with statistical significances (all P<0.05), and the pain scores of group B and group C were significantly different from the score of group A (both P<0.05). ROM of the neck was increased than before the treatment in the three groups, with statistical significances (all P<0.05), and the ROM of the neck of group B was significantly different from those of group A and group C (both P<0.05). Conclusion: Warm needling moxibustion plus Mulligan dynamic joint mobilization can effectively improve the neck ROM and relieve pain in patients with cervical radiculopathy.
2.Effects of different target plasma concentrations of propofol by TCI on end-tidal isoflurane concentration at BIS 50

Objective To investigate the effects of different target plasma concentrations of propofol given by TCI on end-tidal isoflurane concentration when the depth of anesthesia was maintained at BIS 50 and evaluate the reliability of electromyography (EMG) as an anesthesia depth monitor. Methods Sixty ASA Ⅰ-Ⅱ patients aged 40-65 yrs weighing 40-85 kg undergoing elective abdominal surgery under general anesthesia were randomly allocated to one of three groups with 20 patients in each group : group Ⅰ isoflurane; group Ⅱ isoflurane + TCI propofol (1 ?g?ml-1) and group Ⅲ isoflurane + TCI propofol (2 ?g?ml-1). The patients were premedicated with intramuscular phenobarbital 0.1 g and scopolamine 0.3 mg. Anesthesia was induced with midazolam 0.05 mg?kg , fentanyl 3 ?g?kg-1 and propofol 0.5-1.0 ?g?kg-1. Tracheal intubation was facilitated with vecuronium 0.12 mg?kg-1. After intubation anesthesia was maintained with isoflurane inhalation alone (group Ⅰ) or isoflurane combined with TCI propofol at a target plasma concentration of 1 ?g?ml-1 ( group Ⅱ) or 2 ?g?ml-1 ( group Ⅲ). MAP, HR, SpO2, PETCO2, BIS, EMG and end-tidal isoflurane concentration were continuously monitored during anesthesia. BIS was maintained at 45-55 after tracheal intubation during maintenance of anesthesia.Results The three groups were comparable with respect to age, sex, body weight, duration of anesthesia and the total amount of fentanyl used during anesthesia. There were no significant differences in MAP, HR and SpO2 among the 3 groups. When BIS was maintained at 50, the end-tidal isoflurane concentration was 0.76?0.03% (group Ⅰ), 0.43? 0.08% (group Ⅱ) and 0.21?0.07% (group Ⅲ) respectively. EMG was maintained at 26-29 during operation. During emergence from anesthesia EMG value increased with BIS value. When the patients opened their eyes at command EMG value was about 40. The correlation between BIS and EMG was poor. Conclusion At the samedepth of anesthesia (BIS = 50) the end-tidal isoflurane concentration was greatly reduced when combined with TCI propofol. EMG value decreases with increasing depth of anesthesia but as an anesthesia depth monitor it still needs improving.

6.Analysis of the influence factors on research capacity of nursing staff

Objective To identify main factors affecting the research capacity of nursing staff for efficient elevation of such a capacity. Methods 1000 persons were chosen in a random coded sampling for completion of questionnaires, including a general information questionnaire, research capacity self-assessment questionnaire, nursing professional value scale and work satisfaction scale for nursing staff. Results Main factors for the research capacity range from high to low the following: education, work satisfaction, training involvement, cognitive value, and nature of their employer. Beta values of such factors are found to be 0. 323, 0. 234, 0. 182,0. 064, and 0. 062 respectively. Conclusion Administrators of nursing staff need to focus on the development of the work values and satisfaction of nursing staff, on top of developing their research methodology and knowledge. This approach may encourage them to proactively involve in research activities.

Technetium-99m galactosyl-neoglycoalbumin (99mTc-NGA) is a new hepatocellular receptor imaging ligand Its preparation and preclinical research are reported in this paper. The ligand was synthesized by the chemical coupling and the product was nontoxic and apyrogenic with good stability both in vivo and in vitro. The static images and kinetic data obtained from the liver portal vein ligation group (n = 2), the hepatic necrosis group (n=2) and the control group (n=6) indicate: (1) Tc-NGA imaging can provide excellent hepatic images, and (2) hepatic kinetic data appear to provide a new means for diagnosis of liver diseases.

