2.Clinical Implications of Echocardiographic Findings in Bacterial Endocarditis.

42 patients with the clinical evidences of endocarditis were examined by echocardiography. In 28 of 42 cases(66.7%) vegetation was present, whereas in 14(33.3%) vegetation was not visualized. Alpha-hemolytic streptococcus was the most common infecting organism(47.6%) and rheumatic heart disease was the most common predisposing heart disease(47.6%). Patients with echocardiographically demonstrable vegetation had a higher incidence of congestive heart failure compared to the patients without vegetation(75.0% vs. 21.4% p<0.05). But major embolism was not significantly different in the two groups. Mortality was higher in the patients with vegetation than in the patients without it(39.3% vs. 0%, p<0.05). Among vegetation positive patients, mortality was highest in aortic valve patients.(58.3%) So patietns with aortic valve vegetation should be regarded as high risk group and early surgical intervention should be considered if indicated. The causes of mortality were congestive heart failure(45.5%), cerebral embolism(36.4%), myocardial infarction(9.1%) and ventricular tachycardia(9.1%) in decreasing frequency.

3.Does Teriparatide Improve Fracture Union?:A Systematic Review

We conducted an updated review of the evidence of teriparatide (TPTD) for fracture healing for the following questions. (1) Does it decrease fracture healing time?; (2) Can it be an alternative treatment for nonunion?; (3) Does it aid the union of atypical femoral fracture (AFF)? We searched PubMed, EMBASE, and Cochrane Library including “Fracture” AND “nonunion” AND “Teriparatide”. In total, 57 publications met our inclusion criteria were summarized. This systemic review of the available literature revealed that TPTD works positively with regard to enhancing fracture healing time and union of AFF. There are also many case studies on the use of TPTD could be a potential new safe treatment for nonunion with no side effects. However, level 1 studies on the evidence of TPTD are still lacking so far. Over the last decade, a growing body of evidence has accumulated suggesting that TPTD can be an adjunct to enhance fracture healing or a therapeutic option to treat nonunion, but greater evidences from large volume prospective trials are needed.
4.Video Assisted Minilaparotomy Surgery (VAMS) Live Donor Nephrectomy.

PURPOSE: We have devised a new surgical method of video-assisted minilaparotomy surgery-live donor nephrectomy (VAMS-LDN), which is a hybridized form of laparoscopic and open surgery that combines the advantages of both. We present our series of 202 consecutive patients. METHODS: Since 1993 we have performed 202 successful VAMS-LND. All 202 healthy kidney donors' characteristics and their postoperative courses were retrospectively reviewed and all data were compared to 95 open donor nephrectomies performed during the same period. RESULTS: The mean age and weight of the patients were 37.1+/-9.5 years and 61.6+/-3.3 kg, respectively. The mean operating time was 139+/-39 minutes which was similar to open donor nephrectomy but shorter than laparoscopic donor nephrectomy. There were no major intraoperative complication except two tears to lumbar veins which required transfusion. The mean warm ischemic time was 2.2+/-0.7 minutes which was equal to open donor nephrectomy. Patients experienced less postoperative pain and recovered quicker compared to open donor nephrectomy. CONCLUSION: VAMS-LDN is a safe and minimally invasive technique for live donor nephrectomy, incorporating advantages of both conventional open and laparoscopic methods. VAMS-LDN is a viable option for living donor kidney transplantation.

5.A Study on the Source of Cholera Epidemic of Korea 1995-1996.

E1 tor cholera epidemic have been reported eight times in Korea since 1963. Researchers suspected unattempted importation of cholera bacteria from foreign countries or artificial germination by some people. No researchers have made hypotheses of environmental reservoir as the sources of cholera epidemics until in 1991 in Korea. This study focuses on the aquatic reservoir as the potential source of cholera epidemic. A total of 68 cholera patients were noted in 1995 and only two in 1996. Authors argue for aquatic reservoir as the source of 1995, 1996 cholera epidemic and designate sea fishes as the vehicle of cholera infection The national Institute of Health team cultured Vibrio cholera O1, from the samples of sea water from Kangwha county in September 1995. The sporadic Occurrence of cholera patients around the country who have not traveled out of town support aquatic environmental reservoir as the source of 1995, 1996 cholera epidemic in Korea.

6.Treatment of the Distal Femur Fracture with Retrograde Intramedullary Nailing.

PURPOSE: To evaluate the results and complications of the retrograde intramedullary nailing for the treatment of distal femur fracture. MATERIALS AND METHODS: Thirty three patients who received retrograde IM nailing for fractures of the distal femur between October 1998 to December 2003. Average age was 53.8+/-17 (17~86) years. The average follow up period was 19.4 (12~36) months. Clinical information included age, sex distribution, associated fracture and fracture was classified by AO classification. Functional result was evaluated by Schatzker's criteria. RESULTS: The most common cause of injury was traffic accident (60%). The type of fracture were 6 A1 cases, 5 A2 cases, 11 A3 cases, 5 C2 cases, 6 C3 cases by AO classification. Among the 33 cases, 15 cases were excellent, 9 good, 6 fair and 1 failure according to Schatzker's criteria. Average union time was 9.7+/-3.5 months. CONCLUSION: Treatment of distal femur fracture with retrograde intramedullary nailing was useful due to its minimal invasiveness and early range of motion, more rigid fixation.

7.Acute Pulmonary Edema During a Cesarian Section - A case report .

We recently a previously healthy parturient with ritodrine treatment who suddenly developed severe pulmonary edema during a Cesarian Section under Genera) Anesthesia, For this presented cases, we believe that overloading, Ritodrine, oxytocin, ergot derivatives wi11 be the causative factors. We had good result with PEEP, diuretics, and morphines.
