1.Causes of the acute exacerbation of COPD treated at Department of Respiratory of Bach Mai Hospital in the first six months of the year 2005

Background: Chronic obstructive pulmonary disease (COPD) remains a major public health problem. Objective: To find the causes and the treatment results for acute exacerbation of COPD at Department of Respiratory of Bach Mai Hospital. Subjects and method: A study conducted in 87 patients treated by an acute exacerbation of COPD at Department of Respiratory of Bach Mai Hospital from March to August 2005. Results and conclusions: 88.5% of patients were men. Severe patients were most common in age over 55 years. The risk factor was smoking (over 66.7%). 47.8% of patients were in III stage and 37.7% were in IV stage. The cause of acute exacerbation of COPD was bronchial surinfection (83.9%); and 37.9% of them refused an antibiotic or bronchodilatators at home when they had severe symptoms of exacerbation. Most of them did not have enough condition in their homes for treatment (oxygen, aerosol machine). After 20 days (16.69 \xb1 8.7 days) treated by antibiotics, bronchodilatators, corticoid and oxygen, 90.8% of our patients were at good health. 2 of them were died by severe disease.

9.Meta-analysis of assisted music therapy for chronic schizophrenia.

OBJECTIVETo evaluate the effect of assisted music therapy for chronic schizophrenia.
METHODS11 articles including 603 chronic schizophrenia patients were meta-analyzed using fixed effect model or random effect model.
RESULTS6 randomized controlled trials were synthesized, showing that the difference was significant in statistics between experimental group (patients with music and drugs therapy) and control group (patients with drugs therapy only). Both the scores of SANS and BPRS for the control group were higher than those for the experimental group (SANS, d = 0.68; 95%CI: 0.46-0.90 and BPRS, d = 0.44; 95%CI: 0.06-0.82). In addition, the scores of both SANS and BPRS for the pre-test were higher than those for the post-test (SANS, d = 1.17; 95%CI: 0.02-2.32 and BPRS, d = 2.05; 95%CI: 0.28-3.82).
CONCLUSIONThe short-term effect of assisted music therapy is positive for chronic schizophrenia, but the long-term effect is still to be further studied.

10.Clinical comprehensive treatment of refractory chronic rhinosinusitis.

OBJECTIVE:
To study the methods and efficacy of comprehensive treatment on retractory chronic rhinosinusitis(CRS).
METHOD:
Thirty-two cases of patients with refractory rhinosinusitis took part in individualized comprehensive treatment program, that is, a healthy diet and moderate exercise, psychological counseling, and reasonable operation and gauze changing, and rational use of drug therapy.
RESULT:
All patients were followed up for 6 months or more, 26 cases (81.3%) were completely controlled of the disease, 6 cases (18.7%) were partially controlled; the total effective rate was 100%.
CONCLUSION
In the current situation that CRS etiology and pathogenesis studies have not yet made a breakthrough, and we have not developed a new and effective drug, comprehensive treatment focusing on internal and external factors of refractory rhinosinusitis at the same time, that is, to take a rational healthy diet, moderate exercise, psychological counseling, and reasonable operation and gauze changing, and rational use of drug therapy ect. can obtain better effect.
