1.A Case of Marfan Syndrome with Left Atrial Myxoma Confirmed by Echocardiography.

The Marfan syndrome, a rare disease causing a marked decrease in life expectancy by involving the skeletal, ocular, and cardiovascular systems, is known as a connective tissue disorder that is inherited autosomal dominant. The cardiovascular complications directly related to the cause of death are associated with more than 90% of the Marfan syndrome. Aortic aneurysm with rupture could occur because of dilatation of aorta due to defect of media. Aortic and mitral insufficiency, mitral valve prolapse, bacterial endocarditis, arrhythmia, and aneurysm of interatrial septum are also frequently observed. A few literatures were reported domestically pertaining to the Marfan syndrome thus far, and there are increased discoveries of cardiovascular complications of the syndrome with the application of echocardiogram. However, the Marfan syndrome with LA myxoma has never been reported both domestically and internationally(INDEX MEDICUS, 1966-1989). Therefore we hereby report a case of the Marfan syndrome with LA myxoma from the observation of a patient who was admitted to Eulji General Hospital at Taejeon i August of 1989.

6.Analysis of the Characteristics Appeared in the Cancer Information Provided as a Form of Question and Answer on the Internet.

This study was carried out to investigate the characteristics of contents and utilization for cancer information that is serviced on the internet in the form of Q&A and FAQ. A total of 947 cases (340 in Q&A and 607 in FAQ) were collected from August 20th to November 20th, 2001. Data were analyzed with descriptive statistical methods regarding the type of service, contents, and cancer included in the contents. The prevalent pattern of cancer information service was FAQ, and the distribution of contents and kind of cancer that were serviced differed depending on their service pattern and resources. The contents for prevention and screening, general understanding of cancer, and rehabilitation appeared more often in FAQ than in Q&A and were also more frequent in cancer specialized websites than any others. This pattern may be because FAQ and cancer specialized websites have a view of contents provider. Clinical symptoms and diagnosis were the contents provided more frequently in Q&A than in FAQ. Additionally, information on breast cancer was the most frequently asked question in regard to major cancer in Korea. However, information on hematopoietic system was the most frequently asked in Q&A. From the results of this study, it is clear that cancer-related information varies according to the cancer types, service patterns, and website characteristics. However, the fundamental difference depends on whether the contents are created in terms of the information user or not. This study demonstrated that there is a difference between users and developers in the requested information regarding available cancer-related. It is therefore necessary to establish a basic plan for appropriate cancer information services according to the needs of the users.

7.Usefulness of MR Urography in the Diagnosis of Hydroureteronephrosis.

PURPOSE: To evaluate the clinical usefulness of the MR urography (MRU) in the diagnosis of hydroureteronephrosis. MATERIALS AND METHODS: Twenty-five patients with hydronephrosis (26 cases) underwent MR urography, using a 1.5T MR scanner (Signa Horizon, GE Medical Systems, Milwaukee, U.S.A). The causes of hydronephrosis included benign ureteral stricture (12), ureteral stones (8) and malignant strictures (6), and were confirmed by operation, biopsy, conventional urography or clinical follow up. For MRU, a fat suppressed, respiratory-triggered, heavily T2-weighted fast spin echo (FSE) pulse sequence was used, and the 3-dimensional MR images were subsequently processed by a maximum-intensity-projection algorithm. We evaluated the success rate of MRU and its accuracy in determining the level and cause of obstruction, and compared the degree of hydroureteronephrosis seen on MRU with that seen on conventional urography. RESULTS: In all cases, a urogram was successfully obtained. In determining the degree of hydroureteronephrosis and the level of obstruction (in the upper ureter in 11 cases, in the mid-ureter in four, and in the lower ureter in 11), MRU findings matched those of conventional urography. In 24/26 cases (92%), MRU accurately determined the cause of urinary tract obstruction ; the two exceptions were in cases involving ureteral stones. CONCLUSION: MRU is a promising, noninvasive diagnostic method for evaluating hydroureteronephrosis, especially in children, pregnant women and patients with an allergic reaction to iodinated contrast material.

8.Intrathecal Catheter and Subcutaneous Access Port Implantation in Pain Management for Terminal Cancer Patient: A case report.

It is important to treat cancer-related pain in cancer patients to ensure the life quality of the patient, as well as to improve their life span. It has been estimated that at least 5% of cancer patients have pain refractory to medical treatment. Therefore, the need for epidural or intrathecal analgesia with opioids and local anesthetics is indicated if systemic treatment has failed. Intrathecal catheter placement and implantation of the injection port for administration of opioids and local anesthetics may improve pain relief in patients who are unresponsive to epidural routes. Although intrathecal implantation has several complications, similar infection rates have been reported between intrathecal and epidural administration. In addition, intrathecal administration showed better outcomes, including improved pain control, lowered daily doses, and an improvement in the level of drowsiness experienced when compared to epidural administration. We report here a case in which a terminal cancer patient was treated using an intrathecal catheter and subcutaneous port. The patient had cancer-related pain that could not be controlled by epidural opioid administration. Based on the results presented here, we suggest that intrathecal implantation is a feasible long term pain management method for intractable cancer pain patients.

10.A Survey on the Development of Database and Network for the Experts in Cancer.

OBJECTIVE: This research is a basic investigation of collecting experts' opinions for constructing the national cancer expert network. The objective of this research is to evaluate present situation and to use it for the planning of future korean cancer network in the cancer-related area. METHODS: We analyzed the need of the network and degree of information demand for constructing the network from Clinicians and Basic researchers(n=376) who answered themselves 'cancer expert'. The questionnaire consists of 9 questions and it was analyzed by using frequence, t-test and Chi-square. RESULTS: 53.7 percents of respondents answered they are already recognized cancer expert network and 93.5 percents of respondents answered that they need the network. Among the services that would be served by the network, the demand of cancer statistics, development and evaluation of cancer policy, and cancer clinical trials were listed in the high priorities. And recognized societies included in the high priorities were Korean Cancer Study Group, Korean Cancer Association, Korean Association for the Study of Lung Cancer. CONCLUSION: We are going to develop a database and network for the experts to generate, evaluate and verify cancer-related information based on evidence-based cancer information infrastructure. Hence, this research will be contributed to provide fundamental data for activating cancer-related research through joint studies with experts.
