Choroid*

Polypoidal choroidal vasculopathy (PCV) is a retinal disorder characterized by aneurismal polypoidal lesions in choroidal vasculature. PCV appears to preferentially affect pigmented individuals and is considerably high among Asians. Most reports on patterns of PCV around Asia are based on a homogenous race (e.g. Chinese, Japanese) and very few descriptions from a multiracial population like those seen in Malaysia. The present study aimed to describe the demographic features, clinical and investigative characteristics of PCV in a multiracial group at Universiti Kebangsaaan Malaysia Medical Centre (UKMMC). Ninety one eyes of 86 PCV patients, comprising of Chinese (65.1%), Malays (31.4%), Indians (2.3%) and Eurasian (1.2%) were retrospectively reviewed. All underwent complete ophthalmic examination and investigations. Mean patient age was 70.4 years with a male preponderance (59.3%), and mostly unilateral presentation (94.1%). The logMAR mean presenting visual acuity was 0.78 ± 0.64. Polypoidal vascular lesions were located generally within the macula area (86.8%), manifesting mainly as submacular hemorrhage (59.3%). Interestingly a number of eyes (43.9%) had associated drusen. Optical coherence tomography largely demonstrated exudative changes (75.9%) and almost all patients (97.7%) had loss of external limiting membrane (ELM) and IS/OS interface. On indocyanine green angiography, majority of eyes had multiple polyps (82.4%) with ‘cluster’ (58.2%) being the commonest configuration. In conclusion, although the patterns of PCV in UKMMC were mainly similar to other Asian patients, a number of our patients had associated drusen. This indicates that PCV in our population could be a variant of neovascular age related macular degeneration and not solely idiopathic in nature.

One to three percent of childhood brain tumors are choroid plexus tumors. Of these, 30-40% are considered to be choroid plexus carcinoma. We report one case of choroid plexus carcinoma without parenchymal invasion, which showed rapid dissemination along whole CSF pathway and developed postoperative hydrocephalus in spite of gross total resection.

Macular coloboma is a rare atypical form of choroidal coloboma which develop secondary to intrauterine infection or developmental abnormality of the eye. These are usually oval or round with well defined margin sized from less than 1 disc diameter to 10 disc diameters. The macular coloboma was classified into pigmented type, non-pigmented type, and type associated with presence of abnormal vessels. We experienced two cases of macular coloboma, without any family history or other associated physical abnormalities. The first case showed pigmented type in the right eye and intermediated type between pigmented and non-pigmented type in the left eye and the second case showed macular coloboma with abnormal vessels.

Pneumatic retinopexy is a new procedure that is effective in treating the uncomplicated cases of rhegmatogenous retinal detachments. The procedure of pneumatic retinopexy is composed of cryotherapy(photocoagulation), intraocular gas injection and postoperative positioning. The advantages of this procedure are less tissue damage, no hospitalization, reduced expense and minimal complication. For these reasons, this procedure has become popular retinal detachment surgeons. Peports of associated complications are still limited. The authors experienced one case of choroidal detachment after intraocular gas(SF6 gas) injection in rhegmatogenous retinal detachment in pseudophakic eye.

The choroidal vascular structure in macula is anatomically different from that in other region. And choroidal venous drainage in macula represents various types. This study attempted to classify the choroidal venous drainage in macula and focused on the relationship between the choroidal venous drainage type and a disease involving macula, esp. age-related macular degeneration(ARMD). The authors performed indocyanine green(ICG) angiography in 115 eyes (93 patients) without macular disease and in 31 eyes (24 patients) with ARMD, which were presented with choroidal new vessels. The choroidal venous drainage in macula was classified as 4 types. They were superotemporal drainage, inferotemporal drainage, horizontal watershed, and irregular type. The most common type of choroidal venous drainage was horizontal watershed type which was showed in 55 eyes (48%) without macular diease and 15 eyes (48%) with ARMD. Any relationship between a type of choroidal venous, drainage and ARMD was not found.

The anterior choroidal artery has several kinds of variations. Among them, the transposition of anterior choroidal artery and posterior communicating artery origins has been extremely rare. We report a case with cerebral aneurysm arising from posterior communicating artery which origin was distal to the anterior choroidal artery and review the relevant literature.
