1.Clinical analysis of 210 esophageal carcinoma patients undergoing dual-incision surgery of left neck and chest

This investigation was to analyze surgical treatment data of 210 patients undergoing mid- upper-esophagectomy and esophagogastroplasty through esophagus bed during June 1996 and June 2006 to evaluate the efficacy of this alternative strategy.No death was reported.Fistula occurred in 5 patients (2.4%) and cured by suture and dressing change.Thirteen patients (6.2%) developed stricture and received endoscopic balloon ductal dilatation.Of them,9 patients completely recovered,2 improved,and 2 showed no response.These findings suggest that our surgical procedure is simple,fits for physiological function,and results in fewer pest-operative complications and lower mortality,thus could be an ideal treatment option for mid-upper esophageal carcinoma.

Oncomelania hupensis is the intermediate host of Schistosoma japonicum, and its distribution is consistent with that of schistosomiasis.Temperature is an essential environmental factor for the survival of snails.Therefore, it is important to study the impact of climate warming on snail-breeding and schistosomiasis transmission.This paper reviews the importance of climate warming to snail breeding and the spreading of the disease, and the application of relative research techniques.

4.Comparison of the curative effect of two treatment methods on patients with chronic nephritis

Objective To study the curative effect and adverse reactions in patients with chronic nephritis by two treatment methods.Methods 1 00 cases with chronic nephritis were randomly divided into alone western medicine group(control group)50 cases and traditional Chinese combination western medicine group(treatment group)50cases,The index of efficacy,urinary protein of 24h,serum albumin(Alb),serum creatinine(Scr),blood urea nitrogen (BUN)were observed.Results The total effective rate 78.0%in treatment group was significantly higher than 52.0%in control group(x2=3.841,P<0.05);Three sub-types showed no significant efficacy difference(x2=1.841,P>0.05);The contents of urine protein,BUN,Scr levels after treatment were lower than before treatment in two groups(t=2.231,2.245,2.311,all P<0.05).The content of Alb after treatment was higher than before treatment(t=2.241,P<0.05);The contents of urinary protein,BUN,Scr,Alb after treatment were significantly different in two groups(t=2.241,2.285,2.301,2.295,all P<0.05);The adverse reactions in 2 cases(2.4%)in treatment group were significantly lower than 8 patients(16.0%)in control group(x2=6.941,P<0.01).Conclusion Traditional Chinese and western medicine could delay progression and obtain better clinical results in patients with chronic nephritis.
5.THREE DIMENSIONAL QUANTITATIVE STRUCTURE-ACTIVITY RELATIONSHIP OF HETEROCYCLIC-COMPOUNDS WITH DI-TERT-BULTYLPHENYL INHIBITORS

AIM To quatitatively disclose the relationship between activity and structure of a new class of COX-II inhibitors containing dialkylphenyl-linked heterocyclic moieties. METHODS AND RESULTS Seventeen COX-II inhibitors from literature as a training set were investigated with the aim of developing a 3D-QSAR model using comparative molecular field analysis (CoMFA). To reveal the pharmacophoric pattern, several modes of superimposition were explored. The significant model shows a higher ability to explain and predict the activity of COX-II inhibitors, with the cross-validation RCV2=0.718, non cross-validation R2=0.992, F=260.624, and SEE (standard error of estimate)=0.072. Three compounds were selected as a predicting set, the low deviations of calculated values from the measured ones suggesting a powerful predictive ability of the model. CONCLUSION The 3D-QSAR explains the dependence of COX-2 inhibition upon the structures of the compounds. Some structure information for design of new COX-II inhibitors with higher activity has been given.
6.Antimicrobial Resistance of Enterobacter cloacae in our Hospital 2005-2007

OBJECTIVE To investigate antimicrobial resistance of Enterobacter cloacae in our hospital,for guiding the treatment of these infections in clinical practice.METHODS Bacterial susceptibility testing was done by Kirby-Bauer method and bacteria were identified by VITEK-32.Retrospective analysis of the drug resistance was done to the E.cloacae isolated from our hospital in the recent 3 years.RESULTS A total of 364 E.cloacae strains were isolated,which mainly isolated from sputum.They were resistant to 13 types of antibacterial agents but the resistance rate to imipenem was 3.30%.The resistant rate to third and fourth generation cepholosporins(excepting cefoperazone/sulbactam) and quinolones was more than 30% and to ampicillin and cefazolin was more than 94.00%.CONCLUSIONS E.cloacae is severely resistant to cepholosporins and quinolones.More attention should be paid to the surveillance of such strains.Imipenem may be considered for use in critical patients.
7.Short-term effect of thymosin al combined with MVP chemotherapy on quality of life in patients with non-small cell lung

