2.One case of abnormal high HCG level in a normal pregnancy

We reported here an abnormal high urinary HCG level (30000 frog unit/l) at 11 weeks and 5 days of pregnancy in a normal pregnant woman. Clinical signs of this patient were vomit and severe morning sickness. These signs appeared where the HCG level were still low and last until HCG level were reduced by internal therapy

3.Study on beta hCG level before molar pregnancy abortment

Using the retrospective cohort study 0f 71 patients with molar pregnancy at Hai Phong Gynecology - Obstetric Hospital from 1/1999 to 12/2000. Average beta hCG level of patients with molar pregnancy was 510.240 mUL/ml. The patients with molar pregnancy < 8 weeks had the average beta hCG 76.516 mUL/ml, although in was not specific but this level was 10 times higher than that of normal pregnancy. 4.Primary pulmonary Epithelioid Trophoblastic tumor co-existing with Choriocarcinoma

A 28-year old, G5P4 (4014), noted neck lymph nodes associated with cough. A chest X-ray was done showing a left nodular opacity. Antibiotics were prescribed with a resolution of symptoms. Five months after, a routine chest X-ray revealed interval progression in size of the lung nodule. A chest computed tomography (CT) scan and positron-emission tomography scan were done subsequently showing the precise location and size of the nodule and with no other focus of tumor seen. Transvaginal ultrasound was normal. With an initial diagnosis of lung carcinoma, a percutaneous needle aspiration biopsy under CT scan guidance was done. Immunohistochemical staining panel showed that beta-human chorionic gonadotropin (hCG) was positive. Subsequently, a serum beta-hCG done showed low levels from 33.48 to 59.7 mIU/ml. The final diagnosis given was a poorly differentiated malignancy highly suggestive of malignant trophoblastic tumor. A video-assisted left upper lobectomy was performed with histopathology and immunohistochemistry consistent with epithelioid trophoblastic tumor with co-existing choriocarcinoma elements. Postoperative beta-hCG level dropped to normal and remained so for 2½ years.

The group 1 of 75 cases of post molar evacuation and the group 2 of 292 cases of gestational trophoblastic tumors (GTT) were followed up until beta-hCG level serum, determined by IMX, reaches the threshold of < 5 IU/l, there was not any complication after 2 years. There were 7.5% recurrents cases in GTT group, in some cases, beta-hCG reaches 0 UI/l. In the recurrent group, the rate of resistance to the treatment got to 40%, and the rate of resistance to single chemotherapy by MTX got to 30%. 50% of recurrent patients have had uterine histological exams with 36% choriocarcinoma, 4.5% invasive molar

9.Some comments about monitoring serum beta hCG level at pre-evacuatin and after evacuation of molar pregnancy in the Institue for the protection of Mothers and Newborns Ha Noi

94 women with hydraform mole pregnancy were studied in the Institute of Protection of Mother and Newborn from March 1999 to March 2001. Their beta-hCG levels were evaluated weekly before and after abolition of hydatoform mole, the results were dramatically different. After abolition, the monitoring of beta-hCG curve to detect the mast blastocyte tumors reached a sensivity of 79% and a specificity of 100%. Among the group having complications, the bleeding more prolonged than the groups of no complication of mast blastocyte tumor. Beta-hCG blood level was in safe threshold.

10.Primarily noticement in the study on the Beta HCG in the ectopic pregnancy

A study on 33 patients with the ectopic pregnancy during 2-6/1997 was carried out. The Beta HCG in the peripheral blood was quantified. The results have shown that the serum concentration of Beta HCG in patients with the ectopic pregnancy always was low, the lowest concentration was 41mlIU/ml. This concentration was very different from one patient to another's and was not distributed as standard so the average concentration of HCG has not been clinical significant.