ObjectiveTo investigate the status of benefit finding in diabetic patients,and mediating effect of resilience between family support and benefit finding in diabetic patients.MethodsFive hundred and thirteen diabetic patients from a hospital of Harbin were recruited as investigative subjects from September 2015 to June 2016.The resilience scale,family adaptability,cohesion scale and benefit finding scale were used in the study.ResultsThe scores of benefit finding,resilience and family adaptability and cohesion were 55.67±12.87,70.65±10.39 and 104.25±15.38,separately.The total score of family adaptability and cohesion was positively related to the total score of benefit finding(r=0.389,P<0.05).The total score of resilience was positively related to the total score of benefit finding(r=0.155,P<0.05).The total score of family adaptability and cohesion was positively related to the total score of resilience(r=0.112,P<0.05).Benefit finding as the dependent variable,the results of hierarchical regression analysis indicated that family adaptability and cohesion (β=0.382,P<0.01),resilience(β=0.098,P<0.01)entered the regression equation.There was a partial mediating effect of resilience between family support and benefit finding in diabetic patients.The mediating effect was 0.011,accounted for 2.8% of the total effect.The resilience was the important protective factor for benefit finding.ConclusionFamily support has a direct influence on benefit finding and an indirect effect mediated by resilience on benefit finding.
9.Clinical distribution and antimicrobial resistance of pathogens causing healthcare-associated infection in a comprehensive hospital

Objective To understand the clinical distribution characteristics and antimicrobial resistance of pathogens causing healthcare-associated infection(HAI) in a comprehensive hospital.Methods Clinical data of patients with HAI in this hospital between May 2012 and May 2015 were collected,the distribution and antimicrobial resistance of pathogens isolated from patients were analyzed.Results A total of 6 563 cases of HAI occurred among 183 850 patients,incidence of HAI was 3.57%,445 patients were isolated at least two kinds of pathogens,375 (84.27%) patients were isolated two kinds of pathogens,132 of whom were infected with both gram negative bacilli.4 478 specimens were sent for pathogenic detection,2 503 (55.90%) of which were isolated pathogens;a total of 2 755 pathogens were isolated,including 1 713(62.18%) strains of gram-negative bacilli,732(26.57%) gram positive cocci,304(11.03%) yeast-like fungi,and 6(0.22%) anaerobic bacteria.524(19.02%)strains were mainly from patients in department of neurology.The main specimen was sputum (n =1 340,48.64%).The isolation rates of carbapenem-resistant Escherichia coli (CREC),Klebsiella pneumoniae (CRKP),Acinetobacter baumnannii (CRAB),and Pseudomonas aeruginosa (CRPA) were 0.39% (2/510),1.66% (3/181),59.14% (207/ 350),and 5.29 % (11/208) respectively;isolation rate of methicillin-resistant Staphylococcus aureus (MRSA) was 21.55%(25/116).Conclusion Multidrug-resistant organisms causing HAl are various,it is necessary to understand distribution characteristics and prevalence of pathogens,monitor multidrug-resistant organisms,and implement contact isolation measures,so as to prevent the outbreak of HAI.
10.Construction of expressing vector of pSUPER-shRNA/mrp1 and its expression in vitro

Objective To construct the expressing vector of shRNA/mrp1 and study its expression in vitro. Methods 64bp oligonucleotides of pSUPER and the targeted senquence of siRNA/mrp1 were synthesized and annealed to form duplex strand, then were cloned into pSUPER to construct pSUPER-shRNA/mrp1 vector. Competenced Ecoli was transfected by vector of pSUPER-shRNA/ mrp1 to screen the positive clones for sequencing and extracting plasmid. The plasmids extracted were used to transfected HepG2/mrp1 cells with a control groups by negative vectors. The expression of mrp1 mRNA and MRP1 was measured by real-time PCR and resistance of HepG2/mrp1 by flowcytometry. Results pSUPER-shRNA/mrp1 was established successfully and was sequenced to test its accuracy. Expression of mrp1 mRNA in HepG2/mrp1-si was lower than that in HepG2/mrp1 (1-fold vs 179.76-fold, P<0.001). Compared to HepG2/mrp1, the expression of MRP1 in HepG2/mrp1-si was lower (11.2% vs 97.6%, P<0. 05). The sensitivity of HepG2/mrp1-si to adiramycin was higher than that of HepG2/mrp1(45.0-fold vs 1.2-fold, P<0.01). Meanwhile, the accumulation of DNR in HepG2/mrp1-si increased significantly as compared with the control (78.58 % vs 38.44%,P<0.05).Conclusion Vector of pSUPER-shRNA/mrp1 can be constructed by the technique of enzymatic incision. The multidrug resistance of HepG2/mrp1 can be reversed by RNA interference.