Objective To explore the effect of thymosin and MVP chemotherapy on the life quality of non-small cell lung cancer(NSCLC) patients. Methods 50 cases of NSCLC patients who received MVP(MMC, VDS, DDP) chemotherapy,were randomly divided into experimental group (using thymosin α1) and control group;before chemotherapy and at the end of chemotherapy (the first 2 weeks), the quality of life was evaluated and analyzed comparatively by the FACT-L questionnaire of patients with lung cancer-Chinese version (V4.0). Results The re-sults of the experimental group score increased by (3.13±2.29),and control group score increased by(-1.07± 2.19) with significant differences (P<0.01). Conclusions Thymosin α1 can improve the short-term quality of life of the non-small cell lung cancer patients who received MVP chemotherapy.
8.Effect evaluation of preventing ventilator associated pneumoia of patients with the oral trachea can-nula by air impact method to remove airbags retentate

Objective To evaluate effect of preventing ventilator associated pneumoia (VAP) of patients with oral cavity endotracheal intubation by air impulse clearing away secretion drainage. Methods 348 patients with oral trachea cannula and mechanical ventilation (MV) in intensive care unit (ICU) were divided into control group (n=174) and observational group (n=174). The control group adopted conventional airway management methods without the subglottic secretion drainage while the observational group adopted conventional airway management methods on the basis of combining air impact method remove stranded on airbags. Other treatment and care for all of patients were basically the same. The MV time, duration in ICU and incident rate on VAP were compared between the two groups. Results The average of MV time in control group was (5.25±1.18) days, (5.62±3.20) days in ICU and the rate of VAP was 10.34%(18/174). In observational, it was (8.96 ±5.43) days, (10.43 ±4.96 ) days and 34.48%(60/174), respectively. The MV time, duration in ICU and incident rate on VAP were significantly different (P<0.05), with statistical significance. Conclusions The air impact method to remove airbags retentate could shorten time of MV and during time in ICU, and could reduce the rate of VAP.
9.Negative Impact of Adverse Events to the National Immunization Programs and the Appropriate Responses

Objective It is well known that vaccines and national immunization program have helped to protect peoples,especially the children from infectious diseases such as smallpox,polio,measles and pertussis across the globe.However,adverse events may occur following immunization,which may lead to public concerns about the vaccine safety.Such concerns may threaten the success of an immunization program.Prompt and appropriate responses to adverse events can help to avoid the negative impacts to the National Immunization Program of China.
10.Thoracoscopic sympathetic trunk clipping for craniofacial hyperhidrosis: Analysis of 10 cases

Objective To investigate the feasibility of video-assisted thoracoscopic sympathetic trunk clipping in the treatment of craniofacial hyperhidrosis. Methods A total of 10 patients were operated on under general anesthesia with double-lumen endotracheal intubation. The patients were placed in lateral recumbent position with one-lung ventilation. A 7 mm trocar and a 4.5 mm trocar were inserted at the 2~3 intercostal space on the midaxillary line and at the 4~5 intercostal space on the posterior axillary line, respectively, to introduce surgical instruments and thoracoscopic camera. Alongside the sympathetic chain, the sympathetic nerve trunk immediately below the second costal margin was blocked with small-sized titanium clips. Then the lung was inflated and the incision sutured. Afterwards, the procedure in the contralateral hemithorax was performed using the same method. Results The operating time was 55~130 min (mean, 110 min). Symptoms of craniofacial hyperhidrosis disappeared in the 10 patients, all of who were satisfied with curative results. The postoperative hospital stay was 2~3 days. Neither Horner’s syndrome nor other serious complications were observed. Seven of the patients developed slight compensation hyperhidrosis in their chest, abdomen, back or legs. All the patients had normally returned to work and physical exercises in 7~10 days. Postoperative follow-up for 1~9 months (mean,6.3 months) in all the patients found no recurrence. Conclusions Video-assisted thoracoscopic block of sympathetic trunk below the second costal margin for craniofacial hyperhidrosis is safe and effective.
